Painful, Clumpy Ejaculations After Starting Propecia

Hi there-

I’ve surfed through past entries, and was unable to find a post specific to the problem I’m having.

I began noticing the signs of MPB in my late teens, and a more prolific loss in my early twenties. I’m 24 years old now, and have been on a Propecia/Rogaine regiment since visiting a dermotologist last year.

Within two weeks of consistent Propecia use, I began to have painful ejaculations and the consistency of my semen became clumpy/very gelatenous. I was told that sexual side effects usually subside with continued use, and indeed, ejaculation is no longer painful. My semen consistency is mostly normal now, but it occasionally reverts back to that of the initial months of taking the medication.

Ever heard of a similar experience?

I have been aware that the consistency of the semen changes in some men, but I have not heard about painful ejaculation. Talk to your prescribing doctor about this issue.

Pain in Left Testicle while on Finasteride

Is this a common symptom because of finasteride? It also aches a lot when I lift heavy in the gym, especially when doing squats. My left testicle also got bigger than my right testicle, is this normal? My right testicle doesn’t ache at all, only left.

Pain in a testicle can be from many causes, and if it is swollen it would worry me even more. You should see a doctor as some of the causes can be threatening to the testicle.

Pain in Donor Area One Day After Transplant

Dear Dr. Rassman,
I am one day post-op from my first hair transplant. Right now, the only problem that I’m having is pain in the donor area. Can I sleep normally at night without disturbing the recently placed grafts? They told me to rest sitting up, but this is uncomfortable for me. Thanks.

Sleeping normally will work after a day or two post transplant. This information is something that should have been covered. Did you receive written instructions? By the time you read the post, you should be better. I always wonder why someone would ask me (a doctor they have not met) rather important questions when they have a doctor that they trusted to perform the surgery. I do appreciate the question, but your doctor should outline all of these issues for you.

Pain from FUE

Hi I’m getting this horrible pain after FUE. I know its been some time please let me know how long does this pain last its been 4 Weeks now I am still with burning sensation and pressure pain all around my donor area and face

You should go back and speak with your doctor. Pain following an FUE might be the result of damage to the many small nerves around the scalp (there are thousands of them) so many are cut by either the FUE punch or the needles used to make the recipient area. The good news is that the pain you are reporting almost always goes away in week. A few people might see the pain a couple of months of so as it does take time for the nerves to heal.

Correct me if I am wrong. In this article that someone posted to you it states that finasteride lowers T levels(testosterone) as well.

Here is my question. I thought finasteride lowers DHT and increases testosterone by about 15% or something. Does finasteride lower testosterone as well? And how would this affect building muscle and body building?

Propecia slightly raises testosterone initially but eventually your hormone levels should return to normal. The reports in the literature seems to go both ways. Some reports indicate a rise in Testosterone while others show a decrease. Unless you are a biochemist/pharmacist doing research, I do not think these are significant issues. Propecia will not make you into Arnold Schwarzenegger or Urkle.

You are trying to make sense and correlate biochemical hormonal variation with clinical issues that may not be related. If you are asking your question for pure scientific purposes, I would take a course a biochemistry for a better understanding or speak to a research pharmacist/biochemist. My desire to revisit my biochemistry days and contemplate professorial issues are not really there.

I do occasionally run into men who are heavily into weight lifting and body building and use anabolic steroids. One of them was convinced that Propecia made them weaker and gave them a disadvantage. A few have insisted taking Propecia to counteract the hair loss from their anabolic steroid use and didn’t report any changes in strength or muscle mass. But these are all anecdotal (non-scientific) and subjective stories. In my humble opinion, Propecia has negligible impact on testosterone and strength. Even the professional world of sports have taken the position that it is not a performance enhancing drug.

I loved yesterday’s question on the smelly hair system. It does not even compare to my problem. I have thinning hair at my temples. I am in the Army and have to wear my hair up everyday while at work. My hairstylist says that my hair will grow back once I stop putting it up. If your hair dresser is correct and you have traction alopecia (hair loss from traction), then you can expect little relief. Whatever is causing the hair loss must be addressed by stopping the pulling. There are other conditions, such as triangular alopecia, where people lose hair in the temples from genetic causes. This is part of the differential diagnosis that you needs to have made. See a good doctor and discuss these two conditions with him/her.

Procerin and Curetage – Hair Loss Information – Balding Blog

I just turned 20 years old and for a year now my hairline has receeded and started to thin dramaticly. Which I am very self-conscious of. I have been taking a natural suppliment called Procerin and using a daily treatment called Curetage for a little over a month and a half, but have noticed no difference. Do external treatments work, and pills work? Or are they just a waste of time? I also heard that hair transplants can’t be done on young men, is this true? Is there anything else i can do to make my hair grow back?

I have written about Procerin before (see past blog entries: Procerin and Procerin Info). The only topical treatment that seems to work is Minoxidil. Many add compounds to Minoxidil to try to make it more effective (e.g. Retin-A, which adds irritation to the skin and some burn to it as well) or combine it with other ‘magical’ formulations to sell it at a high price to take advantage of the value of its marketing label. The best and only real treatment for you is Propecia. There are many things done for hair loss and many people making money from the panic of hair loss victims. ‘Curetage’ sounds like one of them.

Stick with what works and do not risk permanent hair loss to go outside the known effective treatments that are available. Get a good doctor to work with you.

I am a 25 year old woman and I have been losing my hair since the age of 12. All of the women in my family have been affected, but I am the first to lose it as such a young age and actually have it the worst. Taking Propecia While Trying to Have a Baby – Hair Loss Information by Dr. William Rassman

I have been taking Propecia for about 8 years. My wife and I are anxious to have another child. I completely stopped taking Propecia for about 5 months when my wife and I started trying to have our first child two years ago as I was afraid of side affects. He is perfectly healthy. Was that necessary or advisable? Should I stop taking it while we try to conceive again? Thank you.

There are many answers to this question depending upon the doctor and his interest in practicing and giving advice on what could be sensitive medical legal issues which are at time at odds with what is in your best interests. Very little, if any, Propecia comes from the sperm into the woman’s womb. Merck (the manufacturer of Propecia) does not warn men wishing to have children to stop taking the medications for your situation. That, in itself, tells much of the answer to your question. Some doctors, however, tell their patients if they are worried to do the following:

  1. stop taking the Propecia during the one week that fertility is present or stop it through the entire period from conception to the delivery of the baby
  2. use a condom when your wife is pregnant
  3. stop the drug completely during the entire pregnancy

I feel that the first part of #1 may be wise if you do not trust the drug company disclosures and to adopt #2 if you are really worried. These two steps are extreme, while #3 is almost crazy because you will abandon all of the benefits of the drug and get progressive hair loss during the 9 month pregnancy period, based upon no evidence that this is a problem.
