I have seen many people who had their hair pulled out. If it is a single pull, it will come back in about 6 months. In contrast, repeated pulls can cause traction alopecia.

This patient came in last week, a few years after having 709 grafts taken from a very depleted donor area. Half of the grafts contained just one hair. Getting those 709 grafts was a challenge, as his scalp was tight and the donor area heavily scarred. Because of the anticipated poor yield, I elected to spread the grafts over an inch from the plugs (forward) creating a loosely built thin hairline. When he allows his hair to stand up rather than pull it back as shown here, the results are even more spectacular. This patient gave up his wig and is thrilled with his result. We will try to do another procedure with up to 700 grafts in the recipient area to thicken it, but that will be even more of a challenge because the scarring is very bad and the density of the donor area is very low and he knows it.
Many of what I will call the “normal transplant patients” we perform surgery on today who have not had the old type of plugs, may look at pictures like those shown here and get very frightened. I understand the fear that gets invoked by these patients, but some of my most satisfied patients are those who walked around for years and found that people never looked into their eyes, only to their hairline. This man is an example of a patient where just a small change had a major impact on his life, allowing him freedom from a wig he despised to cover a pluggy look he hated.

Note the thick hairs and the many very tiny and thin hairs throughout the viewing field. This is full blown miniaturiation which is best treated with finasteride in hope the miniaturization will reverse. This is more probable in men in their teens or early 2os. The long term question that we don’t know is if the hair does all growS out, how long will it last? The experience is minimal for follow-up of this treatment in men, but I would guess, based upon human skin transplanted in mice with dormant human hair and microneedling, that the hair will last one anagen cycle and then fall out (an anagen cycle in a young man is about 2-3 years on average). At that point the treatment would have to start again, or alternatively a hair transplant may become an option if a restart of the treatment doesn’t work.

I am on finasteride for 6 months and all of my miniaturized hairs turned in to thicker terminal hairs except temple hairs.,Why is that? Will topical finasteride or minoxidil helps in this situation?
Finasteride does not work on all miniaturized hairs. Sometimes the genes are too strong for the drug and the drug doesn’t impact it. Maybe, Topical Finasteride with minoxidil will help.

Yes, you are an excellent candidate for a hair transplant. I classify you as a Norwood Class 3A pattern with a residual forelock. This forelock will eventually disappear as you age so creating a hairline for someone like you will give you a much younger look as the man with the before and after photos below.
