I was wondering if you could expand on your entry “Could Transplanted Hair Fall Out Months After a Successful Procedure?

Does all of the transplanted hair fall out or just some of it? How long after the HT have you seen this occur? From what you’ve seen does it happen in younger or older patients? Most importantly can you re-transplant new grafts into the area? Would the same thing happen again or could there have been a mistake made during procedure?

I am not sure about your question. The newly transplanted hair usually falls out in a month or so after the surgery and it comes back 2-5 months after in waves of hair growth. Once you’re past the 7th month, everything should be stable and your transplanted hair should be there for the rest of your life. If transplanted hair falls out, it may reflect hair that was not taken from the permanent zone and in that case it may reflect an error from the surgeon.

At least 5-10 years ago I had multiple sessions of hair transplants. All these years later, I am still getting scabby type growths at up to dozens of the implant recipient sites.They dont hurt ,but they feel annoying-as they feel like small crusty raised bumps that I have to kind of pick and tease off since hair is growing up threw the center of them.I am fair skinned Irish so I am very careful not to get a sunburn on my scalp or it makes this problem MUCH worse .Any advice/product or Rx recommended?? I would have to see you and look at your skin to understand the problem. Flaky skin (dandruff) can be something brought on by a scalp surgery. Zinc and tar based shampoos will work to control this. You can buy this in any neighborhood drugstore.

Haven’t Seen Much Growth 7 Months After My Hair Transplant – Hair Loss Information – Balding Blog

Hi,

I had a hair transplant about 7 months ago and I really have not seen any major improvement. I had the transplant of the hair line. What has happened is that some of the transplanted hair never fell out and has continued to grow but I have seen no regrowth where the transplants shed.

Is this a failure or will it still grow.

My hairline is still a little red.

At 7 months, you should see growth from the transplants. It sounds like it’s possible that your transplants grew in right away (seen in 5% of patients), but you may have experienced shock loss in the surrounding hair, leaving you no better off than before you started. As for redness, it should’ve subsided months ago. I don’t know enough about your procedure (number of grafts, hair type, technique used) or your skin type to be able to reflect on the persistent redness or if the transplant was a failure or not. I would discuss your concerns with the physician that performed the surgery and use the pre-operative photographs when you discuss this with him/her.

I had a hair transplant about 7 months ago. What I noticed is that the transplanted hair which never shed has remained but I have yet to see any other growth. The hairline looks sparse and not what I was told that they would achieve, which is thickness. Can the transplants still develop or will I expect to see no more improvements.

At 7 months after surgery, I would expect to see most of the growth. Wait a few more months and if there’s still nothing, then ask your doctor what went wrong (I am taking what you said at face value).

hello doc, I have to questions

i recently had a HT via FUE on thursday, today is tuesday so it has been 5 days so far. I have been wearing a hat and realized that it was breaking off a few transplanted grafts. When I look closely the stub of transplanted hair is still in my scalp but the rest of it has broken off. Do you think I killed my grafts this way?

Another Question. I washed my hair on friday night (next day after ht procedure) but then i read some where that I wasn’t suppose to wash it for 3 days. Could washing my grafts with a sponge and water next day after HT damage the grafts completely?

HatI suppose if you see stubs of hair follicles out of your body with bulbs on them, it could reflect grafts and if that’s the case, what you lost will never grow. I’m really not sure what you’re seeing. A tight hat can pull out grafts, though the grafts should’ve been secured by the 5 day mark. I don’t know enough about your surgery — like where you had your hair transplant or what was really done.

You can wash your hair 12 hours after the surgery. As always, I am happy to answer all questions, but sometimes questions like these bother me. Why are you not asking this to your doctor? Why haven’t you got instruction on how to wash your hair after a hair transplant? Special instructions should’ve been supplied to you for the washing technique.

Progaine Shampoo – Balding Blog

Doctor, you mentioned in one of your recent posts that you supply one bottle of Progaine Shampoo to your patients after the surgery. I use Progaine a lot but can only find the Progaine Volumizing Shampoo available. The other Progaine products do not seem to be available at this time over the internet or in stores. Do you know why only one Progaine product is available at this time? When would the other products under the brand name be available again? If you could provide any information, it would be appreciated very much!

Progaine is just a volumizing shampoo like any other volumizing shampoo. It does not grow hair. There were other formulations, but I believe they’ve been discontinued. I just did a quick search online and you’re right -only the volumizing shampoo seems to still be for sale.

But… um… I must’ve had a temporary memory lapse when I said that Progaine was what I give to patients after their hair transplant surgery. I used to provide Progaine, but for years now I’ve provided GraftCyte shampoo (which contains copper peptide to promote better healing). You can find more info about that here. Sorry for the confusion!

My Hairline Was Transplanted Too Straight – Balding Blog

Hi Dr Rasman – many thanks for the very appreciated blog.

2 years ago I underwent an aggressive hairline FUE procedure – 3,000 grafts all in the hairline with temple closure.

I’ve recently started shaving my head and, despite the white-dotted donor area, I feel much better about myself. I’d love to continue shaving my head – the problem is the recipient area. My recipient hairs, though not particularly dense, are much thicker than the natural hair behind them and begin with a very sudden, ‘flat’ line. The texture of the skin is bumpy and *may* form a small ridge at the start of the hairline; it’s difficult to tell whether it’s scarring, or just hair follicles pushing up the skin.

If I were to remove the grafts, is there any hope of restoring a natural-looking, albeit bald area where my current transplanted hairline is? If so, what sort of treatments would you suggest?

Oh wow! 3000 grafts to the hairline and temples does sound very aggressive, if not impossible from my personal experience. The bumpy skin texture you describe can be scarring from your surgery and if it is scarring there really is no treatment for it. I am not implying you had a bad surgery, but hair transplant surgery is not reversible. Hair transplant surgery is permanent and you need to have a good foundation and understanding of what you are getting yourself into, as well as the long term complication and issues (such as scarring and shaving) before you regret the decision.

