I would like to ask a question from my side. I had a hair transplant surgery 4 months ago. I was also advised to use minoxidil 2 weeks after the surgery in order to prevent healthy existing hair from falling off. Everything was pretty good until the end of the 3rd month where the minoxidil has caused me sort of irritation and pealing on my head. I stopped it for 3-4 days but i realized that the problem did not get away. Is minoxidil helpful to help the new transplanted hair grow up rapidly? What is your opinion about and what do you suggest me to do?

Furthermore, how long does it take for the new hair to grow and become strong? I realized that the hair that just popped up on my head first looked strong but now some hair are thicker than before.

Thanks a lot for your help and i would appreciate if i could get an answer to you the earliest convenient.

Minoxidil has no impact on stimulating the transplanted hair to grow. They will cycle when they are ready, usually 3-5 months from the date of the surgery. Your complications indicate that you should stop using the minoxidil (although my opinion may not be shared by your doctor). I never prescribe minoxidil post-transplant, because of the negative effects of it as you reported.

Hello Dr. Rassman and staff.

1st – Thank you for this Q/A blog. I am sure it benefits many people and speaks well of you and your organization to take the time to help people with their situations.

I will not bore you with a long story of my situation and contempt for the HT doctors out there (not so much the procedures, rather the blatant lies as to the outcome to expect), I am sure you have heard them all. I will try to keep it short and to the questions. I did search most if not all previous posts, but none seemed to directly hit the mark on my questions. If you can, please forward your thoughts:

Desired goal:

Shaved head with as little evidence of past misguided decisions. No desire what so ever for further surgery, even if given an iron clad guarantee that it would give full coverage and a great result.

Current Situation:

  • 42 y/o male
  • 3 “procedures” in 1991 in 3 consecutive months
  • Mini and micro graphs – approx. 240-300 graphs total
  • 3 scars in donor – approx 1-1/2 to 2″ in length
  • these scars feel slightly raised, particularly on ends
  • Can not tell the width of the scars, but appear to be fairly narrow (Not pencil thin as promised)
  • Continued loss after HT, probably a NW 5/6

Recipient area:

  • Darker coarser hair in “V” shape, all alone in front of bald head
  • Some dimples, approx 5 to 10
  • No raised graphs
  • slightly pink color at many of the graph sites
  • Wear a hat all time, so no tan skin in this area, very white and greater contrast for the pink and the coarser hairs present.
  • When I shaved a test area in the recipient area, the skin surface seems to show little dark marks with the pink still present

Questions:

  1. Can the raised scars in donor area be cut out and reclosed using a more aesthetic plastic surgery type closure that would truly leave a pencil thin and flat scar. Other scars on my body heal very well, thin and flat. These other scars even take on some what of a tan.
  2. I assume that the majority of the dark marks in the recipient area is the thicker, darker hair below the skin surface showing through. I believe laser removal of this misplaced hair would remove the majority of these marks – with the hairs gone, would that eliminate the majority of the pinkness also? If not, what can be done to address the pinkness?
  3. What affect do you feel the laser hair removal, a tan and microdermabrasion, if needed, would have on these marks and the over all skin appearance.
  4. If microdermabrasion is recommended, what negative effects can this or even laser hair removal have on the recipient area? Could I end up more scarred? Could the recipient area end up not taking a tan like the rest of the head?

I realize a lot of answers are dependent on ones own body and its ability to heal. I am looking for general outcomes as I find I usually fall with in “normal” ranges of just about everything. If these are questions you can and will address, but would like to see some photos, please let me know and I will obtain some pics of the recipient area in varying lights.

Again, thanks to you and your organization. I suspect that your group is genuine and in the field of HT, that is truly rare.

I would have to see you, or at the very least, I’d need to see some good photos as a starting point. Of course, I will keep your photos absolutely confidential (as a doctor this is not only a moral obligation, but a legal requirement under California law). If sending, please reference this post. Then we can talk over the telephone and discuss the matter. Face-to-face meeting is much more preferred though for your situation.

What you are looking for may not be possible. What you want is not standard and I would not want to represent anything that is not reasonable. The use of hair removal lasers can cause loss of pigmentation and microdermabrasion produces permanent changes on the skin quality. In other words, I don’t want to be adding to your suffering. Just from your questions, the caution I must give you is “buyer beware” and to suggest that you do your research before you start any process. There is no price for honesty and integrity and had you known what you were getting into 16 years ago, you would be in a better place. Normal balding is not freaky, as it is the norm for half of the male population to some degree.

My heart goes out for you, but it sounds like you need a personal consultation and even pictures won’t be the same as a one-on-one meeting between us. I look forward to hearing more from you and offering my assistance.

