Losing Crown Hair While Maintaining a Juvenile Hairline?

Hello Dr. Rassman,

Is it possible to start balding in the vertex area and scalp area while still maintaining a juvenile hairline? I noticed that I still have hair under my furrowed brow, but the overall thickness of my scalp hair has dramatically decreased. I am 22 years of age.
Thank you.

Yes! You can start balding from the vertex (crown) area while maintaining a juvenile hairline. Those patients are a subset of the normal Norwood hair loss patterns with a persistent frontal hairline.

In rare instances, someone with this type of hair loss may even continue to have his juvenile hairline for the rest of his life, even though his crown has become bald.

Yes, you have a very early recession developing with miniaturization of the hairs in the frontal hairline. This could be reversed with minoxidil, finasteride and possibly microneedling.

Losing more hair on one side of my head

So I am thinning fast on only one side of my head, a direct line in the middle of my head from the back of my neck to my forehead separates normal hair to thinning short hair, it’s actually messed up. I am 21 and have had nice thick hair my whole life until I noticed this last year. There is no stimulation or feeling when I massage my scalp on the one side of my head, that side just feels like its barely staying alive lol. I went out and bought, nizoral shampoo and castor oil today and even just rubbing the castor oil mixture in there were tons of hairs covering my hand like im shedding (Again only on that one side of my head) nothing falls out on the other side when I message it. I want to know if ANY of you have seen any positive results from Minoxidi or rogaine. I’m not desperate enough to get my doctor to give me Fin I personally like my hair but due to the fact I need my dick to work and so does my girlfriend the side effects threw me off.

hair loss is not symmetrical. Get a HAIRCHECK test (https://baldingblog.com/haircheck-test-how-it-is-done-video/ ) to put numbers on your hair loss as metrics are the key to diagnosis and building a Personalized Master Plan.

I’ve been looking into FIN and MIN but the amount of hair left on my head suggests that both of those might not work. I’ve also heard a lot about hair systems but there’s not much information on it on google from what I have searched. Where are you supposed to go for a hair system, barbers? I’m located in the Bay Area and don’t know much about hair systems cause I don’t know anyone who does it or has gotten it before.

Is there anything I can do for this amount of hair left? Seems like there isn’t but I still have hope. Don’t want to completely accept being bald just yet.(pic in comment)

For 19, it looks bad, but hair can be maintained for life with medications and/or hair transplants. You need to be evaluated by an expert who can measure your donor density and then determine a Personalized Master Plan for you. Since you are 19, drugs like finasteride have reversed the balding in some men, but working with a good doctor has no substitute if it is the hair you want to keep. Some men will do Scalp Micropigmentation (SMP) which will give you the look of a shaved head for life (see here: https://scalpmicropigmentation.com/ but be selective on the men you look at on our site).

Lowest Donor Density?

What is the lowest donor density a person can have in their late twenties, in order to have a successful transplant?

There is no simple answer to this question on donor density. First, I would want to make sure that the person should not have Diffuse Unpatterned Alopecia, which would reflect a depletee donor area in a young man (1-2% of the male population). Then, the size of the balding area (the area of the demand) and the eventual pattern of balding that would have to be maintained must be balanced in a well defined master plan with a worse case scenario put together. A good doctor can make a judgment on the value of the donor density.

Male Balding and DHT

I have just started balding and I’ve just found out about DHT and hormones affecting hair loss. What can I do to remove DHT in my scalp without shampoos specifically for hair loss? Will Pine Tar shampoo work?

The cause of hair loss in men is almost always due to genetic causes, and it is known as male pattern baldness (MPB) or androgenic alopecia (AGA). Scientists have discovered that genetic balding is linked to a hormone called dihydrotestosterone (DHT), a byproduct of testosterone. While everybody (men and women, balding and non-balding) has DHT, it is the men who have the genetic trait of balding that will lose hair in a specific “male pattern”… and DHT must be present. The hair follicle makes DHT. It may have nothing to do with DHT blood levels, but you must have the genetic predisposition for hair loss.

There are medications, such as Propecia, which block the impact of DHT on the hair follicle and it may stop or reverse the balding process, but it is not a cure. To date, there is no cure for hair loss. Pine tar shampoo won’t stop, slow down, or reverse genetic hair loss.

Antiandrogens

Dear Sir,
I am 32 years old male suffering from male pattern baldness. I ask you can I take antiandrogens for treatment of baldness? Looking forward to receive your answer.

Antiandrogen is a group of medications that block different forms of male hormones. What we generally recommend for treatment of male pattern baldness is finasteride, which is also classified as an anti-androgen in some sources. Finasteride blocks production of DHT (dihydroxytestosterone), which is one of the forms of testosterone that can affect hair growth in prone patients.

The typical antiandrogens can block testosterone. Although these medications are used for treatment of prostate cancer, they are not recommended for treatment of hair loss because of their numerous side effects. If you are a man using these antiandrogens for other medical conditions, they may positively affect your hair but may kill your sex life so you should not use them merely for treatment of male pattern baldness.
