My Hair Has Become Thinner Over the Years

I’m a 37 year old male who has a full head of hair, however my hair has always been very fine and frizzy/slightly curly. As a result, I shampoo it daily and I’ve always applied gel and then used a blow dryer daily to straighten it too.

In the last few years, my hair has gotten much more fine, and overall it is less thick than it used to be and much more prone to static, especially if i let it dry naturally and don’t use product/blow dry. It’s clearly changed in thickness during my mid thirties, however I don’t seem to be receding dramatically or anything like that. Does this the fact that my hair has gotten much more fine mean that I might be beginning to go bald and is it likely that this process will continue? Do I need to take steps now to prevent it getting more fine?

Some people will see a change in the hair shaft diameter towards finer as they get older. That could explain what you are telling me. You want to be sure, however, that the finer hair is uniform all over the head (including the sides and back). The lack of uniformity could suggest a problem, so we generally like to map out the hair throughout the entire scalp for miniaturization to determine if you have some condition that is causing it. If uniformity is seen, then using styling agents to thicken the hair shaft is the way to go.

I have thin hair and soon will have bariatric surgery. I am worried about hair loss. I will follow my doctor to the letter, however is there something else I can do to minimize hair loss?

It seem like you have a conflict in choosing the lesser of two evils — lose weight or lose hair. I do not know the answer to your dilemma.

Bariatric surgery does not necessarily cause hair loss in everyone that has it done, but it is a possibility. I do not know of anything that can minimize that chance, though.

My Hair Is Thinning Where My Head Lays on My Pillow

I have a strange question for you. For the last few months I have been sleeping with my hand kind of positioned as a pillow, if that makes sense. I also sleep on one side and it just so happens my hair has thinned out just in this area. Between the ear and my temple right at the front edge. This is exactly where my hand is when I sleep. Could this be caused from this. I have changed my sleeping pattern, will the hair grow back. Thanks

I have never seen this before and only have had one person that I can recall even writing to me about a similar situation (Ergonomic Pillow and Hair Loss), and except for people who have trichotillomania and pull on their hair even in their sleep.

You should be examined by a good doctor. A miniaturization study should be done to see just what is happening. If your hair is miniaturized and you are rubbing on it, it might come out.

My Hair Keeps Going Through Seasonal Sheds That Worry Me

I am a 22 years old male with very little family history of male pattern baldness. Yet a few months ago (I think last may or april) I started to experience some drastic hair loss. I was literally shedding all over my clothes, the tub, the kitchen floor… As I grew really desperate I went to visit a dermatologist who told me I was going through telogen effluvium. I actually changed my diet at the beginning of the year by becoming a vegetarian, so I figured it made sense that my hair would undergo a moment of weakness. I took various vitamins for two months or so and things seemed to get better until october.

I have always been very sensitive to the changing of season, hair-wise, so it didn’t worry me at first, but now I find it weird that this is still going on to the present day. But what confuses me the most is the nature of my sheds. I had already noticed that I would lose hair that were a lot shorter than the others (my hair is mid-long), although they seemed to have the same thickness. This could all be a result of effluvium. However, I started to find these days hairs that were thinner than what they should be… Some of them have a normal length but are just pretty thin and fuzzy. Others are also really small… I have no visible recession of my hairline (yet ?). I can tell my hair still fall from all over my head, even in the areas never affected by AGA. But I am scared to death, are those tiny sheds a definite sign of MPB ?

You should get baseline measurements on your hair bulk to determine the significance of the shedding, and quantify just how much hair you are losing over time.

As for a seasonal shedding, I’ve had a few reports emailed to me from readers over the years about this, but there is no medical explanation that I know of for it. Humans have asynchronous hair growth, which means that we do not shed in seasons, but do so relatively uniformly over the year.

I am a 30 years old male, I found my two temple peaks are different, one is gradually disappearing, i had quite thick temple peak before, but only one side of the temple peak receding/disappearing, is that normal? The disappearing side has actually had some receding in temple as well, but not that obvious, so does that mean one side temple receding will cause its temple peak receding? Cheers

The bad news is that you may be in the process of losing hair in a classic male hair loss pattern.
The good news is that hair loss in the temple peaks will likely even out as it recedes. Temple peak recession does not necessarily follow male pattern balding in the classic sense.
If you’re concerned please see a doctor for an exam and possible ways to correct it with surgery if it bothers you enough.

My Hair Looks Weird After My Bout of Dermatillomania

I recently suffered from an OCD scalp-picking condition (dermatillomania) from high levels of stress, for about a year. Now, there are no scales on my scalp and it is really sensitive. All the hair is still there, it just sits really strangely on my head (without direction). I wanted to take steps to returning it to its original condition, but I’m not sure where to start. I’m not sure how you would be able to correct the direction of the hairs. You might try topical conditioners and test many of them. You may have caused permanent damage to the skin and thus seeing the directional issues with the hair.

