My previous FUT surgery failed, should I do FUE now? I am a female.

I think that you are focusing on the wrong issue. First, you want to make sure that you don’t have some condition that will kill off your hair like Frontal Fibrosing Alopecia or related entities. Assuming that you do not, and you said you were disappointed with the results of your last surgery as some of the grafts failed to grow, it may be likely that the problem was with the surgical team (including the surgeon who is captain of the ship). Both FUE and FUT in the hands of experienced and well disciplined surgeons and teams, work reliably. As a female, an FUT is a better option for two reasons (1) you don’t have to shave the back of your head and (2) you had on FUT before so the surgeon can work in the same place and if there is a scar there, the surgeon can remove it at the time of the next surgery. This time get the right surgeon to do the right job, with FUT or if you are willing to shave the back of your head, then FUE works well.

Preventing shock loss in Hair Transplant Surgery

The only way to prevent shock loss (the loss of your native hair) following a hair transplant is to take the drug finasteride prior to the transplant and then after it for a year or more. When this happens in women, there is no known way to prevent shock loss as the mechanism is different

Preventing hair loss after a hair transplant

I have MPB which has hairs receding from temple region.I went to a dermatologist today and she said even if you do hair transplant, the hair in the middle section of your head will continue to fall. So she said take Finastride 1mg every day for 3 months along with mintop 10% twice a day everyday and vitamin 3 tablets. Also she said she would do PRP once a month for 3 months which will most probably bring some hair back in the temple region.

I would like some opinion on the medications that she has given and the PRP. Please help.

When it comes time for a hair transplant, the drug finasteride will usually protect the native hair including the miniaturized hairs on the head. As far as PRP, I have written about it here: https://baldingblog.com/go-looking-prp-used-hair-transplant-good-thing/. Minoxidil is a good medication, but 10% concentrations tends to produce more side effects than the traditional 5% and 10% it is not a standard dose.

Preventative hair transplant

25 yo. I’m thinking of getting a hair transplant in the next 7 months or so. I will likely need another one after that. My crown is very thin ( can see scalp) and my hairline has receded. I still have quite a bit of hair tho.

I was wondering can one get a preventative hair transplant in areas where you’re likely to lose hair in the future ( transplanting hairs in areas where there’s hair but it’s less dense).

I am strongly against a preventive hair transplant because of this: https://baldingblog.com/need-master-plan-think-hair-transplants-photos/ A good Master Plan is needed before you go forward with the hair transplant process. Here is my explanation for the Master Plan: https://baldingblog.com/so-if-i-replace-my-receding-hairline-what-happens-in-another-5-10-years-when-i-continue-to-recede-answer-a-master-plan-with-a-good-doctor-who-measures-your-hair-bank/

Press Release – Rogaine Sponsored Survey Shows 66% of Balding Men Feel Like Victims

Snippet from the press release:

Survey results released today by the ROGAINE® brand, in partnership with Men’s Health, reveal the extent to which hair loss psychologically impacts men. In a survey of over 900* Men’s Health readers, one-third of men experiencing hair loss admitted to feeling “helpless” and over 66% used negative language like “victim,” “defeated” or “unavoidable” when describing the condition.

This is basically just a press release for a new marketing campaign by Rogaine with some buzzwords that caught my eye (it worked!), but I found the negative language used by the surveyed men to be interesting. Hair loss is treatable with medication if caught early enough, and other treatments such as surgery or SMP are also available.

Press Release – RepliCel Reveals Updated Early Trial Results

Snippet from the press release:

RepliCel Life Sciences Inc. is pleased to report that further analysis of results from its successful first-in-man clinical trial revealed substantial hair growth including seven participants with growth in excess of 10%, including 17.2%, 19.2% and 19.6%. Furthermore, there were no negative health effects.

