Hi Dr, I have been on Propecia and Rogaine for about 12 yrs (41 now) and have had very good results when compared to my brothers who did nothing and lost quite a bit. However this last 18 months I have experienced a general shedding and the top of my head and hairline have been hit especially hard.

I have spent a lot of time in Central America for work and am wondering if some change in my environment may have caused it. Do you have a lab or contact in SoCal that you recommend where I could have a hair shaft analysis to see if there is anything other than plain old MPB going on?

Why Would Shock Loss Cause Permanent Hair Loss? – Balding Blog

Why does the hair that you lose from shock loss not regrow? I asked my dermatologist about my shock loss and he said he expects that the hair will grow back and that it would be unlikely that the scalp would just stop working and stop growing hair because of the surgery.

Nobody really knows for sure. Shock loss in a man with genetic hair loss often just accelerates the hair loss process. In women, a telogen (sleep) phase occurs and the hair almost always grows back. In men, it is thought that hair lost from “shock” after a hair transplant surgery is mainly the weaker miniaturized hairs that were already predestined to fall out as part of the male pattern balding. The “shock” from the surgery would cause these hairs to fall out earlier than normal just as any stressful event (even an appendectomy). Thus Propecia (finasteride 1mg) has a protective effect on preventing some hair loss from “shock” loss in miniaturized hairs. This is the reason why younger men who are actively losing lots of hair are at an increased risk for shock loss and must use finasteride to protect themselves from the awful complication.

I don’t know whether or not you’d be interested in this message. Nevertheless, I would be interested in your reaction to it …

I was a patient (age 34 at the time) of yours in August of ‘94. You transplanted about 2,000 grafts from the back of my scalp to the top of my head. My overall reaction was that the density wasn’t near enough. And there was no way I could afford a second trip through the mill.

While I thought the newly transplanted hairs themselves looked okay, I concluded that your procedure fell far short of my expectation. To be blunt, my head resembled a coconut. So I ended up going to a company that provides rather costly “hair systems” (the gentle term for wigs). They look great – full and natural. It’s an ongoing “expense” that’s become a part of my life. But I thought of you the other day.

The idea of finally just shaving my head and being done with the high cost of masking my hair loss crossed my mind again. So when I took another good look at the white scar that traverses the back of my head from ear to ear, I dropped the shaven head idea.

Maybe when head tattoos are commonplace, I’ll put one right over the scar; a picture of a “bald eagle” or something.

Thank you for contacting me. I had the opportunity to review your photographs and your medical record. Hair loss is a progressive process and it is clear that the communication between us failed. I care about my patients meeting their expectations (first being sure that these expectations are realistic) and following up with a good Master Plan to follow the hair loss over time. I did this in 1994 as well. A single hair transplant procedure of 2000 grafts may have filled in some of your thinning, but it was not enough to cover the complete hair loss pattern that probably evolved. As hair loss is a progressive process and your pictures showed that you were in the process of losing hair at the time of the transplant, clearly you were a moving target. I must have failed to explain that process to you. Drugs like Propecia (finasteride) were not around in 1994, so unfortunately, in those days, I worked with a handicap watching helplessly as people lost their valuable hair over time. As you may know by experience, hair transplants (without the protective effects of drugs like Propecia) can accelerate the hair loss and even produce shock loss, which means that the hair loss process probably accelerated.

I can suggest that you arrange a visit with me and let’s discuss your present status and your goals. There have been many advances in managing scars if that is your main focus today. Any Dermatologist Will Prescribe Avodart! – Hair Loss Information – Balding Blog

Well I am 23 years old and last October I went on Propecia 1MG per day.

This past February I had a hair transplant and now six months later, I just got a prescription for Avodart.

I don’t know what all the hoopla was about getting a prescription for avodart. Any dermatologist who gives Propecia will give Avodart. She said “As long as you know it is offbrand and FDA has not approved it for hair loss, then I will prescribe it.” She also said how most acne medications are offbrand, such as Tetracycline and Minocycline. They are antibiotics meant for bacteria, not acne, but dermatologists prescribe it, just like they prescribe avodart for HAIR LOSS. You dont need to go lie to a doct to get Avodart, just tell them for hair loss and they will warn you but still give you the prescription.

Using Avodart (dutasteride) for hair loss is off-label, which means it is not approved for the treatment for hair loss, but it should not be frivolously compared to using Tetracycline off-label for acne. Avodart is a medication for prostate disease and it lasts many weeks to months in your body long after you stop taking it. For young men to take such medication, there is no data establishing its safety and I have had reports from patients who are taking this medication that sexual side effects (decreased libido and erectile dysfunction) does occur. In patients who took Propecia (finasteride) and then switched to dutasteride, there are reports of more negative sexual side effects. Also keep in mind that using Avodart does not necessarily mean it will work better than Propecia since it works in similar ways of blocking DHT. In other words, blocking DHT is not the end all cure all for hair loss. You need to be well informed about your choices and implications of medication use.

In the end, I have no problem prescribing Avodart as long as my patients are well informed, but I tend to generally dissuade patients from using Avodart because I have not seen a dramatic difference between its uses compared to finasteride 1mg (Propecia) in most cases. There are some cases where the use may be better, but it is a gamble for both doctor and patient.

Another option is hair transplantation.

I have had 3 treatments. I went off of Propecia 2 yrs. ago because I traveled a lot and worked outdoors and it just wasn’t conducive to my lifestyle. My hair has thinned significantly since then. I am thinning allover on top and I am balding in the back. I have noticeable incisions from my donor areas and now the scars show.

You mentioned that you have stopped Propecia 2 years ago. Whatever benefits you might have achieved may be lost, permanently. You can try it again, and you may sill pick up some benefit. I am not clear what scars you are talking about. Hair loss in the donor area (assuming that you had a series of transplants) should not be impacted by stopping Propecia.

I have always heard you can slow the rate of ones hair loss, (especially if the pattern is the infamous “horsehoe”), but you can’t stop it? This has come from a couple of Dermatologists, M.D.. They paint a bleak picture, why would they be so negative, if it weren’t true?

Yes it is true. It can be bleak. Drugs such as Propecia (finasteride) and Rogaine (minoxidil) do not stop hair loss completely and forever. Sometimes the drugs can grow back some hair, but sooner or later the hair loss will continue (albeit at a slower rate).

Can Surrounding Hairs Become Minoxidil Dependent? – Balding Blog

Do you think these claims are true?

“There is still a question as to whether hair that was already there, meaning hair that had not thinned previous to using minoxidil, becomes dependant on the drug. Many speculate that the surrounding hair can, indeed, become dependant on minoxidil, thus discontinuing its use can lead to harming that hair as well as any hair grown as a result of application.”

I’m thinking about minoxidil but don’t want to damage “good hair” in the process. Thanks for your time!

Minoxidil will not benefit (or impact) normal non-balding hair, so if you are treating an area where thinning is occurring, the normal hair around the balding area will not be affected by this drug. Minoxidil can postpone hair loss in the hair follicles that are miniaturized. Discontinuing minoxidil (Rogaine) may cause accelerated loss of those hairs which are miniaturized.
