Dear Doc,

I’m an 18 year old male and worried that my hairline’s begun to recede. I don’t have any particular family history of balding, both my grandfathers had most of their hair and so does my mother’s brother. Its only my father who is balding (he is 50 and has lost lots of his hair on the top of his head).

I’ve noticed that when I run my hands through my hair I occasionally lose one or two. Also when the hair is wet i tend to lose a few if I run my hand through my hair. I don’t see more than two or three on my pillow when I wake up.

I’ve always had a high hairline with temples higher than the rest of my hairline, but I think its starting to move back slowly.

I also have little red spots near the receding areas of my hairline, almost like acne (I have very little acne on the rest of my face). Looking at my pictures, would you say this is just a mature hairline or the start of the balding process. Also sorry for the long e-mail, but I’ve tried to list everything I can. I’d appreciate some advice on what I can do, as my hair is still very much part of my identity, and I don’t want to spend the next couple of years worrying and counting hairs.

It does look like you are developing a mature hairline, but there is also a sign of some thinning that goes further back than just the leading edge, suggestive of miniaturization behind a maturing hairline.

You need to get your scalp mapped out for miniaturization and see if there is thinning behind the leading edge and how far back it goes. This is suggestive of early male pattern balding at a level where a drug like Propecia (finasteride) can stop it. See a doctor who cares about you and what may be early balding.

I’m a 24 year old male and over the past couple months the front (and to some degree various spots behind it) of my hairline have fallen out at a rapid pace. Now I’m not sure if this is due to stresses on my body (lost 50 pounds in 4 months) or just MPB (limited history of it in my family, or even a maturing hairline its hard for me to tell. I’ve already been to a dermatologist and she wasn’t sure and doesn’t check for miniaturization so while i search for one that does i would greatly appreciate your opinion.

I’ve included a couple pictures of the front and side. You may use these on your site if you wish, i don’t mind.

The thinning can be related to stressed induced by the rapid weight loss, a known cause of hair loss in both men and women. The short term loss may be reversible in your case if the miniaturization is extensive and the hairs are still there, just miniaturized. Stabilizing the weight situation (work that out with a good nutritionist) and taking Propecia (finasteride) might help. You won’t know for a good year or so, but you might see a slow reversal towards normal. It is worth a try. Get your hair and scalp mapped out for miniaturization so that you have a basic number for comparison a year from now.

Hello Doctor,

I can’t remember if I asked this already but it didn’t seem to be answered. I was curious if you would characterize the actor Jon Hamm as a textbook NW2? He clearly has a mature hairline at least, but I can’t see any frontal balding.

I recently got a hair transplant and I believe my hairline will mimic his, which is quite promising considering I am about five years his junior. My wife seems to think he’s very handsome, and he clearly does not have a juvenile hairline, so there’s hope for us NW2’s eh?

Jon HammMany of the photos I found of Jon Hamm show his hair longer and covering the corners of his hairline (like this one), but I was able to find a photo that shows what appears to be a Norwood Class 2 pattern (like here or here). In other words, he doesn’t have any real hair loss, just as you described. He appears to have a great head of hair.

To be sure of that NW2 though, I’d need to see his wrinkled brow and from there I can make the appropriate measurements from the highest crease of his wrinkled brow… or see photos from when he was 10 years old and compare the shape of that hairline to those that show in the reference images. But for the sake of this blog, I’ll just take it at face value.

Each case is different, and I can’t give you a 100% guarantee that Propecia will save those hairs in your hairline. I don’t even know where your loss pattern is or how old you are. I can tell you that the younger you are, the better the chances for reversing or really saving the hairline with the medication. I’ve found that men under 25 years old in your situation do better with Propecia than men over 35.

Get your hair and scalp mapped out for miniaturization to establish the baseline measurements you need. Then you can go back 6 months later with another mapping to actually have metrics and see where the Propecia helped.

I’m Noticing Small Hair Growth at the Hairline After 3 Months on Finasteride – Hair Loss Information – Balding Blog

I am a 22 year old male, most likely a Norwood Class 2. I have recently begun taking 1.25 mg generic Proscar, which I started about 3 months ago. I have recently noticed there appear to be some new very small, light, and almost invisible hairs growing at the corners of my hair line. When I say these hairs are almost invisible, I mean that they are very, very hard to see, but in the right light they do show up.

