Is Dupa caused by classic genetic balding?

Maybe. There is no obvious genetic link to the Classic DUPA (Diffuse Unpatterned Alopecia) we originally reported in the Journals. I have, in some patients, seen genetic balding also present (not that common) and when I do, I warn these patient never to have a hair transplant because it will fail.

Do You Think Propecia Will Be Available Over the Counter?

Do you think Propecia will ever be available over the counter, without a prescription? Isn’t that the next logical step now that finasteride 1mg is for sale as a generic?

I mean Rogaine is readily available at most pharmacies and even in the supermarket, and many other OTC medications have much higher risks of side effects than Propecia does. Thoughts?

I doubt Propecia will be available over the counter. It is a medication that really needs to be prescribed by a physician. It is only meant for the treatment of a specific kind of hair loss (androgenic alopecia) and may not be good for some women. There are many medications that remain prescription only, even though a generic is available.

Do you think MPB is playing a part in Justin Bieber’s “deep rooted issues”?

Not trying to just gossip or start drama. Just want opinions. Personally, I think it’s playing a big part. It obviously takes a large toll on all of us going through it, but imagine being someone like Justin Bieber who’s whole life was boosted up based on an attractive appearance, especially the hair. And yes, his hair could be a lot worse. But we can all see he isn’t where he used to be.

All men want hair and don’t like it when they see changes related either to age or some form of male balding. Celebrities are no different and I have seen my share of celebrities to tell you that they are like you, they care about their hair

Do You Think I Have Improved on Finasteride (Photo)

Too early to tell and not the same view. I had a young man like you where I did a HAIRCHECK test to see what degree of balding he had:https://baldingblog.com/19-year-old-parent-told-not-balding-according-haircheck/ I am following him after starting him on finasteride. If he reverses the hair loss on the drug, then he will know that he is succeeding with the treatment.

Do You Think A Better Hair Loss Drug Will Be Approved Within the Next 10 Years?

Hi,

I’ve written before and have another question. Do you think within the next ten years there will be a new hair loss drug that supplants Propecia as the go-to non-surgical option, or is FDA approved Dutasteride (for hair loss) the only new, clinically proven product you see on the horizon?

Thanks

DutasterideDutasteride (marketed as Avodart) is being used by many men, prescribed by some doctors to treat hair loss despite the absence of FDA clearance. In the phase 3 dutasteride study done in Korea, there was no mention of sperm or sterility. This is a concern to me, and I do not believe that this drug will be approved for treating hair loss until issues like that are studied further. That’s my opinion, and I’m sure I’ll get plenty of emails from people telling me how I’m wrong or I’m narrow minded about this medication. Ultimately, going to market is in the hands of the drug company that manufactures it. I am reluctant to prescribe dutasteride, because without clinical clearance from the FDA, I do not know the risk of sterility for those taking the drug for their MPB treatment.

As for other new hair loss drugs… I am not familiar with anything in the short term pipeline of 5 years, so it seems 10 years may be enough for us to hope and short enough for us to wait. Propecia should be off patent in the 1mg finasteride dosage in the US in a few years, so once generics are available in local pharmacies I’d imagine it would cut into their profits. I’m sure the drug companies will want to have a newer product out before then to keep their shareholders happy.

Do You Perform a FUT Test Before Surgery?

Dr.Rassman/Dr.Pak,
I understand that you conduct tests (FOX test) on potential FUE patients to determine their suitability, but do you offer similar tests to FUT patients (e.g. 3 cm x 1 cm excision / ~300 graft mini-HT to ascertain healing quality of recipient and donor areas, donor-strip scar assessment, health of grafts, growth/take of transplants, etc.)?

Thank you.

Unless there is an issue with the patient who has a history of poor wound healing or poor growth, we do not test patients for FUT (follicular unit transplant — also known as “strip method”) surgery. If someone would like a mini hair transplant, that would be another issue.

The FOX Test (for follicular unit extraction / FUE) is only testing 1 to 5 extractions. We do not transplant the grafts unless there is a strong desire expressed by the patient. Thus the FOX test is a very simple 5 to 10 min test which we offer at no cost.

Do you mind expanding slightly on this cardiac side effect of minoxidil?

Some of the symptoms include: chest pain; irregular or very fast heartbeats; dizziness or lightheadedness; or fainting. Minoxidil is a hypotensive agent original designed to treat high blood pressure. Minoxidil is a direct-acting arterial vasodilator. The most dangerous of the complications is pericardial effusion reported with minoxidil treatment both in patients undergoing dialysis and those with normal renal function. Pericardial effusion can be large and result in cardiac tamponade in some cases and if untreated can rarely cause death.
