Will Propecia Work for Years to Come? – Hair Loss Information – Balding Blog

I’m almost 23 and been on Propecia for about a year now. I started to have early frontal hair loss. And also a year ago I had a miniturzation study done and I was on a NW 2A-3A pattern. Like the first inch into my hairline the hair was lighter. Now I’m filled up again and am on a solid NW2 so Propecia was a success.

Is there a possibility Propecia might work for me many years to come? I’m afraid it will not since I know guys who start to have MPB in the early 20s have usually aggressive hair loss, so I know Propecia can’t hang in for too long. And it’s funny how nobody in my family had MPB except my father but not till his mid 30s.

I couldn’t say for sure how long you’ll see lasting benefits from Propecia. I’ve had patients on it for 10+ years and it still shows the same benefits it did in year 1. As I’ve said many times before, each person is different and will react differently to various medications. Here are some facts — Results with Propecia.

Official Word from Merck Regarding Propecia’s Effect on Sperm and Pregnancy – Hair Loss Information – Balding Blog

Dr. Bill
I know that you’ve stated n previous blogs that you don’t think Propecia (finasteride) would be harmful or adversive to a man trying to conceive, but i recently came across an article on WebMD WebMD – Boosting Male Fertility where the doctor said just the opposite. I don’t mean to pit you against another doctor, but i felt like the conclusion may derive from the conflicting views. Thanks

PropeciaI’ve received a great number of emails about pregnancy and Propecia over the years since starting this site, and I’ve recently received a written response from Merck (the drug company that makes Propecia) on the matter which I would like to share with you. There is no doubt that Propecia, if it reached the baby, particularly in the first trimester, would have an impact on the baby and its sexual development. The main issue for young men who want to start a family may be simply: Can finasteride be transferred from the sperm of a man taking Propecia to the pregnant woman from sexual intercourse? Other issues reflect the impact on libido, erectile performance, and sperm count/motility/fertility in the young man taking Propecia. I wrote to Merck to get a better handle on these issue and here is what they wrote back to me —

Dear Doctor Rassman:

This is in response to your recent inquiry regarding PROPECIA (finasteride) and PROSCAR (finasteride). Your inquiry concerned the effect of PROPECIA on the morphology of sperm and on offspring born to partners of men receiving PROPECIA.

Available data indicate that the level of PROPECIA in the semen of a man taking PROPECIA does not pose a risk to an unborn child. Accordingly, a man can take PROPECIA while conceiving a child with his partner, or have sexual intercourse with his partner if she is already pregnant.

There is no evidence of influence on the gender of a child conceived while the male is taking PROPECIA. Finasteride 5 mg (PROSCAR) was first approved for the treatment of benign prostatic hypertrophy (BPH) in 1992, and to date, is marketed in over 100 countries. Finasteride 1 mg (PROPECIA) is approved in over 60 countries, and has been marketed in the U.S. since January 1998. There are over 16 million patient-treatment years of post-marketing experience exist for finasteride 5 mg and over 4 million patient-treatment years of post-marketing data for finasteride 1 mg.

Based on this extensive experience, no significant mechanism-based or non-mechanism-based teratologic effects have been observed in infants bom to women exposed to finasteride via semen during pregnancy.

In a study evaluating the effect of finasteride on semen parameters, 181 men, age 19 to 41 years, were randomized to receive either finasteride 1 mg or placebo for 48 weeks (four spermatogenic cycles). Of the 181 men randomized, 79 were included in a subset for collection and analysis of sequential semen samples. The results showed that, compared with placebo, finasteride 1 mg/day for 48 weeks did not affect sperm concentration, total sperm per ejaculate, the percentage of motile sperm or the percentage of sperm with normal morphology in ejaculated semen [1]. Published medical literature describing data from men with genetic Type 2 5a-reductase deficiency suggest that DHT did not appear to be important for spermatogenesis or the sperm maturation process. These men have lifelong suppression of DHT formation and those without anatomic abnormalities, such as cryptorchidism, may have normal spermatogenesis and are able to have healthy progeny [2]. The absence of any clinically relevant effects of finasteride 1 mg on semen parameters in this study, despite significant changes in serum DHT, supports the hypothesis that testosterone, and not DHT, is the primary androgen regulating spermatogenesis, sperm maturation, and seminal fluid production in the testis, epididymis, and seminal vesicle.

Glina et al. [3] described 3 men (ages 31,32, and 33 years) who showed a severe decrease in spermatogenesis during therapy with finasteride 1 mg/day for 6 months. Patients 1 and 2 were diagnosed with varicocele (attempting conception for 10 and 6 months, respectively) and Patient 3 was obese (attempting conception for 6 months). Seminal analysis was performed either once or twice during therapy with finasteride and again once or twice 3 to 4 months after therapy was discontinued. All 3 patients showed abnormal seminal patterns while using finasteride 1 mg. Alterations were completely reversed in Patients 1 and 2 and improved in Patient 3, three or four months after therapy discontinuation. The authors suggested that finasteride may not dramatically change spermatogenesis in healthy men. However, in patients with pre-existing risk factors for infertility, finasteride may amplify the problem.