There are doctors who do 3000 grafts routinely in a small area of the frontal line and I have seen the work. The hairlines I have seen are straight and many times I doubt that the patient actually got the 3000 grafts they paid for, which makes me question the integrity of the doctor. These hairlines can be repaired by creating a good transition zone in front of the harsh tight hairline, but that assumes that the hairline was not placed too low in the first place. Often in some of these patients the hairline is too low, making a good repair very difficult. Each case is different and I would love to see you either in consultation or good digital photographs of your frontal hairline.

Some Months My Transplanted Hair Looks Thick – Hair Loss Information by Dr. William Rassman

I had a hair transplant nearly two years ago now. every 6 months or so it looks thick and occasionally some months it doesnt. Now becuase the transplant to place maybe as the rest of my hair was at a resting phase, could the hair transplant section and the rest of the hair always be at a different rate of growth as in resting phase. I was wondering whether this was the reason for certain months it looking thick and more hair and other months lacking as much hair. Also as the transplant section isnt as dense and compact could it be that when some hairs fall out in order to be replaced it just isnt as thick?

The general hair cycling is somewhat random when we look at it. I can not imagine why your hair looks better sometimes than other times. We’ve all heard of a “bad hair day,” so maybe that is what you are talking about. Are you doing anything different with your hair during the periods in question?

thank you for the fantastic work you do helping people with their hairloss concerns.

I have had 3 surgeries (1500, 1500, 1000). The last being back in Febuary and the one before that about 11 months prior to the last. Anyway, Im on finasteride 1.25 mg daily, and i use regaine foam daily.

My question is that my hair was looking remarkably thick in the front and temples up until a month ago (2 months after my last surgery) . The last 1 month for some reason has led to some thinning in front….I may be crazy but it seems like Ive lost a decent amount of coverage/thickness. Im chalking this up to post surgical shock…but the hairs that are being lost are full length hairs. Is this normal 3 months post op ? My hair was looking really good!…and now…just “pretty good” ;) I hoping that my new transplanted hair plus my shock loss hair will grow and get me back to “great” status.

Also, im considering going on .5 mg avodart to replace the finasteride. Ive been on Fin for 7 years…and have recently felt that the results have been stagnated (also, do you recommend 0.5 mg Dut vs. Fin 2.5 mg?)

Maybe you are having shock loss… or maybe you are having ongoing natural genetic balding. Usually shock induced loss occurs in months 4-5. I don’t really know what is happening to you without an examination.

I generally discourage Avodart (dutasteride) at this point, because it has not yet been FDA approved for the treatment of hair loss (some doctors prescribe it for off-label use). In patients that are seeing Propecia not retain as much hair in the later years, doubling the dose of finasteride (1/2 of a 5mg pill, provided there are no side negative sexual effects) has been what I’ve recommended. Talk to your prescribing doctor about this. Keep in mind finasteride is not going to completely stop genetic balding.

Throbbing Pain Weeks After Hair Transplant! – Hair Loss Information – Balding Blog

Hello,

I’m a 30 year old male who had my first hair transplant when I was 26. I flew to Thailand and had a 2000 graft strip excision procedure done. The first week was hell but the transplant turned out to be good, but in need of obvious density. I decided to return 4 years later and get another procedure. I was thinking of going to another doctor but the last time he provided great service even though I suspected the 2000 grafts could have looked better.

Fast forward to now, I thought I needed between 1000-1500 and he immediately said 2000. I didn’t think about until after the procedure that this is too much. After the 2nd procedure which I am currently 15 days after the healing process is not going as well as the first time. I will run down the issues and you can tell me if I’m royally screwed or not.

  1. My whole scalp is super tight. I can feel a constant tension ring from ear to ear and even my temples. When I lift my eyebrows all the way up I can even feel it. The most disturbing thing is every time I walk slowly up a flight of stairs I get a throbbing headache/pain which feels as if I’m going to pass out.
  2. The scar (trychophyic closure) is not healing. On the right side there is a indentation with a huge scab (I think that gauze stuff?) which is surrounded by inflammation.
  3. I have this dull ache in my throat when I swallow. It has never happened ever before and started after this procedure.

My questions are:

  1. Will this throbbing pain while walking up stairs go away?
  2. Is the dent and bump in my head going to heal? Is it swelling and or skin just pulled together from the wrong angle?
  3. Is there anything I can do to help the wound heal?

I’ve been using propecia for 4 years now, and just started 5 sprays of minoxidil on recipient area. Please help this is extremely disconcerting because I’m a triathlete who routinely has 3 hour workouts in the gym, and I fear I will never be able to compete again. I would try to talk to the doc thailand but his English isn’t very good and kind of sent me packing without letting me know if I have a normal future. Thanks for reading, I will also try to send pictures although it may be a few weeks (moving, camera packed, etc.)

From what you described, there are clearly problems. Over time things should get better, but you should get an assessment for baseline purposes to plot progress or lack of progress (objectively) over time. It would be best that you be seen by me if you want a better assessment. The exam will be free, but I’m in Los Angeles, so you’ll likely need to do some more traveling. Drugs like minoxidil and Propecia will help prevent further hair loss, but not reverse what has already occurred.

Without knowing more I’m just able to give general info, but here’s a try at your questions:

  1. Pain from damaged nerves may or may not get better depending upon whether or not the nerve was cut.
  2. Dents in wounds, may or may not get better as more healing goes on.
  3. Generally, there is nothing to do to help wound healing. Some readers will likely argue that, so please feel free to leave comments with your wound-healing advice.
