I just turned 23, and I have expierenced hair lost for some time now. I would have to say beginning at 8th grade. The kids would make fun of me and tell me I needed rogaine. Well its obviously not stopping and my hair is not coming back. Its curly and long due to not having a hair cut in a while, and everytime I wash/condition my hair, I can see clumps come out everytime. My self esteem is way down and I have tried products before that just dont work. I even joined the Hair Club and spent lots of money for no improvement. U have any ideas/suggestions what I can do or what causes my loss so much. thanks.

This site is replete with answers to questions like yours. I sympathize with you and your problem, but you need to get to a good doctor who can work with you. Please look through this site for other suggestions, even using the search feature to target what you’re looking for. There are even comments and answers on hair loss in the younger man, many of which can be found in the Age category. You must take proper charge of finding the solution or you will continue searching aimlessly.

Propecia Stopped Working After 3 Years – Balding Blog

I have been on propecia for 3 years, now I am 24 and the effects seem to be wearing off. It has worked well to stop loss up until now. What would cause it to stop working and how can I combat this? Is there something else I can take? What about avodart?

Unfortunately, Propecia (and probably Avodart) will eventually stop working in men with an aggressive genetic hair loss predisposition. In these men, you need to have a Master Plan to determine what you might do when the drugs stop working. In many young men taking Propecia, the effects over time will decrease, but when it does, you should not stop taking it because then you will experience what I call ‘catch-up hair loss’, which means that you will lose all of the benefits of the drug over the previous years within months upon stopping it.

Some doctors who prescribe Avodart are reporting better results in men like you, but this is still no miracle cure. Even if the Avodart works where Propecia fails, the Avodart will eventually fail as well.

My Donor Area is Depleted From Past Procedures – Balding Blog

What is your opinion on ScalpMed a product being sold via infomercial that has been getting reviews? Also, assuming someone had no more donor area from having had a few procedures, but still wanted to grow more hair, what would be the most effective way to do it? I have a full head of thinning hair, barely covering the crown, and thin in the front, but with a hairline.

You’ve indicated that you are located in Beverly Hills, so it would be best for you to be evaluated in person. The depleted donor area is often heavily scarred and this would have to be assessed, balancing donor supply (if available) against need (demand). Kadus Imunal and Hair Loss – Balding Blog

Hi doctor. I tried many oral medicines such as zinc, biotine, vitamins, capsules till the level of acid increased in my stomach. So, my doctor recommended to directly stop any oral capsules for hair loss due to stomach and liver disfunction. I chose Propecia, but the results were catastrophic after 6 months of usage. A decreased sex drive with low ejaculation on a 22 years old male! I did a hormone test that shows the level of sperm and the testosterone had decreased by a massive 1/4. So why? I dont recommend this medicine that plays with a man’s hormone to any one! Recently, I am using tonic + conditioner from a German shampoo named “Kadus Imunal”. Have you heard about it doctor? It sounds very good in Germany due to its good results!

I have little to add to your comments, but I will note that this is not a common experience. I suspect that you are obsessing over your hair loss. Please search for Propecia here to find past blog entries related to this drug.

As for Kadus Imunal, it seems like just a series of shampoo and conditioners. I have no comment on their value.

Best Product for Thickening the Crown – Balding Blog

From all the information out there, I still can’t seem to find a clear and definite answer as to which products actually work in regrowing hair. I’m in my early 50’s and only have a bit of thinning going on around the crown area on top. Which product would be best for my situation? I would like to see it get thicker again in this area. Also, I’ve heard that using minoxidil may work for some, but ceasing use can actually cause more severe hair loss than before starting to use this product. Is this true?

Minoxidil works great in about 15% of people, but it must be applied twice daily. In about half of men there will be a mild effect. The older you get, the less effective Propecia is at regrowing hair, but when it works, it works well in the crown. Both of these medications must be maintained to hold their benefit and when they are stopped, the body loses all of the benefits in months and brings you back to the base line of hair loss, which is where your hair would be had you never taken the medications. For this reason, if it works, you must stay on it for life. There are dozens of other products sold with promises galore, provided you want to part with your money. Most products that are sold over the counter, if they work at all, have minoxidil in it, so I generally recommend buying minoxidil at the supermarket or Costco for far less money than when it is sold inside some other potion or lotion and not clearly marked.

A Special Letter from an NHI Patient – Hair Loss Information by Dr. William Rassman

I just received this email from a patient of mine and thought I’d share it with you, as it brought a smile to my face. Periodically, I want to share the joys of this uniquely wonderful medical practice with my blog audience as I am getting terrific feedback for many of you.