Could these hairs be growth from Proscar? Does initial regrowth usually begin as very fine, light hairs, and later grow in as thicker hairs? Finally, I have noticed that a small number of my thicker hairs are not always uniform in width, one end of a hair my be extremely thick, while the other is thin, and even a different shade of color.

Let’s not check this out hair-by-hair. If you wait out the full 8 months to 1 year you will see just what you are getting. Patience is a virtue in this business. Do you ever actually see grass grow in real-time? I mean, to see it you need to take a step back and wait to see the change over a few weeks for the grass (and months for the hair).

It would be a very rare thing for you to see growth at the hairline, particularly after just 3 months. But I suppose it isn’t out of the realm of possibilities…

I have written to you before about my hairline; I was about to make my TV debut a year ago and was a little freaked out about my hair. My hairline started maturing when I was about 15 and at age 17 all recession stopped. I had some hairs along the edge and corners of the hairline that I suspected I would lose one day. About 2yrs ago I noticed some slight changes as those hairs did miniturize. I am now 38yrs old and I decided to see Dr. Bernstein in New York a couple of weeks ago. Most people laugh when I tell them I am worried about my hair. It is full and thick, and I wear it longer as I always did. Dr. Bernstein said I was on my way to a norwood 3, but I still had a great hairline. He put me on finasteride, and rogaine. He did not map my scalp for miniturization, and said he would not suggest surgery at this time. So I guess I am a norwood 2/3. My hairline looks very much like Johnny Depp’s or Robert De Niro’s. The thing is; I don’t like the way my hairline looks. My hair is almost black, and my skin is very fare which accentuates the hairline. If I decided to get a TP, I wouldn’t need alot of grafts, but it would give me more confidence. Any suggestions?

At 38 years old with a stable hairline and no significant miniaturization, a cosmetic lowering of the hairline starts a process that could impact you as you get older and possibly lose hair. If you want a hair transplant to lower a non-balding look, then that decision will be in the hands of you and an honorable surgeon who will not do harm to you if he transplants you. I need to get your opinion on a photo I found. I was researching something unrelated to hair and I found a single photo with no info provided. I have no idea anything about this particular surgery, including doctor and graft count. But the hairline struck me as really weird looking. Why might a doctor transplant this odd shape? Just poor skill or is there a valid reason? I’ve had a successful hair transplant a few years ago so I’m familiar with how a transplant should look right after surgery, and this looks gross.

You can click the photo to enlarge, but note that this is NOT how a hair transplant should look. I have many criticisms of this work. The frontal line has a series of peaks that will appear like a picket fence when and if the hair grows in. Behind the leading edge, the grafts were placed in rows and that will show in the final result. The grafts appear to be large and elevated, which means that there will probably be pimple-like elevations from which the hair grows.

I would be interested in seeing the final result, but based on this photo I’d expect it to look awful. I’d love to know who the doctor is that performed this atrocity, so if anyone has more info please let me know.

Actor Jon Cryer’s Hair Loss? – Hair Loss Information – Balding Blog

If you haven’t yet had a chance to watch it, the November 23rd episode of Chuck Lorre’s _Two and a Half Men_ has a scene with Alan spraying on a fictional “Captain Terry’s Spray on Hair”, taking some untested hair growth pill and going on J-Date. These are all things I have done in my life and I found it particularly amusing. It’s interesting that they wrote this in since Jon Cryer seems to use some kind of concealer in real life. His hair looks too thick on screen.

Of course, I suspect a lot of actors use concealers, even the ones who do not have balding.

You’re not alone. Tabloid site TMZ did a quick double take on Jon Cryer’s hair situation about a year ago — see here. He could very well be using a product in his hair, or perhaps the producers of the show have him do it to maintain consistency or youthfulness in the character. There does appear to be something going on with his hair, but I really don’t know for sure.

And yes, I’d guess that your suspicion is correct about many actors using concealers. A youthful look is something many people strive for, and of course those celebs have many reasons to look their best.

When the juvenile hairline matures, what happens to the characteristic of the hair on the juvenile hairline as this happens, does it miniaturise? Does it all go at once or one hair at a time? You suggested these hairs go through aptosis but during this process would these hairs become weaker every cycle?

I have not seen miniaturization in those undergoing the shift from a juvenile to mature hairline as a rule. It just falls out without miniaturization, but this is not an all or nothing statement as miniaturization could occur.
