Hairline transplanted too low

I had a hair transplant and the hairline I was given is too low. The doctor said he would FUE out the hairline grafts.

Putting a hairline too low is a real bad problem. Just trying to do FUE to the grafts in the hairline does not work well. The laser hair removal systems can kill the grafts but leave you with marks in the frontal hairline. Best to send me photos and let me see the extent of the problem.

Hairline too low

This young man went to a clinic that clearly didn’t know what they were doing. His hairline is almost down to his eyebrows, less than 1 inch. Every hair in the frontal hairline that is too low will have to be removed, hair by hair. He will probably be scarred as well. This is a very costly repair. and it will take some time to see if he can get all of these hairs out. As soon as I saw this, my mind wondered into the type of people who can do this type of mutilation to a young man that just wanted to look better. Worse, they are probably doing this over and over again. I blocked out his face to preserve his dignity. He posted this on Reddit. I wonder if this could be a photoshopped photo, exaggerating it to get comments.

Hairmax Claims They Formally Challenged Dr Rassman

Hairmax claimed that at the ISHRS they “formally challenged you on the issue and you were unable to justify the accuracy of your hair counts you presented and you therefore had to retract your comments.” Is this true?

I remember that we had some verbal interchange, but I never retracted any of my comments.

HairMax LaserComb Receives FDA Approval?

I received two emails recently about the HairMax LaserComb, so I’ll combine my response into one…

EMAIL 1:

I was watching The Big Idea with Donny Deutsch last night, and a representative from the makers of the Hairmax Lasercomb announced point blank that they had received FDA approval to make claims for hair growth. He mentioned something like 19 new hairs per centimeter. This is a big deal. I was expecting a bigger announcement, and now I cannot find any press release or report announcing Hairmax’s FDA approval. If the comb is in fact FDA approved to stop hairloss/regrow hair, it’s something I would consider adding to my regimen. Have you heard anything about Hairmax gaining FDA approval?

Hello Dr. Rassman,
I recently read that the Hairmax Laser Comb has now been cleared by the FDA as a product which is “clinically proven to promote hair growth in males with androgenetic alopecia (Norwood II A to V with Fitzpatrick skin types I to IV)”. Here is the link: hairmaxforum.com

I’m a bit suprised that I have not heard more media buzz about this. Is this now the 3rd FDA approved method to treat MPB? I have read your prior posts on the subject and am just intrested to know what this new FDA approval actually means. Any thoughts? Thank you

This came out of the FDA and this is the exact wording in the final page of the FDA letter in January 2007:

Indications for Use:
The LaserComb is indicated to promote hair growth in males with androgenetic alopecia who have Norwood Hamilton Classifications of la to V and Fitzpatrick Skin Types I to IV.

It is important for the readership to note that just because this is cleared by the FDA, it does not mean that it is guaranteed to grow hair. I would like to see an independent scientific study with adequate statistics, done by a third party. If I saw that, I would be comfortable in recommending it depending on the results the study found. I have not seen the company’s submission science, so I can not endorse this and would not recommend that everyone go out and buy one. Frankly, I’m just not convinced, despite FDA clearance.

My hairline is uneven, what should I do?

Uneven hairlines in men usually indicate that recession from genetic balding has started. Showing up on only one side is not uncommon but eventually it will impact the other side and more recession of the entire hairline is very probable. See a doctor who can make the correct diagnosis. If you are over 25, you can transplant it to correct the uneven appearance but you should also take the drug finasteride to stop the progression of the balding process.

When I teach doctors how to do a hair transplant, I always discuss randomness for graft placement. The doctor generally fights his tendencies for orderliness and orderliness means alignment of the grafts in columns or rows, not normal as you can see. When the hair grows, it will show the orderliness when you look carefully at it. Also, note the picket fence hairline, which again is not normal. As this man already has hair, these issues may not bother him, but it bothers my aesthetic sense of things.

You should have only single hairs in the first 4 mm of your frontal hairline or the hair transplant shows as a hair transplant. Your surgeon may not have put only single hairs there. A transition zone of single hairs is critical to make for a no-hairline hairline. This is what you should now do, see a surgeon that knows how to do this and have it placed in-front of your existing hairline and you will look better than normal.

Hairs Falling Out in the Shower, and How It Relates to Growth Cycles

how do the hairs that come out in the shower relate to hair growth cycles?

As stated here before, it is normal to lose about 100 hairs a day. To clarify that more, as hair falls out there are hairs that are also starting to grow. Hair grows in cycles and you may notice more hair falling out some days. Hair follicle cells have three phases of growth:

  1. Growth phase (Anagen phase) which lasts anywhere from 2 to 6 years. This is the phase where your hair is actively growing at approximately 10cm per year. 85% of hair is at this phase at any given time.
  2. Transitional phase (Catagen phase) which lasts about 2 weeks. This is the phase where the hair follicle shrinks and prepares to enter the resting phase.
  3. Resting phase (Telogen phase) which lasts about 1- 6 months. This is the phase where hair does not grow but stays attached to the follicle. Some hairs are shed at this phase, but at the end the hair follicle re-enters the growth phase to start the cycle over again. 10-15% of hairs are at this phase at any given time.

If you think about it, the average person has 100,000 hairs on their head, with 100 hairs falling out each day. So in three years, the total hair population will have been replaced (you can do the math for yourself).

Hairs Come Out from Top, But Not from the Sides When I Pull On It

When I do a gentle pull test for my hair, I always seem to get about 10 or so hairs from the top front of my head,whereas I usually don’t get any (maybe one or so) from the sides and back. I’m wondering if this has anything to do with MPB or perhaps the hairs on the side and back come out at other times i.e. combing, hands through hair, etc.

Why are you pulling your hair? As the name implies, there is a pattern to male pattern baldness (MPB) — most of the hair is lost from the top and if it is impacted with miniaturization I would expect that you can easily pull out the hairs that are miniaturized. Hair impacted from genetic hair loss that is very early in the process should not pull out with the pull test. So if you have MPB, pulling your hair is not the way to go about diagnosing it. Mapping our your hair for miniaturization will tell you what you need to know for balding purposes.
