Does masturbating somehow effect finasteride effect? I had my med school exam around the time I started using finasteride and I was masturbating everyday to release stress. I have been on finasteride for six months now and my hair fall hasn’t slowed down

Masturbating is a healthy release! Finasteride is good for hair loss. I am sure that the stress from your examinations added to the hair loss process. The two are not related.

Dear Doctor,
I have always heard you can slow the rate of ones hair loss, (especially if the pattern is the infamous “horsehoe”), but you can’t stop it? This has come from a couple of Dermatologists, M.D.. They paint a bleak picture, why would they be so negative, if it weren’t true?

Yes it is true. It can be bleak. Drugs such as Propecia (finasteride) and Rogaine (minoxidil) do not stop hair loss completely and forever. Sometimes the drugs can grow back some hair, but sooner or later the hair loss will continue (albeit at a slower rate).

Finasteride and the Frontal Hairline

I’ve read time and time again from your responses that finasteride does not work well on the frontal hairlines. Does that mean frontal hairline recession will occur regardless of using finasteride? or are you saying no regrowth would occur with finasteride?

Take 100 people with a Norwood class 3 or 3A hairline and put them on finasteride 1mg daily. If they are under 25 years old, there is a small chance that some hair will grow in this area (I would guess less than 5%). The older the patient, the less likely that we will see significant hair growth.

There is a more important second scenario with those people in the Norwood class 3-5 pattern who are losing frontal hair. Again, in men under 25, there is a good chance (I would estimate at a better than 40% chance) that the hair loss will stop progressing or at least slow down the progression.

From the two observations, I think that the consumer has decisions to make. One of my sons with early frontal loss reversed it completely on the drug and started when he was 28 years old (he is now 40 and stable with regard to no hair loss). That says that all hope is not lost with the use of finasteride for treating frontal loss. It would take a full 2 years to know if it works. A bulk assessment in our office prior to starting the medication will have value for comparing before and after the 2 year treatment period.

Dr. Robert Bernstein has collected a large number of frontal hairline medication reversals, as have many other doctors, but cases like these are few and far between, despite this large collection.

So I started fin about 10 months ago. The last two months i’ve started to notice a lot of new hairs sprouting at the front of my hairline and it has moved forward a considerable amount. The only problem is that behind my hairline, the hair in front has only begun shedding more and is now very thin. Can I expect my hair to fill in where I see regrowth, or will I likely only maintain or lose more density?

It is hard to predict what will happen. Give it a full 18 months and then you will know

For the scientists in our group who are interested in how the drug works, finasteride is a competitive inhibitor to DHT and the effectiveness of this is shown in the graph below. Note that the inhibition works even at lower doses. The degree of inhibition at the AR receptor does not correlate directly with the effectiveness of the dose based on clinical finding; however it is nevertheless close to the degree of inhibition.

Starting to Experience Side Effects After 17 Months on Propecia

I have been a frequent reader and an occasional poster on your blog for a while now. I have been on finasteride (1 mg) for the past year and a half with mixed results. The drug has definitely slowed down my hair loss to an extent and could have also initiated some regrowth in the crown area. Of late (for about a month now) , I have been experiencing some sexual side effects that I had never experienced during the past year and 5 months. Although I do not want to discontinue the medication (since it seems to be working) I am apprehensive about the long term ramification of these new developments. Under the circumstances, would you recommend reducing the dosage from 1 mg a day to 0.5? I might have come across a post on this blog, where you had mentioned about virtually no difference in the potency of the medication at dosages of 0.5 and 1 mg (I might be wrong). It would be great if you could respond to this query.

I am not your doctor and Propecia is a prescription medication that a doctor needs to prescribe to you. You need to ask this question to the doctor you get your prescription from. You should ask your doctor about changing doses in your medication. As for the reduced dosage, I’ve read a study that cites about 80% effectiveness with 0.5mg.

There may be other issues (not only the possible side effect from Propecia). Some men (even in their 20’s) experience impotence, decreased libido, etc, WITHOUT any medications. These issues are not discussed openly in public, because of their sensitive nature… but it does happen. Find out if your issue is due to Propecia or other medical or psychological issues.

Steroid Nasal Spray and Hair Loss

Dear Dr. Rassman:
I’m confused about steroid-based medications and hair loss. My ENT recently prescribed me a steroid cortasone nasal spray for chronic rhinitis. Can this potentially have adverse side
effect on my already progressing MPB?

Yes, steroids can produce accelerated MPB. When using the steroids through a nasal spray, body absorption is as good as any other route. Sounds like you need to have a conversation with your otolaryngologist (ENT).
