Transgender Male-to-Female Taking Dutasteride – Balding Blog

Hi Dr. Rassman,

I am a genetically born male, who just turned 23 years old. I am a transgendered and just recently began taking hormones 2 months ago to transition into a woman.

At age 21, as a male, I noticed my hair thinning in the temple regions. My mother father began balding pretty heavily in his 20’s. I knew I was transgendered but was unable to afford transitioning and beginning hormones at that age so I went to two dermatologists and was prescribed Propecia both times. I was on it for a little over a year. Surprisingly after a year I seemed to have less hair in the temple region and front hairline region. I can’t say it didn’t work because who would have known if I would have lost that hair anyways and Propecia helped me loose less? I’m not sure. My hair is definitely thinner and some family noticed but it’s not “socially” noticeable.

Two months ago I stopped taking Propecia I began a female hormone regimen to transition into a female begin that I am transgendered. I take 6mg of Estrace as well as Avodart daily. I also get a monthly injection of Depot Lupron 7.5mg used to treat advance prostate cancer in men and is similar to a “chemical castration”. So needless to say my testosterone levels will be taken WAY down and hopefully Avodart will absorb the leftover DHT in my system. However, at this point, now living as a female with female hairstyles I still have an issue because my hairline has started to mature in a masculine look and it’s very thin in the temples and at my hairline.

With my situation, and based on the medication’s I am taking – should I expect more hair growth than I did on Propecia? Honestly I think I need a forehead reduction but this could be particularly tricky with my thus receed hairline. What are you suggestions, or observations? Should I give it time? I CAN NOT loose ANYMORE hair and I REALLY need regrowth! I haven’t tried Rogaine because I was told in doesn’t work in the temple region on genetic men – but now that I’m on female hormones does that change that?

PLEASE HELP ME DOCTOR!!!!!!

Thank you SOOO much for this wonderful opportunity!

You have chosen non-surgical, hormonal treatments for eliminating your male hormones. Dutasteride, although not yet FDA approved for regular male pattern baldness, is studied and believed to be more potent than Propecia (finasteride) for prevention of male pattern baldness. You are right in that using other anti-androgen medications are also important to slow down the process of your male patterned hair loss. I am not sure whether Rogaine can add anything to the medication regimen that you are currently on. You have to know that it is unlikely that you can grow your receded hair completely back and medical treatment can only prevent further loss and in some cases thicken the miniaturized hair in thinning area.

For treating the receded areas that are not recovered by the maximum hormonal treatments that you are doing and to make a female hairline, the available options include hair transplantation and possible hairline lowering procedures. The latter runs a risk if you should continue to bald from your genetics, while a hair transplant has been successfully used for people like you who have male pattern hairlines and wish to migrate to a feminine hairline. Please consider coming in for a consultation so that a Master Plan will have will be clearly defined for you.

Why Does Hair Character Change As We Age? – Hair Loss Information by Dr. William Rassman

In an earlier post you said “With regard to the change in hair character from straight to wavy, it is not unusual for hair character to change as we get older…”

Why does this happen?

As our body ages, we see changes in our hair character. The hair of a newborn is very soft and very fine, the child’s hair becomes thicker and as we age, we might peak with our hair shaft thickness in childhood or early adolescence. As we move into adulthood and then older ages, the hair again changes. All of these changes are genetically programmed. Unfortunately, we do not know much about the programming, but one day we will learn more as we command the issue of hair growth, balding, and hair thickness.

Melanocytes and Hair Color – Hair Loss Information by Dr. William Rassman

I have a question about greying process.

Scientists say that someday in the future it would be possible to reverse grey hair back to its original colour, because grey hair still have melanocytes but they are reduce in number. What do you think how long melanocytes survive in grey hair, and is it true that grey hair still have melanocytes but white hair have no more melanocytes.

DO you think that it will be possible to reverse also those hair which have no more melanocytes. (white hair?)