Hair Transplant 6 Months Ago – Now I See Small Bumps Instead of Hairs – Hair Loss Information – Balding Blog

Hello Dr,

I had a 3,000 grafts ht about 6 months ago. I’ve had growth, although hair is still fine in many places. My hairline and scalp, however, are the areas of concern. I can see a slight popping effect of some of the hairs on my hairline, there is also some ridging I think, and a lot of small bumps where hair is growing or supposed to grow. It creates a very unnatural look depending on the lighting.

Is this a result of having packed the grafts too close? Am I still healing? Anything I should do to speed things now? If this is permanent, are there repair options available? Would’nt want to miss an opportunity to fix this while it is still early. Thanks for your answer.

At 6 months, it is difficult to tell what you are talking about. In another 2-3 months, what you have will be evident as the hair should be grown. Ridging is very unusual for FUT (follicular unit transplantation). You should speak with your doctor about your concerns and your doctor should embrace the opportunity to speak with you on any issue, for it insures good doctor/patient bonding. In your particular case, it might conceivably take 3 weeks for the wound to heal so that any rubbing from the helmet will not impact the recovery. For a Follicular Unit Extraction (FUE) procedure, on the other hand, you can go back to wearing a helmet in 4-5 days.

Since shock loss is caused by inflamation and swelling, do you think ibuprofen (600 – 800 mg doses) could be as effective at preventing shock loss as propecia?

Do you think that there is any adverse interaction between ibuprofen and propecia? Thanks.

There is no connection between ibuprofen and preventing shock loss after hair transplantation. To my knowledge there is no connection between Propecia and ibuprofen either.

After a couple of weeks you should be able to get a regular haircut. Don’t expect that the transplanted hair will grow out in less than 5 months.

Experiencing Shock Loss Months After My Hair Transplant – Hair Loss Information by Dr. William Rassman

Ok here’s my situation, about 7 months ago I got about 1400 grafts performed. The standard strip style surgery. I’ve healed up well and I would say less than 2 months after surgery the hairs immediately started to grow. I also started propecia right before the surgery. I experienced shock loss immediately after the surgery and it ceased about 2 months after. I shaved my head completely and my hair grew in like crazy very thick and flowing. About 4 months after the surgery my hair started to fall out again and has since been falling out and progressively getting worse. I apply rogaine and copper peptide solution daily and am not doing anything any different. What can you suggest as a course of action?

Because of the protective effect against shock hair loss, you should’ve been taking Propecia (finasteride 1mg) prior to the hair transplant surgery — so the question now is whether you started it early enough (best a few weeks before the procedure). Accelerated hair loss associated with a hair transplant is most probably caused by shock loss. One must use the history and the association to point to the cause. You might be going through a hair loss cycle unrelated to what you are doing and there may not be anything you can do except to wait for your hair cycle to take its course. If there was a considerable amount of miniaturized hairs, then you would be a ’set-up’ for shock loss. Studying your hair miniaturization and distribution may point to more information in helping to make a diagnosis, but as I am not your doctor, I have no access to your patient records and can’t provide much more help than suggesting you post these questions to your doctor.

My Transplanted Hair Started Growing Immediately – Hair Loss Information by Dr. William Rassman

I had a hair transplant surgery 3 months ago. Immediately after my hair transplant,and im talking about the 1.5 to 2nd month, i realized my hair to grow up as now im on the 3rd month and my right hairline is covered with hair, something that does not happen on the right. My doctor also had confirmed this with me in front.My question is, is it guaranteed that my hair where the transplanted area is will continue to grow and generate new hair until the 6-8 month as expected?is there any chance to stop growing from now on?

Some hair will grow immediately in about 5% of patients and 5% of the hairs. You might still see more over time, but not more than 100% of what was transplanted. Nobody ever gets two for one.

Post-Op Questions After Hair Transplant 6 Weeks Ago – Hair Loss Information by Dr. William Rassman

I recently had a hair transplant (approx 6 weeks ago). The doctor cut 5 strips and transplanted into the vertex and front hairline, leaving the top of the head alone.

Since then I have noticed a lot of shedding (counting about 50-60 hairs in the shower as opposed to the typical 10 before). I realize there will be shedding after a procedure, however, I notice considerable thinning directly on the top of my head. A lot of the hairs that are shedding are long and thick.

I have 2 questions:

  1. Is it common for hair not in the donor site or the recipient site to shed so much?
  2. How much longer is this going to keep going on for?

Assuming you are male, then you may be experiencing shock loss, as it seems to relate to your recent hair transplant. Shock loss in young men often does not reverse. Were you taking finasteride (Propecia) which tends to protect against shock loss? Most importantly, I think these questions should be addressed to your doctor who performed your surgery and/or these things should have been explained to you in advance. At the very least, they should’ve been written out in your pre and post operative hand out.