The authors concluded that further studies are needed to evaluate the effect of finasteride on patients with fertility problems.

Lewis et al. [4] conducted a double-blind, placebo-controlled study of the effect of finasteride on semen production and sexual function in a total of 47 men, aged 30-50 years. The men were followed during 12 weeks of therapy with finasteride 5 mg daily (n=24) or placebo (n=23) and were reevaluated 12 weeks after the drug had been discontinued. Finasteride significantly reduced the volume of ejaculate by 0.5 mL (25%) and slightly increased the pH of the semen. No changes in sperm concentration, total sperm per ejaculation, motility, or morphology were observed. There were no effects of therapy on sexual function. There were no changes in the size of the testicles, the seminal vesicles, or the prostate in young men without BPH. The drug was well tolerated, and no significant toxicity was reported. It was concluded that finasteride finasteride appears to have no clinically significant effect on sexual function or semen production in young men.

Using an Estrogen Reducer with Propecia to Avoid Possible Gynecomastia? – Hair Loss Information – Balding Blog

There are some reports of users developing gyno while on propecia. I can only assume this has to do with the fact that DHT is an anti-estrogen, and Propecia inhibits DHT production resulting in more estrogen.

My question is, can I successfully use a SERM (Selective Estrogen Receptor Modulators) like Nolvadex to reduce the amount of estrogen in the body while I take Propecia? These drugs are used by bodybuilders after their steroid cycles to ensure they do not develop gyno either. I know you’re probably going to tell me to check with my doctor (which I don’t have and can’t afford), but I’m just asking for your opinion mainly… from a medical standpoint.

Thank you!

Developing breast tenderness, nodules, or even gynecomastia can be a side effect of Propecia, but this is a rare occurrence. You are correct that Propecia blocks the conversion of testosterone to DHT, which causes an increase in testosterone in the blood that can be aromatized (converted) to estrogen, thereby causing the side effects. You can always decrease the dose of Propecia under the advice of your physician if you develop any unwanted side effects. In my experience, however, those who developed gynecomastia did not get a benefit of halving the dose.

The use of other drugs must be decisions between you and your doctor, remembering that this is just a website which can not make recommendations in your treatment.

Body Hair from Treating High Blood Pressure with Minoxidil – Hair Loss Information – Balding Blog

The following comes from Dr. Michael Beehner, reflecting back to the days when minoxidil was used in the treatment of high blood pressure (1970s). He writes…

I’ll never forget this one older lady in her 80’s when I was in family practice, who had one of the highest blood pressure readings I had ever seen. After trying a variety of other drugs and combinations, I found that the only thing that would bring her blood pressure down into the normal range was Minoxidil, along with a Thiazide (water pill). I followed her for a few years on this and always marveled at how hairy her arms, the back of her hands, and virtually all of her body was. It was like evolution going backwards, in the days when mankind had body hair. In hindsight it didn’t take too many brain cells for the person who figured out that there was a connection between the Minoxidil and hair growth!

Can I Put Minoxidil On My Face to Grow a Beard – Hair Loss Information – Balding Blog

Hello,

I’m 23 years old and have lot of spots with no facial hairs. It’s really embarassing because if it’s not shaved, it looks awfull. Can Minoxidil can be used for that case ?

Thanks a lot.

Block Quote

Rogaine (minoxidil) can cause facial hair growth if it is applied to the face, but the hair that grows may not look like beard hair. I’m not suggesting you do this, as you should follow the instructions outlined on the packaging. It is an over the counter medication though, so you’ll have to use your own judgment.

Hair Loss InformationXandrox15 Plus – Hair Loss Information – Balding Blog

Hi

What’s your opinion on the new Xandrox15 Plus product which includes 0.01% finasteride?

Does finasteride work as part of a topical solution? Does it still carry the same risks ie erectile dysfunction, male breast etc…

Thanks

Block Quote

Xandrox15I have reviewed the product’s website. Xandrox15 Plus seems to have two known active ingredients:

  • minoxidil, which is known to be absorbed through the skin
  • finasteride, which is not known to be absorbed through the skin at the very low dose they utilize

They say it contains, “15% minoxidil, 5% azelaic acid and 0.1% finasteride by weight. Xandrox15-PLUS is available in 125mL bottles. Each mL of Xandrox15-PLUS contains 150mg of minoxidil, 50mg of azelaic acid and 1mg finasteride.”