The reason that your hair becomes gray and eventually white is that over time you lose the cells that make melanin, the pigment responsible for hair color. It is very likely that we can reverse the process of graying hair with gene therapy in the future by prolonging the life of melanocytes.

I can not predict how soon we can have a method to reverse the color of gray hair to its original color and I recommend that you use dye to color your hair for now, rather than waiting for scientists to come up with that solution. Be careful that you use a professional quality hair dye and follow the instructions provided with the dye. I don’t want to see you back here with questions about treating chemical burns and experiencing hair loss!

Growing Long Hair After a Transplant – Hair Loss Information by Dr. William Rassman

Dear doctor,

can you tell me why every hair transplant picture that I see, the hair is normally short and combed back. I understand that this is to demonstrate how natural the hair line can look,
however, can people who have the opp grow their hair long? If so, do you have any pictures?

Kind Regards.

I believe the hundreds of patients shown on the New Hair Institute website have just about every imaginable hair style. This is all individual preference. A good hair transplant means that you decide how to groom your hair, not the doctor. Short, long, combed forward, back, to the side — whatever. I have even had a few patients visit after getting a crew cut.

It Feels Like Something Is Moving In My Scalp – Balding Blog

Hi doc,

I am a 26 year old black female and I have been experiencing hair loss. For the past year or so, I’ve noticed that my hair has been thinnning (front of scalp), and then lately, I’ve notice
white clumps around my hair shaft. And sometimes, I would feel as though something is moving in my scalp (not lice). And its getting worse. I have set an appt. with a dermotalogist, but I
doubt it they will find out what’s wrong with me. I’ve tried everything and nothing seems to help. I wash my hair every week and I get a relaxer every 4 months. I would greatly appreciate
your advice-thank you.

You need an examination. Parasites like lice are a primary diagnosis, but your visit to your dermatologist is the right first step.

My Hair Turns White Then Back to My Natural Color – Hair Loss Information by Dr. William Rassman

Hi, I’m 22 yr. old female, and I’ve had a few strands of white hair since I was like 15 yrs. old. The thing is the white hair doesn’t start growing in white immediately. First it grows in my natural hair color, then turns white, the afterwards it turns black again. it’s really weird, And i used to pluck it but now since only the root of its white, i don’t even bother anymore. what does this mean?

First of all, plucking of the hair is not good as it eventually kills the hair. I have heard of dark hair that lightens (sun causes bleaching of the hair), but I have never heard of hair that turns black after first turning white (other than in a person with hormonal altering brain tumors). My first reaction is wondering what are you doing to your hair to cause this phenomenon. Unfortunately, I have no advice for you as this is a new one for me.

After Moving and Exercising Daily, My Hair Feels Stronger – Hair Loss Information by Dr. William Rassman

Dr Rassman,
I am 27 years old and have noticed thinning more on my left side, particularly the left occipito-parietal area, higher up nearer the crown. Since returning from India ( there for 4 years) I have noticed new hair on the frontal hairline, which is more on the left side. Also, hairs are growing longer but not thicker. I jog every other day and have noticed my hair feels a little stronger. Is there any medical basis for this or is it all just psychological?! Thanks

Hair loss in genetic balding people is impacted by stress or even the absence of it. Maybe that is what you are noticing.

Hair Loss InformationLaser Resurfacing for Scalp Scars – Hair Loss Information – Balding Blog

Dear DOC, what do you think of LASER FRAXEL SR1500 resurfacing of scalp scarring?

Block Quote

Any resurfacing of the scalp will damage the pigment cells and cause the color of the skin to lighten. It may also change the character of the skin surface and this may be more true when used for the treatment of a scar, which will require a deeper burn to get below the scar. I am not excited about resurfacing the scalp skin. I am not an expert on this though, so do ask your doctor for what his/her goals are — and specifically if the potential problems I discussed here are reasonable concerns.
