Hair Loss Started Again After a Couple Years on Propecia

Hello,
I have been taking Propecia now for about 2 1/2 years and believe it has slowed my hair loss down but has not stopped it. In the last 6 months I have noticed significant hair loss and thinning. What can I do? Also is Avodart or any other promising drugs soon to be approved to treat hair loss by the FDA? And if so when? Thank you for your time

Propecia does not show the same benefits forever. Every person will eventually lose the benefits of this drug, but the open question is when. Avodart might be more effective in people like you, but as you know, it is not FDA approved for use in treating hair loss. I couldn’t say when or if it will be approved for this, nor do I have any inside tips on upcoming drugs that are pending approval. There’s a lot of money to be made if a medication can be proven safe and effective, so I’m sure there are companies out there trying to find the next best treatment. I just don’t know who they are…

Trichup Oil

Dear Sir,
I came to know about this website thru one of the search engine. I am 52. I was getting bald because of stressful life. One of my friend in London recommended me one very interesting product. The name of the product is Trichup Oil. It is a special kind of hair oil I used for a month and I found my hair loss stopped. In the second month I saw new hair growth. It was really surpriced for me but now I am regularly using this oil and my hair is very healthy.

Hair Multiplication Won’t Give a Full Head of Hair

Hello Dr,
I have been looking at articles in relation to hair cloning recently and something caught my attention. Researchers are warning that the first available hair multiplication procedures won’t give a full head of hair. If this is the case do you think hair multiplication will be able to give a full head of hair in the near future? Do you think these kinks will be ironed out? Your views on this would be much appreciated.

The solution where one hair can make many, is years away. We really do not understand the process and can not replicate it at this time. Too much hype, in my opinion and years away from a solution. When ever I report this, I get many emails that knock my pessimism — but I can’t please everyone.

Turkey experience

This young man went to Turkey 3 months ago for a hair transplant because the fee was much lower than quotes near his home. He had a medium level balding crown and very small frontal corner recession. When he went to the clinic, the building was beautiful. He came in on the first day for the surgery evaluation and they suggested 4200 grafts. The next day he had the surgery and found the entire surgical crew unprofessional, ignoring him, laughing and chatting and not focusing on his surgery. He found out that many of them were hired recently, “just off the street” and they were being trained on his head. That day, he counted more than 20 patients having surgery at that facility. He knows that this was a terrible mistake and I am going to work with him after he gets to the 8 month postoperative date when he sees where he is. I am concerned about his donor site, which he believes has been damaged.

Hair Regrowth Cycles

Dear Dr. Rassman,

I am a 43 year old lady with increased hairloss usually between 150-200 hairs a day at the moment.

3 years ago I had two lots of traumas which caused my initial hairloss. After eventually seeing a trichologist and starting estradex lotion (containing minidoxil and phyto hormones)my hairloss reduced this took 3-4 months to see benefit. Initial the estradex made more hair come out which trichologist said normal as the estradex gets rid of the bad hair making room for better quality hair. I was also put on a antidepressant dotheipin 75mg. 12 months ago I stopped estradex, hairloss returned, not that it totally stopped but hair did look thicker. I have started back on estradex about 3 months on side of head but the partening looking slighlty wider on one side so started using estradex along this area for last 6 weeks. Still taking dothiepin 50mg – 75 mg alt days. I have recently started vitamins saw palmetto and pygeum and stinging nettle root. Sure my telogen as increased within a day of taking this vitamin could it be related. Taking high doses off this up to 2000mg. Or do you think its the estradex getting into my system (which contains 5% minidoxil)?

When I woke up yesterday, the birds were chirping and the air was crisp and I had a great day. Today when I woke up the dog was barking and the air was smoggy and I had a bad day. There’s too many variables and no direct proven relationships in the information you provided. It is normal to lose 100+ hairs a day. For most normal people, the regrowing hair will start at about the same time as the hair is lost so that the total hair count of hairs growing on the head stays constant. If you do the math, 100 hairs lost per day is about 109,500 hairs in three years (36,500/year) and that is the normal adult hair population. The hair cycling we have discussed before on this web site, is responsible for the fall out and the regrowth that is every person’s normal. The average growth cycle is 3 years for most people, so that means that in three years, most people replace 100% of the hairs on the head with new hair, one day at a time.

