I was wondering if you could shine some light on this in effort to give hope to NW5+ men. I think a great option for high NW men would be to obtain a low density (30/cm) transplant and supplement it with SMP.

What do you think? Also, with this strategy, would it be possible to keep hair at a 2 guard without it looking funny/weird?

A low density hair transplant with SMP (Scalp MicroPigmentation) in the scalp may or may not be enough to achieve a fully normal appearance. There is certainly a threshold where where too few transplants will not be adequately augmented by SMP.

My question is in regards to the SMP (scalp micropigmentation) procedure. I’ve been taking propecia about four years now-started when I was 18. My hair has halted the balding process, but I cant tell it’s slowly creeping back up, which is what I expected anyways. I’m very interested in the SMP procedure, since scarring (even minimal) and a life-time commitment to propecia just isn’t for me. My questions are:

1) As opposed to hair transplant, how old does one have to be to become a good candidate for SMP?
2) If the procedure was done, would constantly shaving the head destroy the tattooed “hair”?
3) If after SMP, one would to stop propecia and lose their hair that was retained from the drug, would the loss of natural hair be noticeable even with SMP?
4) Lastly, are there any more recent photos you could share with us other than the ones already listed?

Any age above 18 years old will work for me if the patient understands the positive and negatives associated with the process. There’s a list of frequently asked questions at the SMP site.

Many people will shave the head every day or every other day. This is a personal preference. Shaving your head shouldn’t destroy the SMP.

If you stopped the Propecia, you would lose all benefits of the drug and the hair loss will play ‘catch up’ to what it would have been.

We only post the photos of patients that grant us permission, but we keep updating the SMP site with new photos all the time. Keep watch on it.

SMP (Scalp Micropigmentation) must be protected from exposure to strong sunlight, so can self-tan products be safely applied to a scalp that has undergone SMP – i.e. chemical reaction to pigments? Thank you.

Tanning products are not similar to sun exposure so yes, you should be able to use tanning products after Scalp MicroPigmentation (SMP). I would wait a few weeks before doing this so that the reaction to the skin needle wounds are well healed. There should be no chemical reaction of the tanning products to the pigments used with SMP once full skin healing is complete.

I read about Scalp MicroPigmentation (SMP) and I think this might be a good choice for me, because I am not candidates for hair transplant surgery. However, I am waiting for the day that the hair cloning technology can be used by people like me. My question is: Is SMP a barrier for future hair cloning?

When properly designed, Scalp MicroPigmentation (SMP) can work with hair transplants and possibly with a good cloning process once cloning is practical. Hair cloning is “not ready for prime time”, but I expect that once it is viable, any good cloning procedure must restore a normal appearance, including the leading edge of the hairline which is critical to a normal look.

This process is often ignored by tattoo people who do SMP type of work, partly because the patient pushes a hairline design that may not be normal, or the tattoo person does not appreciate the blend between the natural balding process, the maturing hairline, and what they should look like.

The influence of someone like Dr. Pak (who designs all of our SMP work) is critical to the excellent results we obtain. I would imagine that the same conditions would apply to the cloning process.

I had the idea of combining SMP with the occasional use of hairpieces to basically give me the hair style options of any non balding man (at a much higher price of course.) This way, each would get rid of suspicion of the other; being able to grow hair and being able to shave it. My question is, would the glue used to bond the hairpiece to the head ever damage the SMP? I’d never excessively use hair pieces- just now and then for special occasions.

The initial laser-reversible pigment we obtained was tested on a very limited basis and in small cases. Unfortunately, we were unable to obtain more than the test supplies and have been unable to find a substitute… so it’s not something we pursued much further.

We currently only use permanent pigment in SMP sessions.

I have several scars on the back of the head, a recent procedure gave me yet a big new one to cover and deal with..

I am concerned about the look, once my hair is wet- these now are very visible, unnatural and difficult to explain-a subject I’d rather not discuss at all. Is the Micro Pigmentation a possible “fill in fix” to these scars, and if so, how do I deal with the way my hair color will look in say, 10 years when I am completely grey/white and thinning yet even more in the back? Matching skin, hair color is important.

I currently have dark brown/grey hair which I dye for the time being. Thank you.

The pigments have their limitations, but are particularly good for men who want their hair shaved to almost scalp level (closely cropped). You are correct that as you gray, the pigment may produce problems, but usually these pigments gray a bit anyway. I would have to see you, at least see the pictures of your scars and their locations before rendering more thoughts.

Repair of scalp scars takes experience and your situation of having a surgical correction that made it worse is common when the surgeon does not really understand scalp repairs. I have seen many scar repairs by well known plastic surgeons and still they get wider after the surgery. If you are in the Los Angeles area, there is no substitute for a good examination, as it may be possible to fix these scars.

Regarding the use of SMP for creating a thicker look in those with existing, thin transplants and limited remaining donor hair….are you still creating simulated follicle “dots” or are you “painting” the whole scalp. If the latter is required for reasonable cosmetic difference, it seems like a really poor choice in terms of any unavoidable future hair loss (say, chemotherapy induced, etc).

The way we do SMP (scalp micropigmentation) is by creating individual “dots” that mimic a shaved scalp and an end view of the follicles. We don’t “paint” the scalp.

If you should lose your hair from chemotherapy or further hair loss should develop, it should not matter because SMP can be performed over the entire scalp. In some men, shaving the head is a good option and allows you to exploit the shaved look that SMP offers. If a man lost his crown hair, we do not just put the pigment over the bald crown, because that would look strange (it would look like a person shaved the crown and left the rest of the hair long). For women who keep their hair longer, if they go through chemotherapy but the recovery only has a partial return of their hair leaving it thin, then we may consider doing SMP to make it appear thicker in the areas where thinning is a problem.

Since there is definitely a limitation of SMP (mainly the life-long shaving of your scalp in men who want that look) we are highly selective on the clients we choose for this. There must be maturity and a clear understanding of the limitations of the process. For those who have had chemotherapy, SMP may be an option to make the hair appear thicker, but the decision to undertake this process must be clearly understood. Yes, this is a good option… but you must get educated on the facts that go along with it. Remember, this is a permanent decision so it needs to be thought through carefully.

Here is a 22 year old in your similar situation — Client 17. He started losing his hair when he was 16 years old and had worn a hat everywhere he went. After the SMP procedure he told us he went out for the first time without a hat on! SMP is not for everyone, but it does have its use in certain segments of the balding men.

Here is a question on the Scalp micro Pigmentation. Once a patient decides to do a cosmetic pigmentation especially on certain parts of the scalp, but not the full scalp, how is he going to look when his real hair will become gray eventually? Do you use a permanent or a temporary pigment?

I think we’ve answered this before, but in general, we do not perform SMP to certain parts of the scalp since as you point out, it would not blend in with the changing of hair color. For example, we sometimes leave the areas around the ears and neck.

Scalp Micropigmentation is permanent. If you want a temporary solution, I’d try Toppik or DermMatch.
