Dear Dr. Rassman,

First I would like to take this opportunity to thank you for this great website.

I have two questions and i would be very greatful if you can answer them :

1. I had an FUE procedure done on the front area (frontal hair line)with approx. 1200 grafts before three months ago, the strange thing that i am experiancing is some hair loss in the area behind, which is in the mid-scalp area, is this shock loss? shedding period? or the normal genetic hair loss pattern? and how can i tell the difference between them?

2. If your answer to question no.1 is shock loss then can you tell me what is the solution for it? and is there any chance of hair regrowth for those that were lost? (By the way i have used propecia for 12 months before a few years ago but with no success and i experianced terrible testical pain)

If you were not on finasteride when you had your FUE procedure, it would almost be certain that you had shock loss. You should have worked this through with your doctor prior to having the surgery. I always assess the status of the hair behind the transplant recipient site and if it showed significant miniaturization, one could have predicted this outcome. I would need to see you and understand what the loss area shows now.

I would also have considered lower doses of finasteride, which may have allowed you to use the drug without the complication. One quarter of a pill finasteride (0.25mg) can be effective at minimizing hair loss from shock.

Does Sensitive Skin Mean Sensitive to DHT? – Hair Loss Information by Dr. William Rassman

Hello Dr. Rassman.

I have a theory, can the type of skin (texture, etc) determine if a person will go bald? Sensitive skin equals sensitive to DHT? For example a Caucasian male can freckle easy and have allergies easily.

I suggest that you scrap that theory. I assume by “sensitive to DHT” you’re equating it to mean genetic hair loss. What is below the skin and what is above the skin (what it looks like) are very different things.

Folliculitis and Hair Loss in the Crown – Balding Blog

I have folliculitis which has caused hairloss at the crown. Will minoxidil regrow this hair? If it does, will the hair be lost once application has stopped or will the hair continue to survive on its own as my dermatologist says I have no MPB only the folliculitis.

There is a condition called folliculitis decalvans, which is an autoimmune disease that often reflects extensive hair loss. First, one would have to make a diagnosis (something I can not do without seeing you myself). If it is just a small patch from the infection of a few hair follicles that caused hair to fall out and does not reflect an underlying disease, then it might regrow. Minoxidil may help. I can’t say much more without knowing much more about you, what it looks like, and more of your history with the folliculitis. There’s a lot of info and photos of folliculitis decalvans here.

Some Months My Transplanted Hair Looks Thick – Hair Loss Information by Dr. William Rassman

I had a hair transplant nearly two years ago now. every 6 months or so it looks thick and occasionally some months it doesnt. Now becuase the transplant to place maybe as the rest of my hair was at a resting phase, could the hair transplant section and the rest of the hair always be at a different rate of growth as in resting phase. I was wondering whether this was the reason for certain months it looking thick and more hair and other months lacking as much hair. Also as the transplant section isnt as dense and compact could it be that when some hairs fall out in order to be replaced it just isnt as thick?

The general hair cycling is somewhat random when we look at it. I can not imagine why your hair looks better sometimes than other times. We’ve all heard of a “bad hair day,” so maybe that is what you are talking about. Are you doing anything different with your hair during the periods in question?

My Dad, Grandfathers, and Brother All Have Their Hair — So Why Am I Balding? – Hair Loss Information by Dr. William Rassman

My dad has hair, both my grandpas have hair, and my brother who is four years older than me has hair. I don’t know if cousins matter, but I have four male ones that all have hair too. Its not just hair though, all these family members have full heads of hair, there is not one male in the my family who is balding, and yet im 18 and seem to be losing all my hair. Im trying to except it, but im curious if there is anything I can do?

The expression of the balding gene does skip generations, though I am not sure how many it could potentially skip. The more generations that are skipped, however, the less likely it will be. I’ve written about it before here.

As for what you can do — I’d see a doctor and ask about a prescription for Propecia. If you have early genetic hair loss, the drug should halt the progression for some time.

Can a Man Have a Female Hair Loss Pattern? – Hair Loss Information by Dr. William Rassman

It is in my opinion by looking at my pattern of hairloss that it resembles a female type pattern of loss. I have compared myself to both the Norwood scale and the female Ludwig scale. I’m curious if this is possible and have you seen it with any of you othe patients?? Just curious. Also could you reccomend a hair specialist in the New York, New Jersey area. Thanks in advance…

Yep, it’s possible and I have seen this condition in some male patients. In the NY area, I would recommend Dr. Robert Bernstein.

