Donald Trump had a Scalp Reduction surgery in the early 1990s (the surgical removal of his balding area), and may have had hair transplants producing a depleted donor area, strongly suggested by what we see in the video. The back of his head now appears bald which is most likely the result of many hair transplants procedures. The fact that he dyes his hair platinum blonde, again, strongly suggests that he had the old fashioned plugs, which is heavily softened by the blonde color. At the time his hair was worked on, few doctors were doing the surgery that we do today. I have been performing today’s technique since 1991 at about the same time as when he had his scalp reduction surgery. He has to keep his hair sprayed to keep it together, but obviously, the wind was worse than anticipated.

Reviving dormant follicles is not something that may always happen. The places were we see revival is usually when the loss is less than a year, two at most. Those who have had long term balding may not be able to revive those hairs. The traditional approaches all have value including Microneedling, minoxidil (oral or topical) and finasteride. Estrogens in the face of high testosterone may have more limited value.

You should never select a technology. A technology is only as good as the doctor who uses it. I have seen disastrous results with good technology. Instead, you should find an outstanding surgeon who has performed many FUE procedures and sees his patients by meeting them one-on-one, as I offer in my clinic to all prospective patients every month.

I apologise if this question has been asked and answered previously, but who would you recommend as the more accomplished surgeons performing FUE in the US. Kind regards

There are a handful of hair transplant surgeons who claim expertise in follicular unit extraction (FUE) in the U.S. You should visit the International Society of Hair Restoration Surgery (ISHRS) for a listing of qualified surgeons and there is a sub-group from this list that do this procedure. Some good hair transplant doctors do not do this procedure, by choice. At NHI, we created the FUE technique and published it in medical journals. There are a few doctors with publication credentials, but many claim expertise. Unfortunately, I have heard about many failures by some of the doctors who claim expertise so you need to do the research yourself and meet with the surgeons on your list (who claim FUE expertise). Ask to meet some of their patients so that you know what the surgeon is capable of doing. I realize this may not be the answer you are looking for, but there is some work required on your part to assure yourself of a good result. Even for me, sorting out the doctors who can perform successful FUE procedures is very difficult as there are so many who claim such expertise and just can’t cut it.

If there are many small spots of balding, then a visit with a doctor is critical because you may have a medical problem that requires expert insights. If you are a man, I am assuming that you are talking about male pattern ‘genetic’ balding. Both Propecia and Minoxidil may work. Minoxidil, applied to the balding area like a lotion, can be purchased over the counter in many stores. Propecia, a daily pill, requires a doctor’s prescription. Propecia, made only for use by men, can stop and even reverse the hair loss in some of the younger men.

Best Procedure for an Advanced Balding Pattern?

I have a Class 6 pattern of balding. Which is the best procedure for me? An FUE or the strip FUT?

Norwood 6For someone with Norwood class 6 pattern balding, the FUT (strip) technique will allow the entire job to be done in one or two sessions easily, allowing for ranges of between 2500-3500 grafts per session. A class 6 patient needs somewhere between 4,000-6,500 grafts. The wide range of transplants reflect the variations seen in the Class 6 patterns and the quality/thickness of the hair shafts. Men with fine hair, generally require more grafts but those with more confined Class 6 patterns and with medium or more coarse hair, can often get away with just one session.

The other technique, contrary to what you may read or hear, are generally limited to 1000-2000 grafts per session in the best of hands. For ideal work, you need to have a good quality of hair and enough donor density to allow for a safe surgery.

I would like the bald crown filled in as much as possible without that large ugly scar from the old methods of hair transplantation. Is the FUE and ARTAS are reasonable option? I only want the best!

Any hair transplant will fill in the crown. There are stip procedures and there are FUE procedures which are manual or robotic. You should shop this around and then make the decision yourself. Find a doctor you like with a great reputation and years of experience in the field, a doctor respected by other doctors. In the hands of a good doctor, any of these procedures is a good procedure. Your hair loss looks like mine did before I had strip procedure and now I don’t have a bald crown nor any scar as a result of thes trip procedures?

Best Post-Op Hair Transplant Shampoo?

what is the best shampoo to use after H.T. my head is peeling its been about 2 weeks and when can i start using rogaine again. Thanks

After 2 weeks you head should not be peeling. You need to just use your regular shampoo and make sure you are washing your scalp properly. Follow up with your doctor and make sure you are washing your hair correctly.

Usually, I’d recommend waiting 7-10 days before using Rogaine after a hair transplant. Talk to your surgeon.

Best Hair Thickening Agent?

I am a 42 year old male with thinning hair both at the front & crown area of my head. Without entertaining hair transplants, what do you recommend to improve the appearance of my hair? What is the best Thickening agent/shampoo out there? Are there any products that actually work in your opinion? I used minoxidol years ago but gave it up quickly.

There are many thickening agents/shampoos on the market and people respond differently to each depending upon texture, shaft thickness, the wave in the hair, and other factors. I generally tell people to buy a series of such products and then test what works best, or ask a hair stylist who works with such products daily and get a recommendation from them. Minoxidil does not impact the texture or shaft thickness from above the skin as thickening agents/shampoo will.