Good afternoon, Dr. Rassman. I thought this might be of help to your new transplant patients and those considering a procedure. I have been a happy client of NHI since 1999. Your organization actually turned me away prior to that until you thought I would receive sufficient benefit.

In September of 2004 you yourself did a whopping 1400 or so in the crown area (your organization did 1200 grafts prior) . Now this is the reason I am writing to you (in addition to just finding this blog business on your website): your prospective patients, especially the younger guys, need to know that balding is a long process. Although they may awake to the process all at once with the “bell in the brain” event, it is likely that the process has been in place for sometime and will likely continue. In my case technology and my finances came together and I was able to get ahead so that I never had a bald spot period in my life and managed to keep the “shine through” months to a minimum. To this day it is a hoot to tell new friends or business aqaintances that I’ve had transplants and would have a spectacular bald spot had I not chosen NHI. None, and I repeat none, believe me. Even when I show them evidence of the donor scar most assume I am pulling their leg. Now, for the second reason for writing after reading a number of your blogs. Last week one and a half years after I started the process, I was lolling in the tub and discovered a veritable thicket of new stumps in the thinest area of my swirl. I made a mental genuflection to LA and hollered to the wife: blog the good doctor and tell him that, as in the previous two sessions, his theory of waves of hair growth continues to be proven.

Sorry about the length of email but continue to reassure your younger patients with confidence that it takes time to grow. My only regret continues to be that in never having gone bald I never got to qualify for your “before and after” photos. However, my dream may yet come true. At the advanced age of 57 my front hairline is going. You may see me back in LA or San Jose for splotch in the front. Please let me know if you are running any “forelock specials” and be prepared to guarantee me a minimum half-page pic in your next updated booklet and I’m there. Remember, there are many other hair docs who would benefit greatly from claiming your good work and artistry for their own were I to rent out to them my forehead and crown as an advert. Perhaps I should hold out for a 3/4 page or maybe an inside back cover. Thank you again for your time and attention to my skull. My wife sends her regards and hints that she needs another free shopping day so she’s hoping the front falls out. I’ll remind you that it was an inappropriate comment you made during my last session that all the other wives who drive their husbands get five free hours at the mall with the husband’s credit card. Slowly Losing My Transplanted Hair – Hair Loss Information – Balding Blog

Hi Dr. Rassman,

I am a 49 year old man. I had a transplant of 1800 hairs about 4 years ago. One year after the transplant, I was very pleased with the result. In fact, it looked so good that I resisted the temptation to do a second “fill-in” procedure. Everyone commented on how great it looked.

But since then, I have been very slowly losing my transplanted hair. It watch it very closely and I see the individual hairs miniaturize and then finally disappear. I estimate that I have lost 25 to 30 percent of the transplant. I would like to get another transplant but feel like there is no point if the hair will eventually be lost.

I have an excellent diet and health and also take Propecia. I was led to believe that I would keep my transplanted hair forever…

Could you give me some advice?

Most men who report this are actually seeing the original hair population drop out, not the transplanted hair. Losing transplants over time is rare, but if it is really transplanted hair being lost, the miniaturization will be evident in your existing donor area. By mapping over the donor area for miniaturization, that diagnosis will be evident. See our article referring to Diffuse Unpatterned Alopecia, which states:

The diffuse androgenetic alopecias, that we will refer to simply as DA and which consist of Diffuse Patterned Alopecia (DPA) and Diffuse Unpatterned Alopecia (DUPA), can be further divided into various stages of progression with DA 0 representing the pre-balding state. The earliest stage of loss, in these diffuse alopecias, DA 1 , would be characterized clinically by a “slightly thinning” look in the front, top, and vertex, best visualized under strong lights or when the hair is wet. At other times it might not be noticeable. Almost all patients have subjective complaints of less fullness in the affected areas. Preliminary densitometry studies in the thinning area of these patients have revealed miniaturization to be in the range of 20-50%. DA 2 represents the stage when there is obvious thinning evident under normal lighting, but if the hair is styled properly, the degree of hair loss may be acceptable. Miniaturization for DA 2 is on the order of 50-75%. Patients with DA 3 have significant hair loss in the transplanted area, and the coverage it provides is no longer adequate. The frontal hairline, though still recognizable in it’s position as the mature hairline, does not have enough density to frame the face. Miniaturization in DA 3 is usually around 75-95%. The DA 4 patient has lost most of his terminal hair in the balding area (miniaturization > 95%), generally has involvement of the crown, and is similar in appearance to the typical Norwood Class VII.

