Do You Have Any Patients That Conceived a Child While on Propecia?
Dear Balding Blog,
I know fertility studies have been closely examined in relation to propecia usage at 1mg per day; however, my question is whether the doctors at balding blog know of anyone (patients) who have personally conceived whilst on propecia? I would like to hear anecdotal evidence, rather than clinical, and their experiences in relation to birth defects etc.

I am certain that some of our patients on Propecia have achieved pregnancy with their wives/girlfriends. There’s a couple of commenters on this site that have posted in the past about conceiving a child while taking the medication. Some patients ask us about this and we generally give the go-ahead, but we do discuss the theoretical risks.
If you think about it, millions of young men are on the drug and some must have achieved pregnancy. If birth defects were to occur (ambiguous genitalia) lawsuits would be common. The incidence of ambiguous genitalia is one in 2000 births so it would be difficult to connect the appearance of this problem with the drug finasteride. Play with the numbers — assume 2,000,000 young men on finasteride, that would produce 1000 cases of ambiguous genitalia.
How would one relate the drug impact with the natural incidence of this condition in the general birth populations? I don’t know the answer to this question.

Do You Examine for Hair Loss When the Hair is Wet or Dry?
When judging a persons hair loss, do you look at the hair when it is wet or dry or both? I find my hair looks thinner when it is wet or gel has been applied. When it is dry it looks thicker, that is why I like to you use hair spray because it makes my hair thicker. And yes, i am thinning in the front of my scalp
Well, if your hair is wet, it is pretty obvious that your hair will look thinner. Same goes for if your hair is cut short or under harsh lighting.
We check for miniaturization of the hair. It is ideal to have your hair dry without any styling product on it. We also examine the pattern of hair loss (for MPB). Wet or dry you will see a pattern. I suppose you can see the pattern more if the hair is wet and short.
Develop a Master Plan with your doctor, laying out what will happen to you over time (best and worst case scenarios). There are many options, so why not lay them all out in front of you?

Do You Ever Do Small Transplant Procedures of 500 Grafts?
Do you ever do transplants with grafts as low as 500? My hair is generally really good, my hairline is normal, the top of my hair is really thick, just the crown is slightly thinner than it should be but there’s still plenty of hair there. I just wonder if they’d do such a small transplant. I’m currently taking fin and my loss is completely stabilized.
Finasteride peaking is an observation reported by doctors and Merck in the original study of the drug. Yes, I do small hair transplant sessions (FUE or Strip) with the proper indication, but I would rarely do such small procedures on a man under 25, unless I understood the reason for it. I make individual assessments each and every time I perform a hair transplant with the mandate: “Never do harm to my patient”.

Do you change to dutasteride if finasteride is not working well?
So im almost 11 months on fin,And my hair changed sooooo much , thining all over and actually getting quite big bald spot in the crown. I did buzzcut , and thank god my gf dosent hate it. Few days ago i went to one of the top dermatologist in my country to advice me what to do next. I explained to him that fin failed , and i keep losing alot . I suggest moving to dut and he warn me about it.
As I develop a relationship with my patients and if after my relationship shows that finasteride is not working, I may add dutasteride; however, as I develop a Personalized Master Plan with my patients, I fit the results of the drug into that plan and then work with the patient addressing each successive step.

Do Vellus Hairs Turn Into Normal Hairs from Finasteride?
Is is safe to assume that vellus hairs turn into normal hairs on finasteride?
Vellus hairs stay as vellus hairs. In every follicular unit, there are one or two vellus hairs (short like stunted hairs). Miniaturized hairs, on the other hand, are terminal hairs (normal hairs) that lost their bulk and these are the hairs that get positively impacted from finasteride by increasing their shaft thickness when they are miniaturized.

Does thinning hair lead to balding?
Do the thinning areas in whatever balding pattern you have always end up totally losing all the hair in those areas? Or can those areas just thin to a certain percentage of it’s original density and then stabilize remain that way
Miniaturized hairs, if left untreated, will become balding areas. I have to follow the same people for more than one visit, and many balding patterns start with miniaturization (seen as thinning) and then advance to complete balding if untreated. The only patients I follow are those I am treating with finasteride, which is likely slowing the process and preventing balding. When discussing thinning, we discuss miniaturization in the front, top, or crown of the head, not the donor area. Donor area thinning reflects a condition called Diffuse Unpattterned Alopecia (DUOA)

Do Thin Eyebrows Mean a Thyroid Condition?
Hi doctor, I see that you mention that a thyroid condition may cause balding. I’ve read that one of the symptoms of a thyroid conditions is thin eyebrows. If I’ve always had thin eyebrows since I was a kid (especially on the outer edge), does that mean I have a thyroid condition – or that I just had naturally thin eyebrows?
Thin eyebrows just means you have thin eyebrows, especially since you report having this as a child. It doesn’t necessarily mean you have a thyroid issue. If you think you have a thyroid condition, then you should see your primary care doctor. It is a simple blood test.
If you want thicker eyebrows, you can always consider an eyebrow transplant, which takes hair from the back of the scalp. We’ve successfully transplanted eyebrows for many patients with great results, but the patient has to remember to trim their new eyebrows regularly since the hairs will continue to grow like it would on the scalp.

I’m not trying to suggest that swimming causes MPB or anything like that. I’m just wondering if the chlorine or other chemicals in the pool could be detrimental for those of us already struggling?
The chlorine in the swimming pool may damage your hair, but the growth mechanism is below the scalp, not reached by the pool water. There is a difference between damage to hair that has already grown and new hair that just emerges into your world. If you look at a long hair carefully, the part of the hair closes to the scalp is new and in its best shape, unless your have miniaturization associated with genetic hair loss
