Just a quick note. I’ve been an avid reader of your site for some time now, so, firstly, I’d like to congratulate you on your blog, to thank you for taking the subject of male pattern baldness so seriously and for answering the questions readers have posed over the years so professionally.

I’m 30 years old, I’ve been taking Propecia for 16 months now and while my hairline has continued to mature (which can only be expected), I have found that with the help of Propecia my hair loss has stabilised. However, one side effect that I wasn’t expecting is the thinning of my arm hair! Can I take this as proof that Propecia is indeed blocking DHT (and therefore working)!?

Yes, loss of body hair is a known rare side effect with finasteride. Hair on various parts of the body does seem to be dependent upon the presence of DHT (more in some individuals than others). I wish it had that impact on my body hair also.

I’d say that if your hair loss stabilized that the Propecia is indeed working.

Scraping Off Minoxidil Residue – Hair Loss Information – Balding Blog

Hi,

I recently started minoxidil topical (3 months ago) and have considerable dry residue on my scalp. I’ve been using a comb to scrap the residue off before washing with Nizoral. Am I doing any damage to my hair by scraping my scalp with the comb?

I’ve considered trying the foam, but changed my mind because of the bad feedback Ive been reading on the internet.

A good set of washes should remove all residue from your hair. The foam has alcohol in it and may not leave residue, but I do not have any other suggestions other than trying the foam. I am not sure what bad feedback you are getting over the internet, but if you try it and don’t like it, let it go.

I Think I Had Shock Loss Even Though I’ve Been Taking Finasteride – Hair Loss Information by Dr. William Rassman

Thanks for providing all the information you are providing to hair loss sufferers. I have a question. I had a Transplant a 7 weeks ago(Strip method) on my crown area. I was on (and still on) Finasteride prior to my Transplant. Lately I lost my native hair and I am seeing that I have less hair on my top now compared to hair that I had before the transplant. I learnt that this is because of the shock loss. But I was on finasteride before the HT. So, do you think this is temporary and can expect m native hair back. If so how much time does it take?

I try to educate my patients and readers everyday… and this must be one of the top ten things I want the public to understand.

Taking Propecia (finasteride) does not protect you 100% from experiencing shock loss — it only lowers the risk. If you have shock loss, there’s not much you can do (actually, nothing you can do)… and if it is in miniaturized hairs, it may very well be permanent! Statistics do not matter to those who have shock loss even while on Propecia. This is one of the risks of a hair transplant and you must always balance the benefits of surgery with the risks of surgery.

I’m 9 1/2 months into Propecia 1mg a day treatment and everything is about the same since I started. I’m currently a NW2 but under miniaturization I have a NW3 with mid anterior thinning. Right now I have early frontal loss all I needed is thickening of miniaturized hairs and that would give me a way fuller look. I’m 22 1/2 year old guy and started Propecia only like 4 months or so after I noticed I’m losing hair so I would be a candidate for reversal but now it looks slim because I’m close to hit that 1 year mark! Is there a still a chance? The reason I want to have those hairs thicken because I’m very worried if Propecia would lose effectiveness and MPB will start again those hairs will be gone before I know it but if they would thickened up I could at least spot it that I’m losing hair again and do something about it and also I want to do different hairstyles!

Sorry to break this to you, but there is no cure for male pattern baldness (MPB) and no guarantee that Propecia will work on you. There’s a new genetic test to determine if finasteride (Propecia) will be of benefit to you — HairDX Tests for Finasteride Response.

Propecia does work, but it is not the cure and will not completely stop hair loss. The drug very rarely reverses hair loss in the frontal area, but you should be able to retain the hair you had (which is what it sounds like it’s doing if you say everything is the same 9 months later). You need to sit down with your prescribing doctor and have a discussion on realistic expectations.

I have read that rogaine promotes hair re-growth by enlarging the hair follicle. I have also read that propecia doesn’t do as well with hair re-growth but maintains hair by preventing excess DHT from shrinking the hair follicle.

Will propecia prevent the shrinking of a hair follicle that was enlarged by rogaine? So that you could use rogaine to get re-growth, but then stop using the rogaine as long you continue with the propecia?

Or will the new growth hair fall out when you stop using the rogaine even if you continue the propecia?

You are attempting to make a logical conclusion to things that we just don’t understand (at least I don’t). This is a far stretch of logic. It’s been my experience that once you stop using Rogaine (minoxidil), the Propecia will not retain those hairs (and vice versa).