You would have to take the word of the seller here, something that I generally do not like to do as we do not like becoming a third party endorsement. The seller does warn about sexual side effects of this product and worse, the finasteride is a controlled drug that requires a doctor’s prescription. So it seems that the company has violated some FDA issues including labeling, safety, and effectiveness proofs, and the ability to sell this over the counter. Even if the seller is a doctor, I believe that does not allow the seller to make unsubstantiated medical claims on safety and effectiveness, particularly with regard to the topical Propecia.

Is Propecia Making Me Constipated? – Balding Blog

I been on Propecia for 11 days now and since like day 5 I had constipation and lack of bowel movement. I couldn’t find any info that Propecia causes this. I also take Zegerid 40mg 2x daily and Carafate Liquid 2x daiy but I been takin those for like almost 2 years for my severe throat acid reflux. But I never had a prob with those. Also within this time I had no food or lifestyle changes the constipation was just sudden.

Could it be that Propecia is kinda interacting with those meds? But my doc said Propecia doesn’t interact with anything especially my meds.

Bread - fiberPropecia (finasteride) is not known to cause constipation. It is unlikely that Propecia when taken with your other medications is causing you to be constipated, however, all medications have the possibility of interacting with one another. You need to discuss with your doctor other varied yet potentially treatable causes of constipation such as hypothyroidism, dehydration, depression, etc. Perhaps you need more fiber in your diet?

Inherited Baldness Gene – Will the Pattern Be the Same? – Hair Loss Information – Balding Blog

Hi, I have some questions about Propecia. First, after about nine months and a very disappointing 1 month bout with Hair Club (which I think did more harm than good), my hair loss seems to have stabilized. Currently, I’m somwewhere between a Norwood 2 and 3 and there is actually a little bit of regrowth around the temples of all places.

Here are my main questions:

  1. I seem to have inherited my baldness gene from my maternal grandfather, who went totally bald. I’m 26, so, if Propecia has currently stopped the process and there are reports of patients keeping what they have up to 10 years, is it possible that my ultimate hair loss destiny may be signifcantly postponed?
  2. I read on one message here that “even though you inherit the baldness gene it can be expressed differently.” Does this mean it is possible for you to get a slightly diffeent pattern than the person you inherited it from. My hair loss seems more like that of my maternal grandfather but not exactly.

I know without a personal meeting you can’t answer 100%, but I just wondered if these things are a possibility.

Block Quote

You’re right — a personal one-on-one examination of your scalp is best, but I’ll try to answer your questions as best as I can:

  1. I do not believe genetic hair loss (androgenic alopecia) can be completely stopped with drugs. Propecia (finasteride 1mg) is a great medication, but it may not stop the process completely… and it almost certainly will slow it down. Results are variable for each patient as everybody has a different “destiny”.
  2. Just because your maternal grandfather was completely bald does not mean you will be completely bald, even if you inherit his genes, because the expression differs. Also, there is more than one gene for balding and if you inherit one, it does not mean that the rest follow. Again, everybody is different.

Hair Loss InformationVery High Estrogen Levels After 4 Months on Propecia – Hair Loss Information – Balding Blog

Hi there!
I’ve been suffering from side effects from propecia. However, after 4 1/2 months it worked very well for me. I had some bloodwork done and my estrogene lvl was HIGH above range (2x the maximum). Is it possible that not the reduction of dht, but the raise of estrogens (which we all know that can be caused by propecia and which may lead to effects like gynecomastia) and not the reduction of dht is beneficial for the hair? And do you think i could have success with a lower dose finasteride together with a hormonal balance supplement like “DIM” to reduce side effects? Or is it to dangerous for a guy like me (i’m 19, you saw that i reacted quite sensitively to hormonal drugs). thanks

Block Quote

You need to discuss this with your prescribing doctor, of course. Possibly a lower dosage (1/2 dose of Propecia) will have less impact on your estrogen levels. The impact of Propecia on estrogen appears to be as an antagonist and the body has feedback mechanisms that may very well be what you are observing.

Hair Loss InformationCan I Replace Propecia With an Herbal Remedy? – Hair Loss Information – Balding Blog

Blue ballIf you have been on Propecia (finasteride 1mg) for three years already, I doubt that what you have experienced is related to a Propecia side effect. Some doctor prescribed it to you, so check with that doctor and get yourself examined! While side effects may be a possibility, having an erection from sexual excitement for a prolonged period of time can cause pain in the groin (vasocongestion). I believe the lay term would be also known as “blue balls”.

Unfortunately, there is no herbal alternative to Propecia. If you are concerned about the side effects of Propecia, you should consult with the physician who has prescribed the medication to you and come up with plan that may suit your needs. It would be a shame to give up on the benefits of what you achieved by stopping the drug when what you may be experiencing has little to do with the side effects of Propecia. You can read the Propecia prescribing information on the official Propecia.com website.