Two post operative patients shown one day after hair transplants (with photos)

Two patients one day after surgery. Neither patient has crusting and are clean as can be. They will not be detectable within a few days after the hair transplant. The first patient had 1800 grafts (FUE) in a completely bald area and the second patient has 2800 grafts (strip surgery) which is easily hidden by his residual hair. I will continue to show post operative patients so that you, our readers who are afraid what they will look like, will see that they won’t look freaky.

Importance of hair shaft thickness

Can you explain more about the hair weight? You’re saying patient ones hair weighs more, and is 4 times thicker than patient twos thin/fine hair, which weighs 1/4 of first patient? Do I have that right? Is hair weight the same as hair volume?

How many grafts have each of those patients had? Both had the same amount of grafts?

Then how come every time someone gets a hair transplant they’re asked how many grafts they got? If 1,000 grafts on patient one could equal 4,000 grafts on another patient, the number of grafts doesn’t mean much without also knowing hair thickness/volume numbers as well. Do I have this correct?

Last question lol, when someone has fine hair being 1/4 the weight of average, do they usually have 4x the number of grafts in their donor area? So thick hair will have 4,000 total donor and fine hair can have 16,000 donor grafts?

Thank you I’m advance! This is a subject I’ve rarely seen talked about. It’s always ONLY how many grafts.

Think of this as building a fence. You have three types of wood 1×1″, 2×2″ and 4×4″. If you built the fence with 4×4 it would be a better fence than with 1×1, Four 1×1 = a singe 2×2 and four 2×2 equals a single 4×4. This is essentially comparable to fine, medium and coarse hair. Although the ratios are not the same a fine hair is 30 microns, a medium hair is 60 microns and a coarse hair is 90-100 microns, the metaphor does apply.

Hair transplant disappointing, need your opinion

Hello Dr. Rassman, I need your opinion on my situation. I’m just looking for an informal opinion.

I received a 2900 graft transplant for my nw 3 last year to rebuild my hairline/temples and strengthen my temple points. I went to a prominent surgeon in Europe. I have very thick/coarse hair with a 2.81 hair per graft ratio. My average hair thickness is 61 microns.

My result turned out ok. I got good coverage but was expecting more in terms of density due to my hair characteristics.

I consulted with my surgeon at the end of one year and he noticed that a good portion of multi haired 3,4, and some 5 hair grafts had only 2 hairs and some 3. He thinks my yield is fine (85-90%) however. He suspects there was trauma either during extraction or implantation when using an implanter pen. The first surgery utilized a .9mm manual punch which he says it typically the biggest he uses. His plan is to do a touch up and increase both the punch size to a .95mm and increase the size of the implanter pen. He will also use pre made incisions for the large 5 hair grafts.

I asked if the trauma could be spotted during graft inspection since they are evaluated under magnification. He said no as the trauma is likely on the root sheath and that can’t be seen.

My questions are:

  1. Do you think this is a good plan moving forward to achieve better density? I know no surgery is risk free
  2. Do you think the trauma explanation regarding the root sheath is feasible?

Your doctors sounds like he understands the problem. Small punches can be a problem but with a 61 micron hair thickness, that is an average hair weight, which should not have been a problem. The high hair count per Follicular unit could have produced damage during excision as he suggested. It is good that he is sticking with you and doing what is right to make you happy! That is a class act.

Use Minoxidil After Transplant?

Hi,
I am 6 weeks post op. I received 1500 fu’s and earlier punch graft work repaired. Other than some minor shock loss in the donor area, I seem to be healing fine for 6 weeks. My question is this; I have never used Propecia or Minoxidil before. I am curious if 5% Minoxidil would speed up and/or enhance grafts. It states that Minoxidil only works in the vertex area. Would that not apply to recipient area on the top of my head? After all, the grafts were taken from below the vertex. Also, I’m using Mederma for scarring from old plug extraction on crown. Is it wise or safe to use Mederma and Minoxidil at the same time? I have purchased the Minoxidil, but have not used it yet. Any information would be appreciated. Thank you.

The donor hair will not be affected by Minoxidil so I do not recommend its use after a hair transplant. I generally put people on Propecia if they are young and in the balding process at almost any stage, as hair transplants can accelerate any hair loss process that is active. Mederma is an over-the-counter herbal medication that is recommended for (1) old scars, and (2) new scars. Its claims include that it softens the scars. It has no defined scientific benefit that I know of. I suspect that it is safe, but I do not have first hand knowledge about it.
