I get watery semen with Saw Palmetto

I get watery semen every time I take 320mg SP extract. I reproduced this issue a dozen times. Doesn’t that suggest it blocks DHT significantly?

I have never recommended Saw Palmetto, so I have no personal experience with it other than what I read. It does sound like a DHT blocking effect.

I Found this List of What I Can and Can’t Do After Surgery…

Hi Dr. R!

Captain Forehead here. I’m one week post-op. Staples are good so far. Not too much discomfort.

Wow, there is SO much different info online about what you can and can’t do after surgery. I found this elsewhere on baldingblog.com, a very helpful guide.

Patients…

* Can lie flat
* Can have full aerobic exercise in 1 week (even run a marathon if they want)
* Should limit weightlifting greater than 25 lbs for 4 weeks and keep the back straight
* No sit-ups or pull-ups for 4 weeks
* May play with pets in 1 day (but no licking the transplants by your dog until the crusts are off – it had to be said)
* Smoke and dust in the environment are not a problem unless you are not clean
* Passive sex is ok for the first 2 nights, then everything after is a go

It’s just weird ’cause sometimes I feel more tension while yawning than I do after the pushup I just tried, of course while keeping my head totally back!

I guess I’m just writing to say, I wish I knew for sure how “zombie-like” I have to remain after surgery and how much of my history of widened scars is due to junky genetics.

Thoughts?

P.S. See ya at the Open House in L.A. this Saturday!

You don’t have to be a zombie after a hair transplant surgery. The precautions are so that it protects and minimized the donor area scarring and most of the rules are to protect the people who are not generally careful and do unsafe things. I am generally open to shortening all of the time lines with more careful people. To simply think about it, the more tension you put (sit-ups, etc), the more there is a risk of widened scarring when your neck muscles are pulled. You will know that if you put your hand on the back of the neck and see how much pull you get from what you are doing.

I found an alternative to SMP.

I just tanned my head haha.

Tanning works like anything that changes the contrast between hair and skin color. The darker your skin with dark hair, the less the contrast between hair and skin color. I often recommend that men with very white skin get a tan. Dying it blonde works even better for the white guys, maybe not for darker haired men. SMP also changes the contrast between hair and skin color, see here: https://scalpmicropigmentation.com/

I Found a Lump Under My Nipple After Taking Propecia

I was on Propecia for a year after about 9 months I got a lump under my left nipple. I also had to work really hard to get an erection. I stopped taking the drug and all sides went back to normal and I have just become a father so no damage done. I was taking 1 pill per day I would like to start taking the drug again as I was having good results. is it possible for me to lower the dosage and will this reduce the sides. I look forward to hearing from you thank you and keep up the good work

If you have or had breast lumps, this can be a serious issue and you need to have a physical exam. Your doctor needs to be aware of it. If you have erectile dysfunction you need to at least see a doctor to make sure there are no other medical issues such as high blood pressure, diabetes, or other medical conditions that may contribute to the issue. I realize this site is for discussing hair loss issues and Propecia is a medication that may cause the side effects you are describing, but nevertheless you still need to see your doctor about these issues.

I am glad to hear that your side effects all went away. If you want to restart Propecia, talk to your doctor regarding a possible dosage change that may diminish or ameliorate the side effects. Remember that 1/2 of a Propecia (0.5mg finasteride) is considered 70 to 80% as effective as the full 1mg dose in treating genetic MPB.

I Forgot to Bring My Propecia When I Was Out of Town for a Weekend

Doctor,

I’ve been on propecia daily for 9 months. however, I went to Spain for the weekend and forgot my pills. 48 hours+ from one pill to another. Will this alter its effectiveness in anyway?

Thanks

There is enough Propecia fixed to your hair follicles, which makes the 2 or 3 days you skipped OK. Just take it as normal when you’re back home.

I Feel Tingling on My Scalp and I Think I’ll End Up a Norwood Class 6

Hi Dr Rassman

Im a 18-year old boy having male patterned hairloss. I started balding when i was 16 and am a full norwood 3. Since the last year a have felt some tingling feeling when i run my hand trough my head, all of it except on the sides. I dont feel pain nor do i have some disease of what i know but just the tingling feeling. Its like i know what my hairloss pattern will be a (norwood 6) wich i can tell from the feeling.I have looked trough the internet to see if anyone has experienced the same thing and interestingly alot of people have. I was wondering if you heard about this? And if you have please tell

Tingling on the scalp does not mean you will lose hair. While people who are balding may experience these symptoms, people who are NOT balding also experience these symptoms (they just don’t write about on the Internet hair groups). You may consider dandruff, head lice, scabies, or other conditions (not related to hair loss). Or you may having nothing at all! I really can’t say what you are experiencing, but you may want to see a doctor if the problem persists.

