Dear Sir,

I know you have touched on the efect of hormone supplementation in some earlier messages.

There are mixed views on whether DHEA supplmentation (say 25mg per day) in men leads to increased hair loss. While it certainly has made improvements to overall wellbeing, I always detect (or believe I do) increased hair loss while I am on DHEA and topical testosterone.

In your view, do these supplements increase risk of male hair loss and can this risk be minimised while using them? Thank you.

DHEA and steroids do make hair loss worse. Propecia may minimize the impact and it is doubtful that it would hurt. Check this out with the doctor who is prescribing your medications.

Good morning doc. I’m a 26 year old male who is starting to go gray, pretty heavy on the sides & starting to in the front as well. Is this a good sign that said gray hair won’t fall out? Does this mean the gray hair has already passed a barrier to balding or is it just as likely to fall out as the other “youthful” (brown) colored hairs? Thanks for the great blog!

The graying of hairs is independent of balding (no relationship). Graying is genetic and not necessarily reflecting getting ‘old’, and you are not alone. I have seen many men and women who turn gray in their 20’s. The reason most people do not see them is that they hit the dying bottle and never show their secret.

Hello. I have a rare type of mild skin condition called “Raynaud’s disease” or “Raynaud’s phenomenon”. It basically consists of a tingling sensation on the surface of my skin and scalp, in a designated area for a second or two, when I experience stress or anxiety. For a long time I have wondered if it would eventually lead to thinning or balding on my scalp. I have not experienced any bad side effects yet, but I wanted professional opinions. Do you think my condition will lead to hair-loss? Do people that lose their hair physically feel it prior? Do I have any reason to be concerned?

Raynaud’s phenomenon and Raynaud’s disease, a similar disorder, may be associated with autoimmune disorders such as rheumatoid arthritis, systemic lupus erythematosus, and scleroderma. Genetic female balding is present in 50% of women (most are past 50) and these women can get Raynaud’s disease as well. I would not focus upon hair loss that you might get. This would not be a productive exercise. People with full blown autoimmune diseases like the ones mentioned above, can experience hair loss.

Is it true that by the age of around 33, if no genetic balding pattern has shown much of a sign of appearing it is unlikely that it will appear in a way that will result in major hairloss?

You need to find out what you have in terms of miniaturization. It would be unlikely that you would bald significantly if: (1) there was no miniaturization of the hair in the front, top or crown at 33, and (2) no one in your family started to bald later than 33. You can check your scalp for miniaturization by mapping it out. If none exists, the probability is that you may never bald, but nothing is ever certain, even for James Bond.

Update: A reader pointed out something I was oblivious to. Sean Connery was bald in his 20s.

i was just wanting to ask a little question for over a year now. i have been thinning on one side of my head, my right side above the ears and surrounding the temple area. what it is i want to know is when i get stressed i get this burning sensation around the thinning area like it feels like it is falling out as i get stressed. is it possible that when i get stressed i get really stressed?

I do not understand your symptoms. An examination is critical to understand what you are thinking is thinning above the ear on one side. It would be unusual for stress to impact one area cyclically, while leaving the rest of your scalp unaffected.

If someone has a mother’s father that is bald, will that person then be bald? Does this linkage dictate male balding genetics?

No, I do not believe that this is the case. The expression of the balding genes are not well understood and that is why it is so difficult to predict what will happen to any particular individual with balding in his family line.

Is it true that a man could escape the balding risk if his mother doesn’t inherit the MPB gene from her father and instead inherits genes from the mother’s side?

Again, there is little predictive value from knowing just two generations and certainly no value in knowing or determining what one might inherit from the mother. We have not really identified the genes that are responsible for genetic balding and I believe that there are many of them working together.

When we look at the fathers side of the family to predict any sort of inheritance pattern concerning MPB, should we exclusively just look at the male’s father and grandfather?

If one inherits the father’s genes associated with MPB, you can not assume that the son will have the same hair loss pattern of his father. I have seen sons that have exactly the father’s pattern and a brother that has no balding what-so-ever.

I have seen many women with generalized thinning (woman’s version of the balding process) and they have reported many women in their families that have had the same problem. There appears to be a link between women who show thinning and their mothers, grandmothers, and sisters, many of whom will also have hair thinning/balding. As I think about it more (while answering your questions), I only remember women to women links from taking the history of women. Men have rarely reported that their mothers have had thinning hair, but I know that my mother did have considerable thinning after menopause and I have genetic balding (I was transplanted in the crown), and my brother has it but has not elected to get the problem addressed.

Steroids are strange in that they theoretically can kick-start a balding process. It is possible that a short course of steroids might start what may have otherwise taken years to start. Once started, anything goes. Your question should be: Do I have active genetic hair loss? The answer is get yourself examined; have your scalp mapped out for miniaturization to find out if you have genetic balding. With that knowledge, you can make intelligent decisions on what to do. I have been on propecia and rogaine for 1 year but 20 days ago I started to have systemic sides ,problems caused by minoxidil and my doctor told me to stop using minoxidil
I asked him to recommend me something else inseat of minoxidil or at least to keep my hair healty cause propecia works very little on me for mpb but he said he does not know the answer so I wanna ask you same question cause you have experienced as I found out in internet and I wanna know if I can use aminexil and sod (copper peptides)for whole life time as in minoxidil

I have wrote to you before about Propecia losing it’s effectiveness for me after 6+/- years.

I have had just about every blood test done which were all in a normal range. Thyrioid is OK.
The only thing that appeared high was my Estrogen/Estradiol levels.

Last stop was an Endrocinologist who thought the level was a bit high but not horribly high.

The problem is my hair is just coming out daily. I can pull on it and get a shed. Wash it and get a shed. Nothing is slowing down the loss.

My question is, should I try Avodart since nothing else seems to be working.

I am 40 and basically a healthy guy.
But I am desperate as well.

Is it normal for all of the top to just go in such a short amount of time (6+/- months)?

Hair loss is an ongoing process with or without using Propecia. However, hair loss may slow down or temporarily stop once on Propecia. Avodart appears to be a longer acting drug in the same class as Propecia. I am not certain if it will be better than Propecia. I need to see more data before recommending you to try it, but without the FDA’s approval, I am not willing to recommend it yet. Despite the theory behind Avodart, there is no concrete study out there to prove that this drug is better than Propecia.

Hi dr. Rassman. recently I was prescribed an antidandruff shampoo called nizoral 2% ketoconazole. I used it religiously and initially felt some relief. but i was only supposed to use it 2-3 times per week and instead i used it every day. now i have noticed tha ti have a bunch of white hairs or hairs that are both white and black on the same follicle or new brown hairs. I have also noticed that i have lost quite a bit of hair on one side in the front and increased itching/burning in that area. I am only 17 years old and am positively not suffering from male pattern baldness ( i have hair like growing down to my eyes). but I am wondering if you think that the hair i lost in front with nizoral will grow back or if i permanently damaged my scalp beyond repair. Also note that there is no pain anymore that i have discontinued use of it for a month. but the shedding associated with it only stopped a couple of weeks ago

Possibly, you are allergic to the chemical in nizoral (2% ketoconazole). If your hair loss is due to an allergic reaction to medication or “chemical irritation,” then it will likely return back in 6 to 8 months after the offending chemical has been removed. Check back with me if there is no regrowth after 6-8 months.