Propecia, Green Tea, and Soy – Hair Loss Information by Dr. William Rassman

Hi Doctor, I’m moving to Japan shortly for several years, I’ve read some contradictory information about green tea interacting w/ propecia as well as soy products. Since Asian cuisine uses much soy as well as green tea on a daily basis, I’m wondering if this will effect the benefits of propecia. I do enjoy green tea & would like to drink it & use soy products, do you think either of these natural can be consumed regularly while still receiving all the benefits from propecia? Thanks.

Upon reviewing your CV, I am extremely impressed with all that you have accomplished. It is an honor to be able to read your thoughts on your blog.

I am a medical student at a Philadelphia medical school, and I too am going into the armed forces via the Health Professions Scholarship Program. I see that you served in the Army MSC. How was your experience with that? I am excited to serve my country and have medical school paid for at the same time.

Finally, and as it pertains to the discussion at-hand, I have just begun a propecia 1mg/day regimen today. I also use topical minoxidil (MiN Agent 5%). Is there anything else I can be doing to halt/reverse my thinning hair? As a 23-y/o male how soon can I expect results? What is the advantage of adding nizoral or t-gel to my current regimen? My scalp sometimes itches, but I rarely get dandruff or redness as would be evidence of sebborheic dermatitis.

I recently saw a HT surgeon for my finasteride prescription. He didn’t do a miniaturization study, however he took before/after photos. Is this sufficient, or is a miniaturization study absolutely necessary? It is obvious that my hairs are miniaturized in the front and at the vertex, while there is no “bald spot”, the hair is losing pigment and is noticeably finer.

Finally, how do you feel about the current 1-yr standard training for HT surgeons? It is obvious that you have far more surgical training than that. Do you feel a one-year fellowship is adequate?

At 23 years old, your hair loss is likely in the early stages, so the minoxidil / finasteride combo you’re using is the best you can do for yourself. Nizoral’s active ingredient is ketoconazole, which some believe will help with hair growth (though that’s not been proven)… and Neutrogena T-Gel’s active ingredient is tar, which will offer no benefits for hair growth (yet it still is used by some as a hair loss treatment). Both are good dandruff shampoos, though.

Obviously, a miniaturization study puts numbers to the balding. When I see it reverse from the photographs, I expect that the degree of miniaturization should also be less. I never understood why doctors in this field are so reluctant to put numbers to the degree of miniaturization.

I have one of the few certified facilities that offer training for hair transplant surgeons. When I train them, the fellowship lasts a year. It is very difficult to train these doctors, as I have to volunteer my patients (with their permission) to allow the trainee to work on them. I am right at their side when that happens, so that the work being done is totally under my control. This is one of the many reasons that it takes a year to train. Even after that period, I have seen the difficulties the doctors have when they are completely on their own.

I have dissolved 5mg minoxidil tablet in about a tablespoon of propylene glycol. massage into scalp. Morning and night. Any reasons why this wouldn’t be as good as Commercial stuff ie.Rogaine. or as good as oral admin? I take 1gm finasteride. Thinking of also dissolving an extra 1gm finasteride into mix.

I’m not sure the formulations are equal, but the ingredients you’re using are the same as the commercial version of liquid minoxidil (the hair loss treatment everyone is familiar with). Experimentation is good, but why experiment with an unknown impact when going the commercial route is a known process with known benefits?

As for dissolving finasteride into it, topical finasteride hasn’t been proven to have the same effects as oral finasteride, so you might end up just wasting it.

I’ve been drinking heavily and partying hard for the past few years and have lost a substantial amount of hair. I understand that the weight gain issue is controllable, but is the hair loss something i can prevent? Thanks.

Genetics is the main reason men lose hair, and I assume that you’re in your 20s or 30s if you’re doing heavy drinking and late night partying (just a shot in the dark), which is around the age many men see their hair begin to thin.

If you’ve got male pattern baldness, your doctor should write you a prescription for Propecia (finasteride), but without knowing much more about you (age, for starters), I can’t really give much more advice than that. The hair loss isn’t necessarily preventable, but you can halt the progressive nature and possibly regrow some of what you lost.

The Medical Board referred the case for criminal prosecution and Hageseth was charged with violating Business and Professions Code section 2052, a felony, for practicing medicine in California without a license.