I Feel Terrible Pain When I Lose Hair

Im a 20 year old male, and Ive noticed about 3 months ago ive started to shed my hair along my frontal hairline as well as my scalp. The thinning is gradual, but I can tell that the more I mess with it, touch it, shampoo or comb it it gets much worse. My concern isnt going going bald, my concern is that I feel terrible pains when I do in fact lose hair. It feels as if someone is yanking on my head. Also, afterwards my hair feels very oily, and has a smell to it. Is this normal shedding? Im concerned this isnt male pattern baldness Im being faced with. Thanks in advance for your time, dedication and help.

I’m not making light of your pain, but it sounds like you are overly obsessed with your hair. The pain you feel may be from the emotional roller coaster that you are riding. Do you have someone close that you can talk with? You should try to get some emotional counseling.

Many men experience pain when they lose control over their lives or their health. Hair has been known to reflect such a loss of control. One patient told me, “I have control over everything in my life, what I do, how I feel, how much to work out, who I am friendly with and who become my friends. When it comes to my hair, I know that I have no control over it.”

I Feel Stubble on My Scalp — Is Hair Possibly Regrowing?

Hi Dr,
I just posted a question to u before realizing to ask your advice on another topic. I’m 29 and have been taking propecia and minoxidil for the past year. I have noticed that some of the hair that i am losing is short but not necessarily finer than other hair. Also when i run my fingers over my scalp, i can feel a fair amount of stubble. Is this hair that is growing back or just a part of the hair loss process with hair falling out and growing back with a shorter life than before? If propecia and minoxidil is working for me then i shouldnt see the shorter hairs (about one inch long) falling out right?

Thanks for your advice on this difficult to understand topic!

Unfortunately, I can not comment on your observations just based on what you wrote — I’d need to examine your scalp. What I generally do is examine the hair with a video microscope right where it is growing and then I can tell a great deal about the hair you are talking about. I know that when I had hair transplants, I felt the new hair breaking through, so I have some personal feelings that what you are reporting may be new hair.

With regard to your questions/comments on hair cycling, we know that minoxidil does impact the hair by increasing length of the hair growth phase, but these two medications (Propecia/Minoxidil) may kick in the telogen cycle when it is first started up, making the hair cycle shorter. There are many things that occur (possibly at the same time in some people), but I don’t know if you can draw any conclusions from a year’s experience with these drugs.

I failed to get the scabs off of my recipient area

I was wondering, I looked at a post on newhair.com about two post operative examples where they had minimal to no scabbing/crusts. After my hair transplant I was instructed to wait 24 hours before washing, I washed gently by pouring baby shampoo mixed with water over my head. I did this daily. Now at 8 days post-op, I have quiet a bit of scabs still. The scabs that fell off also shed hairs. I was reading on Wikipedia an article about hair transplant and it stated “Scabs adhere to the hair shaft and increase the risk of losing newly transplanted hair follicles during the first 7 to 10 days post-op.” This has me concerned, as I have bald spots throughout the hair transplant and was shedding scabs as early as day 4. I read through your posts and you had mentioned that you try to reduce scabs as much as possible. Will my prolonged scabbing at day 8 effect my outcome? I washed my hair 3 times today and let it soak but the scabs are still not falling off yet. And as I mentioned, there are bald patches where the scabs came off with the hair.

The key is to wash it the very next day and use a surgical sponge filled with shampoo, rolling the sponge over the recipient area over and over again until all of the crusts are off. Pouring soapy water alone will not work. The Wikipedia report came from a medical article I wrote which you can read here: https://newhair.com/wp-content/uploads/2018/11/mp-2006-graft-anchoring.pdf

Below is an example of what a post operative patient should look like one day after surgery (2150 grafts) with good washing techniques as discussed above. If he keeps washing his graft area, the terrible crusting seen without washing will never develop.