Did I Experience Shock Loss Months After My Hair Transplant? – Hair Loss Information by Dr. William Rassman

Hi Doctor,

thank you for the fantastic work you do helping people with their hairloss concerns.

I have had 3 surgeries (1500, 1500, 1000). The last being back in Febuary and the one before that about 11 months prior to the last. Anyway, Im on finasteride 1.25 mg daily, and i use regaine foam daily.

My question is that my hair was looking remarkably thick in the front and temples up until a month ago (2 months after my last surgery) . The last 1 month for some reason has led to some thinning in front….I may be crazy but it seems like Ive lost a decent amount of coverage/thickness. Im chalking this up to post surgical shock…but the hairs that are being lost are full length hairs. Is this normal 3 months post op ? My hair was looking really good!…and now…just “pretty good” ;) I hoping that my new transplanted hair plus my shock loss hair will grow and get me back to “great” status.

Also, im considering going on .5 mg avodart to replace the finasteride. Ive been on Fin for 7 years…and have recently felt that the results have been stagnated (also, do you recommend 0.5 mg Dut vs. Fin 2.5 mg?)

Maybe you are having shock loss… or maybe you are having ongoing natural genetic balding. Usually shock induced loss occurs in months 4-5. I don’t really know what is happening to you without an examination.

I generally discourage Avodart (dutasteride) at this point, because it has not yet been FDA approved for the treatment of hair loss (some doctors prescribe it for off-label use). In patients that are seeing Propecia not retain as much hair in the later years, doubling the dose of finasteride (1/2 of a 5mg pill, provided there are no side negative sexual effects) has been what I’ve recommended. Talk to your prescribing doctor about this. Keep in mind finasteride is not going to completely stop genetic balding.

Jon Gosselin’s Hair Transplant Looks Awesome! – Hair Loss Information – Balding Blog

People MagThe story that everyone seems to be talking about lately in the entertainment world is about Jon Gosselin. I’ve got at least half a dozen magazines in my office lobby with his face on the cover. Last week, Life & Style Magazine even had a blurb about his hair transplant (see it here). This week, Jon’s the cover story on the June 15 issue of People magazine, and I’ve scanned a great image from that issue and posted it below. A few weeks back I wrote about Jon’s personal troubles only after receiving a number of inquiring emails, and while I’m still not going to discuss that stuff further, I don’t see anything wrong with taking the opportunity to use his increased spotlight as a way to see photos of his awesome hair!

It should be no surprise that Jon Gosselin has had a hair transplant — there was an entire episode of Jon and Kate Plus Eight devoted to it — but as I’ve yet to get good After shots from Jon, I’ve been relying on paparazzi shots and magazine photo shoots to see how it turned out. I Shaved My Head and Am Monitoring My Hairline – Hair Loss Information by Dr. William Rassman

I recently shaved my head and began closely monitoring my hairline. There are a few follicles that stood out from the rest because they were isolated and a bit lower than the front of my hairline. I recently noticed two that had stunted growth and formed what resembled a blackhead. After “popping” them the hair has still not grown back. Do hair follicles have a dormant stage or does this signal that the follicles have died and that I am beginning to lose hair?

If you monitor your hairline this way, I would suggest that you (1) always lift your brow so as to see the highest crease and (2) collect serial photographs with your brow lifted. Good luck to you, as the quest may take a few years to understand what is happening to your hairline.

Panic Attacks and Hair Loss? – Hair Loss Information by Dr. William Rassman

Sometimes, i panic (over nothing really), and because of that, it gets difficult to breath and in some cases, i feel anxiety and get dizzy (because of the panic). Could that contribute to hair loss? I am a male diffusing on the scalp (not sides), shedding very much, and shedded stripes have a clear (big) white bulb at the end.

Yes, you can certainly lose hair from stress caused from your extreme anxiety. It could precipitate your genetic balding earlier than it would’ve, as is generally the case for men that have stress induced loss. It sounds like you need a good, caring doctor to get your anxiety under control.
