My Father Had Awful Transplants from a Dermatologist
I have been on finasteride for 7 years. I have pretty much maintained my same density over this period. A little more thinning in the front. I am now 36. My father had many transplants from a doctor who is a dermatologist. Not from a doctor that specializes in transplants. His transplants are awful. What should my expectations be if I wanted to get off the drug and try transplants? I am fair-skinned with wavy auburn hair, and my individual follicles are thick, but not dense.

The second part is that taking the drug Propecia is a different decision to having transplants. They can be complementary (see: Hair Transplant and Propecia Combination Results). As for expectations about transplants for yourself, meet with your doctor and have him/her develop a Master Plan with you, so that you have your needs addressed before the procedure.

What can I expect from a Class 6 pattern of balding?
The way this should be approached, is to analyse your hair with respect to its hair thickness (fine, medium or coarse, the coarser being better than fine hair), and the donor density. The number of grafts that can be safely moved in a Class 6 pattern of balding depends upon the donor density. The higher the donor density, the more grafts you have to move. A typical Caucasian male has ~110,000 hairs on their head with 50,000 Follicular units, or 2.2 hairs per cm square. There is a difference between FUE and Strip surgery maximum numbers as you can see in this chart: https://newhair.com/resources/#tab-id-4 so your doctor should measure your original donor density so that a plan for both the first transplant and the potential to cover the entire balding area with an additional transplant can be determined. A typical Caucasian with an average weight hair (thickness) can get reasonable coverage of a Class 6 balding pattern. Body hair is never as good, because the growth cycles are short and the length and thickness never matches the donor hair from the scalp in quality and length. Can finasteride make your hair worse?
My dermatologist is offering me to take fin. Im 22 and have a receding hairline, but it doesn’t look too bad yet. I wanna maintain my hair and be proactive, but I’ve been reading stories of fin accelerating their balding and not always regrowing what was lost from fin. I’m scared that this will happen to me. What are your guys experience with this?
Not a problem. Putting the side effects to the side, the only problem with finasteride is that in some men it doesn’t work well, but most men see a slowing of their hair loss or a complete stop of their hair loss. A 70 year old patient of mine was prescribed finasteride by his doctor for his prostate and he came to see me to show that he regrew much of his lost hair, with a big smile telling me that I won’t get any money for a hair transplant on him. Ha Ha I said.

My Hair Looks Worse After Each Transplant!

I started noticing hair loss around the age of 26. I tried the laser for a year and did not believe it worked and took propecia, but felt that did not provide help either. In 2009 I decided to have a hair transplant surgery 2,000 hairs. 1,000 in the front and 1,000 in the back.
After the transplant I began finasteride and rogaine to maintain the amount. I was so happy after the first 6 months, until I started noticed thinning in the front again. Everytime I go to the doctor who will remain nameless they say things are fine. I went for the second procedure a year later to thicken up the front. It has been 5 months and the front looks worse and the back is starting to thin out on the sides and a little in the middle. I have a few questions.
- Can the initial surgery cause this shedding to occur and then stop?
- Could the rogaine cause an adverse affect? Every time I use the rogaine and start combing my hair at least 10-20 hairs come out. I read somewhere that if shedding does not stop after 5 months then stop.
- Does the finasteride not work as well as the propecia? These 2 drugs should keep the back fine, but that is not the case.
- Also could transplanted hair fall out?
I cannot comment too much on your particular issues, because I’ve never examined you… but I suspect there was no Master Plan associated with your hair loss issue and you were chasing after a goal that wasn’t clearly laid out. It’s possible you were never given realistic expectations. Perhaps you should see another surgeon for his/her opinion.
1. I do not know what your initial surgery was, what it was suppose to do, or even if it was successful. In general, you can have shock hair loss from the surgery itself. That’s one possibility. The shedding from shock hair loss is permanent.
2. Rogaine is a topical medication to grow hair, not lose hair. If you have an adverse reaction, then ask your doctor for an examination. Losing 10-20 hairs is normal with or without Rogaine. Finally, some patients report slightly more hair loss when starting Rogaine treatment, but as you continue to use it there should not be a problem. The initial hair loss from Rogaine is thought of as a “cycling” of the old hair falling out in preparation of new hairs to grow.
3. Propecia is finasteride in 1mg strength. There is no difference as it is the same medication. Propecia is the marketing name, like Tylenol is the marketing name for acetaminophen… or Rogaine is the name for minoxidil. I’m not sure what you mean by the “back” … is it the back of the head (donor area) or the top/crown?
4. Transplanted hairs are considered permanent as your donor hair. They do fall out as the hairs cycle, but it regrows.
Can hair loss accelerate from time to time?
Genetic hair loss is not like a straight line if you plotted it out. You inherit a pattern and at different ages some hairs will undergo miniaturization and eventually fall out. So yes, hair loss can accelerate, then slow down and then accelerate again. If you are treating it, you may be treating a stop and go process. Many of my patients tell me that even when they take a drug like finasteride, they may start losing their hair in spirts and stops.

White hair is a genetic trait. You can dye it if you don’t like it, but there’s no pill for it (yet). Eating two onions a day won’t turn your hair black, but it will make your breath horrible.
Can I Alternate Using Rogaine Foam and Rogaine Liquid?

Yes, I don’t see why you’d want to do this… but you can do this. Both the Rogaine liquid and Rogaine Foam are 5% minoxidil, just in different application methods.
My Hairline is Receding on One Corner — Should I Consider a Transplant at 18 Years Old?
Hello, im an 18 year old male and was unlucky enough to be born with not a great hairline to begin with. Now however i notice that my hairline has become a lot worse on one side than the other. Is this natural? And also what would you recommend for treatment? i am already on regaine at the moment which seems to have help a bit and stopped hairloss for now. But would you consider a hair transplant?

You need an evaluation and a Master Plan. At this point, you really don’t know what type of hair loss pattern you have or if you will bald further than the frontal corners. If were to have surgery without a Master Plan with everything under consideration, you could end up with two patchy corner areas of hair and a bald head some years down the line.
You might just be developing a mature hairline or it could be early genetic loss. There’s no way for me to know what you’re seeing or what your treatment options are without an examination. At 18 years old with some hairline corner recession, I wouldn’t anticipate surgery being the recommended path at this time.
Address your depression and self-esteem by educating yourself and seeing a doctor for an examination and a Master Plan of your options.