My Hair Looks the Same 9 Months After Starting Propecia!

I’m 9 1/2 months into Propecia 1mg a day treatment and everything is about the same since I started. I’m currently a NW2 but under miniaturization I have a NW3 with mid anterior thinning. Right now I have early frontal loss all I needed is thickening of miniaturized hairs and that would give me a way fuller look. I’m 22 1/2 year old guy and started Propecia only like 4 months or so after I noticed I’m losing hair so I would be a candidate for reversal but now it looks slim because I’m close to hit that 1 year mark! Is there a still a chance? The reason I want to have those hairs thicken because I’m very worried if Propecia would lose effectiveness and MPB will start again those hairs will be gone before I know it but if they would thickened up I could at least spot it that I’m losing hair again and do something about it and also I want to do different hairstyles!

Sorry to break this to you, but there is no cure for male pattern baldness (MPB) and no guarantee that Propecia will work on you. There’s a new genetic test to determine if finasteride (Propecia) will be of benefit to you — HairDX Tests for Finasteride Response.

Propecia does work, but it is not the cure and will not completely stop hair loss. The drug very rarely reverses hair loss in the frontal area, but you should be able to retain the hair you had (which is what it sounds like it’s doing if you say everything is the same 9 months later). You need to sit down with your prescribing doctor and have a discussion on realistic expectations.

My Hair Looks Worse After Each Transplant!

I started noticing hair loss around the age of 26. I tried the laser for a year and did not believe it worked and took propecia, but felt that did not provide help either. In 2009 I decided to have a hair transplant surgery 2,000 hairs. 1,000 in the front and 1,000 in the back.

After the transplant I began finasteride and rogaine to maintain the amount. I was so happy after the first 6 months, until I started noticed thinning in the front again. Everytime I go to the doctor who will remain nameless they say things are fine. I went for the second procedure a year later to thicken up the front. It has been 5 months and the front looks worse and the back is starting to thin out on the sides and a little in the middle. I have a few questions.

  1. Can the initial surgery cause this shedding to occur and then stop?
  2. Could the rogaine cause an adverse affect? Every time I use the rogaine and start combing my hair at least 10-20 hairs come out. I read somewhere that if shedding does not stop after 5 months then stop.
  3. Does the finasteride not work as well as the propecia? These 2 drugs should keep the back fine, but that is not the case.
  4. Also could transplanted hair fall out?

I cannot comment too much on your particular issues, because I’ve never examined you… but I suspect there was no Master Plan associated with your hair loss issue and you were chasing after a goal that wasn’t clearly laid out. It’s possible you were never given realistic expectations. Perhaps you should see another surgeon for his/her opinion.

1. I do not know what your initial surgery was, what it was suppose to do, or even if it was successful. In general, you can have shock hair loss from the surgery itself. That’s one possibility. The shedding from shock hair loss is permanent.

2. Rogaine is a topical medication to grow hair, not lose hair. If you have an adverse reaction, then ask your doctor for an examination. Losing 10-20 hairs is normal with or without Rogaine. Finally, some patients report slightly more hair loss when starting Rogaine treatment, but as you continue to use it there should not be a problem. The initial hair loss from Rogaine is thought of as a “cycling” of the old hair falling out in preparation of new hairs to grow.

3. Propecia is finasteride in 1mg strength. There is no difference as it is the same medication. Propecia is the marketing name, like Tylenol is the marketing name for acetaminophen… or Rogaine is the name for minoxidil. I’m not sure what you mean by the “back” … is it the back of the head (donor area) or the top/crown?

4. Transplanted hairs are considered permanent as your donor hair. They do fall out as the hairs cycle, but it regrows.

My Hair is Very Thin On One Side

When I wake up – I look in the mirror and it’s obvious that my hair is much thinner on one side of my head. I can create a big parting on this side because the hair is so thin and can’t do so if I try on the other side. I’m 27 and have slightly receded temples and the temple recession is slightly worse on this side but I don’t understand why it’s thin all the way to the back of my head on this side?

Is this common with male pattern baldness? My dad is bald but I have been quite stressed over the last year and don’t want to take propecia if it’s something other than MPB.

It is common to see more thinning on one side in cases of genetic male pattern baldness (MPB), but rest assured that the other side generally catches up. I do not know if you have MPB, but if you believe you are losing hair you should see a doctor and get a diagnosis.

My Hair Knots Up and Pulls Out When Applying Minoxidil

Hello Doctor

I’m a 27yo male with a full head of hair yet. I comb my hair a couple of times a day after my minoxidil application with a wide tooth comb. As I have medium length hair sometimes knots are formed and even if I’m gentle sometimes I feel little painful pulls.

Should I be worried about those? I doubt any hairs are being pulled out by the root because I try to be gentle, but if some hair does, will it grow back?

If you’re being gentle, I don’t know what to tell you. You could try switching to the foam instead of the liquid solution and see if it helps.
