1005 Grafts To Thicken the Hairline (with Photos)

First of all, let me thank you and your technicians and please convey my appreciation to the GREAT job they did in placing my grafts and for the fabulous technical job on my hair. I believe my regrowth was close to 100%. Grafts are very firmly rooted with no weak hairs that lie flat that I know of. I saw substantial new growth between the third and fourth months…I’d say from then up to six months when it was pretty much done. The grafts are very natural looking in the sense that texture and thickness is indistinguishable from other hair.

I’m extremely pleased with the appearance of the hairline and how the temples have filled in…it works very well with my features and the shape of my face…really well done on your part. I pull probably twenty-five white hairs from the first inch of the hairline because they look a little unnatural amongst the black hairs…I think it’s just the low density overall that makes it so. I just wish I had a spare three or four thousand hairs to throw at it!

The downsides? I now have more gray hairs around the scar line. The sutures took a long time to dissolve and were quite irritating. The scar line itself healed well but it’s still a scar line.

Sometimes we get great results with less hair than we would have liked to use, because of donor supply limitations. His surgical history is that he had 1600 grafts from us in 1996, then 1900 grafts from another well known clinic, and finally another procedure of 1005 grafts with us 2 years ago. 1000 hairs lost last night

I have been on finasteride for 2 years but today suddenly in the shower my hair started coming out in clumps. I’ve lost so much density, I bet more than 1000 hair went down the drain and additional hundred more when I was drying it. This includes the back and the sides of my head. I am panicking so much because when I started I only had mild temple recession with no hair loss anywhere else , and now this happened.

How old are you? If you are young, maybe you started to accelerate your balding genetics. This can happen if you are unusually stressed. Does that apply to you? if you are very worried, see a dermatologist of a hair specialist like me if yu in Southern California.

You need to develop a Master Plan with a good doctor to determine your lifetime hair supply and how much has been used so far. You must know this for future planning. Your residual donor density can be measured by your surgeon to establish what the future possibilities are.

100% Donor Scar Elimination?

Dr. Rassman,

Do you think that 100% elimination of donor scars will be possible in the future? Technology is always improving and it seems like scars could be emliminated. Are there any other possibilities other than Juvista and Acell that are being researched? Thank you for all the helpful information.

Scarring happens as a result of all surgeries. There is no such thing as 100% elimination of a scar and treatments like Juvista and Acell will not dispose of scars entirely. The key to dealing with scars is to keep them very, very small. In the case of a donor scar, it can be as small as 1mm wide naturally in good healers, or reduced to that level on occasion. For bad healers, there really is no solution. By “bad healers” I mean those people who stretch their scars after the wound has healed.

Techniques such as the trichophytic closure repair of a wound in the donor area works well in many, but not all patients. For more info on the trichophytic closure technique, see here and here.

10 years of finasteride, now my libido is down

I’m a long term user of Fin since 21 years old. Can honestly say I haven’t experienced one single side the entire time I’ve been taking it. I only take a quarter tablet every night 0.25mg I’m almost 31 in a few weeks. I’ve noticed over the past month or so my libido has decreased enough for me to notice.. I don’t have trouble getting erections, and I still wake up with one as well But I can go days without feeling the need to masturbate where I used to get the urge every day. My interest in the opposite sex has seemingly waned as well, I’m more focused on working out/exercising and my other hobbies. I’m trying to narrow down what could be causing it. Fin was obviously the first thing to spring to mind, could it be after years of no sides I could get them suddenly?

Yes, some men reported this to me. The problem is that as men get older, ED becomes a problem without finasteride and so does a reduced libido in some men, so it is difficult to tell if it is the drug unless they stop it and if they stop it, then the could lose considerable hair volume.

10 year study of finasteride use

This 10 year study of finasteride in Japanese men is very significant. (1) it revealed improvement and prevention of disease progression in 99.1% of the 532 Japanese men with AGA treated with 1 mg/day finasteride for 10 years, (2) younger patients show more improvement than that of older patients with AGA treatment [24,25]. In this study, AGA patients at the early stage of N-H classification showed more improvement than patients at the later stage did.

We evaluated the long-term (10-year) efficacy and safety of AGA treatment with 1 mg/day finasteride in a large study population (532 patients), as the first study of this kind in Japan, to our knowledge. A high objective efficacy was demonstrated by the MGPA, which revealed improvement and prevention of disease progression in 99.1% of the 532 Japanese men with AGA treated with 1 mg/day finasteride for 10 years. Furthermore, the outcome was similar to or better than that reported by other studies in Japan [8-10,13,17]. Differences have been known to occur in the progression of AGA symptoms between Japanese and Caucasian men [8,18]. This efficacy of the investigated treatment in Japanese men exceeded that reported in other studies in Caucasians. The superior response of Japanese men with AGA was reported to likely be attributable to their hair characteristics (greater diameter, black color, and lower density), which facilitated the detection of slight changes [10,19-23]. A novel finding observed in this study was the significant difference in the improvement of AGA following finasteride treatment between the N-H: I/II/III and N-H: IV/V/VI/VII groups at the first visit. The ROC analysis revealed a similar difference, that was performed to classify patients with improvement (MGPA?5) and deteriorating (MGPA<5) condition at year 10 of treatment; the cut-off point was N-H: III (AUC: 0.746). Furthermore, the MGPA of the total study population and the N-H: I/II/III group at the first visit significantly improved from treatment year 5 to 10 (P<0.001). This efficacy was different from that of a 5-year study in Japanese men, which reported that the efficacy began to plateau after 4 years of treatment [10]. Several studies have reported that AGA progresses in N-H classification with age, [7,11,12,18] and that younger patients show more improvement than that of older patients with AGA treatment [24,25]. In this study, AGA patients at the early stage of N-H classification showed more improvement than patients at the later stage did.

10 year Japanese Study of Finasteride use

This graph taken from the article below, tells us a great deal about long term finasteride use by pattern of hair loss. Look at the stabilizing effect on the progressive nature hair genetic hair loss once the pattern has been achieved (about 5 years into the study). It appears that many of the less advanced patterns of hair loss, have even gained some hair over the 10 year study.

10 weeks on finasteride (photo)

This man(in his mid-twenties has recent hair loss in the past 8 months prior to commencing treatment). He must have been very sensitive to the results of good DHT blocking with finasteride. Men like him are young and have more recent hair loss than those who do not respond. A great early result that will just get better in time.