“The RepliCel TS001-2009 trial was a first-in-man trial with a primary endpoint of safety and this was confirmed emphatically,” stated Dr. Rolf Hoffmann, Chief Medical Officer at RepliCel. “Even though the size of the trial was only powered for safety measures, secondary endpoints were included in interim analysis to give us an early look at efficacy to allow us to better drive the design of the next steps of the clinical development of the RepliCel(TM) procedure,” Dr. Hoffmann concluded.

These are updated results that were announced, but the release goes on to say that the trials will be continuing into August 2013, with results to be announced by the end that year.

Press Release – New Laser Cleared by FDA for Female Hair Loss

Snippet from the press release:

The United States Food and Drug Administration (FDA) has just cleared a new laser proven to regrow hair in women. The FDA ruling was based on a study performed by Dr. Grant Koher which showed that low level laser light, specifically with a precise wavelength, provides a medically significant treatment for women’s hair loss. Dr. Koher is the first physician to conduct a study resulting in 510k FDA clearance for a medical device for the treatment of female pattern baldness.

Dr. Koher conducted a three phased medical study to confirm that females with androgenetic alopecia, when exposed to scheduled applications of low level laser light, would have a reduction in hair loss and promotion in hair growth. This study was conducted over 18 months with 82 qualified participants. Each participant was scheduled to receive 52 treatments of 20 minute duration, over a period of 26 weeks. All study procedures included clinical protocol, clinical tracking, and clinical analysis approved by the Western Institutional Review Board (WIRB).

The study that is referenced was done by Dr. Koher in North Carolina, but I can’t find any information about it outside of this press release (not even on the doctor’s website). He apparently presented the study, “Effect of Laser Biostimulation In The Treatment of Female Pattern Hair Loss“, at last year’s ISHRS (International Society of Hair Restoration Surgery) meeting, but I can’t find any evidence that it was published anywhere.

Note that it is not FDA approved, but it is “cleared”. When the FDA clears something like this technology, it means that it is safe… not necessarily effective. For more information on 510(k) clearance, see here.

Press Release — ISHRS on Sexual Side Effects of Propecia

Press release from ISHRS:

International Society Of Hair Restoration Surgery Maintains Safety And Efficacy Of Propecia (Finasteride 1mg) For Male Hair Loss Is Backed By Proven Scientific Data

According to the International Society of Hair Restoration Surgery (ISHRS) – the world’s leading medical authority on hair loss and hair restoration – recent anecdotal reports of persistent sexual dysfunction by some men who have used finasteride 1mg (Propecia) to treat hair loss should not define the safety and effectiveness of this drug. Propecia is the only oral medication approved by the U.S. Food and Drug Administration (FDA) for the treatment of hair loss in men.

From a scientific standpoint, there are no evidence-based data substantiating the link between finasteride and persistent sexual side effects after discontinued use of the drug in numerous, double blinded, placebo controlled studies conducted evaluating the use of Propecia 1mg for hair loss.

“As one of the 900 physician members of the ISHRS, the health and well being of our patients is our utmost concern – whether prescribing medications like Propecia or performing hair restoration surgery,we have treated hundreds of men suffering from hair loss with finasteride 1mg with virtually no side effects.”

Since receiving FDA approval in December of 1997, 20.5 million and 6.7 million patient-years of exposure using Proscar and Propecia respectively are recorded with a low adverse event profile. The ISHRS believes that these are the most current and reliable data available until further studies are conducted.

Sexual dysfunction is a very complex disorder, and its incidence in the general population is known to be quite high – as high as 49% in middle-aged and older men according to one study published in 2006 in the Archives of Internal Medicine. That being said, we owe it to our patients to address valid concerns in a scientific manner and to clarify any misleading reports so that men can make informed choices regarding the use of this medication.

The ISHRS has called on the medical communities of dermatologists, hair loss physicians, urologists, endocrinologists and sexual medicine specialists to join in a colloquium to share all data and experiences with finasteride in a fact-based manner.

The above text is a press release sent to ISHRS member physicians to send out with individual doctors names attached. We’re posting it without the personalization, but we agree with what they’re saying about the lack of evidence-based data when it comes to persistent sexual side effects from Propecia.