We feel that the early identification of the diffuse alopecia patient is important in order to screen out those who will not be surgical candidates. In addition, because the diffuse alopecias are often associated with a rather rapid progression through the four stages described, the young patient is often not emotionally prepared for this degree of hair loss, especially with DUPA. Long-term planning and careful patient counseling is, therefore, critical before any restoration should be considered. We have not yet determined the exact incidence of the diffuse androgenetic alopecias in the general male population, but they appear to be significantly more common than are the Norwood Class A’s.

It is possible that in many instances diffuse, unpatterned alopecia is not a true “androgenetic” alopecia at all but actually represents a similar pathophysiology to what has been termed “senile alopecia,” 2 only occurring at a younger age. These authors evaluate the donor area for a miniaturization in every person consulted in our office for hair loss. We believe that some degree of clinically significant diffuse androgenetic alopecia occurs in a substantial number of men as they age. We have observed this in men as young as 17. Regardless of what the actual pathophysiology might prove to be, these authors believe that it is important to make a quantitative assessment of miniaturization , using densitometry, when evaluating each patient so that the physician can more accurately determine the total available supply of stable donor hair.

Do Balding Celebrities Take Propecia? – Hair Loss Information – Balding Blog

I am a 33yr old male. I first experienced hair loss at age 23. At age 25, Propecia was available on the market so I tried it. I got good results but decided after going back and forth a few times that I did not want to have to get a prescription and take a pill every day. Is there any other proven remedy? I notice celebrities such as Tiger Woods, Matthew McConaughey, Eric Lindros, and Bruce Springsteen to name a few, reverse their balding trend. Help me make the most informed decision please.

Propecia will help reverse some of your hair loss at a young age. Stopping it is not a wise decision. Although Propecia is a wonderful drug, I believe that this drug has its limits and will not reverse balding that is stable like a hair transplant will. There are many celebrities that I know of personally who have had multiple hair transplants and continue to take Propecia to prevent further hair loss. I only wish they would let me use their names, however I respect their wishes and take patient confidentiality very seriously.

Sexual Side Effects from Minoxidil? – Balding Blog

I have used the 5% Minoxidil solution for nearly two years as directed, ie., twice a day. While it seems to have worked with my hair loss, it seems that my sex drive has waned. Also, when I do, it seems erections aren’t what they were before. I am 46 and in otherwise good health.

Are there any indications that long term use of Minoxidil (like I have used it) has any effect on either a male’s sex drive or erectial function? Thanks in advance.

There are no known sexual side effects from topical minoxidil (also known as Rogaine). This is a drug that has been around since the 1970s (it was originally a medication for high blood pressure) so its safety and side effect profile are pretty well known – even for patients who have been on it for decades. It does work to strengthen hair growth.

There are several other possible causes for loss of libido not least of which is the pervasive rumor that “hair loss medications” cause decreased sex drive. While decreased libido/erectile dysfuntion is a rare (+/- one percent) and temporary (1-3 days usually) side effect of Propecia, it does not occur with minoxidil use to our knowledge.

Finally, (for the rest of you readers out there) it bears repeating that human beings are highly suggestable – if you tell someone, man OR woman, that they might possibly have a side effect or if they even just THINK that they will have it, they will often manifest it for you. (It is often joked that this goes double for guys when it comes to their manly functions but both sexes will have this effect.)

Back to the original writer of this blog entry – this is not to say your symptoms are not real – I believe they are. If you persist in having significant symptoms of decreased sex drive or erectile dysfunction, I’d HIGHLY recommend you talk about it with your doctor. In the meantime, I can reassure you that you may continue to use the minoxidil without fear that it is causing your symptoms.

Radiation Therapy in Youth Caused Current-Day Hair Loss? – Hair Loss Information by Dr. William Rassman

Greetings,
I just learned that my husband had been diagnosed as a child with cancer in one of his testicles. He underwent radiation and had lost hair on his head. Could this radiation have caused some permanent hair loss on his head? He is bald at the crown.

It is not clear if he received radiation to the head. Assuming that his radiation was more in the groin, then I doubt that it is producing hair loss. He needs to get a working diagnosis with mapping of his scalp for miniaturization. Treating the balding crown has some good solutions for him but first he needs to get a working diagnosis.
