Pricing a Hair Transplant on a Sliding Scale?

Dear Dr. Rassman:

I thank you for the useful information on this blog and your ethical approach in giving honest answers even if they are not financially beneficial to your practice. My question is the following: Do you utilize a sliding scale when quoting prices for patients? For example, if a one person earns a $60,000.00 salary and if another person earns a $160,000.00 salary, would the person who earns $60,000.00 be quoted a reduced fee?

If you purchased a car, whether a Chevy or a Mercedes, the price reflects the cost to deliver a quality product… not the income of the buyer. My staff is well paid and this reflects their skills to deliver a quality service, so the entire package of labor, surgical supplies, and the costs to run a surgical practice reflect the charge to the patient. The surgeon’s fee component is actually relatively low compared to the rest of the costs for the surgeon.

So to answer your question directly, the income of the patient does not reflect the fees we charge.

There is clearly hair growing in the crown where there was not hair before which reflects growth. It is possible that you did not get enough grafts to do the job right? For the size of your crown, I would have expected at least 2000-2500 grafts depending upon the thickness of your hair, the finer hair requiring more grafts. Men with very fine hair with say 2000 grafts, could get this type of result because the hair just doesn’t have the bulk to it. Propecia Allergy?

I have been taking Propecia for about two years. It has always worked fine until roughly six months ago when certain side effects began to appear. My lips were swelling quite frequently and I would get hives on my body. I was able to combat this with a daily dose of claritan. I am worried though that these effects could be dangerous to my body, even though the hives never appear while taking claritan and the lip swelling is only a few times a month at most. Also, I was curious to know if there is any correlation to these side effects and the ineffectiveness of propecia. I have noticed an increase in hairloss over the past six months, but still nowhere near as bad as before I went on the drug. Any light you could shed on this matter would be great. Also, I thought I would add that I have experienced no sexual side effects.

You need to have a good dermatologist take a look at you. Although some of the symptoms are reflective of a drug allergy, you may also have some other medical problem. Don’t play doctor. There is an old adage: a person who treats themself has a fool for a doctor. Adhere to good common sense and have a professional look at you.

Did the Growth Hormones I Took Cause Permanent Hair Loss?

I am 21 and used Growth Hormone for a little over 2 months i am starting to notice hair loss. Is this temporary or will it grow back once i stop? Or has the GH prematurely started my hair loss and will these lost hairs never grow back? I need to know if it is strictly short term and they will grow back i am very nervous. Thank you Dr

I have to apologize in advance for the strong opinions I have on the subject you introduced in your question. I always want to know why people your age would take growth hormones, and whether they were under the care of a physician or were just experimenting with them. You didn’t state why you were taking them, but growth hormones can be dangerous and I strongly recommend that you follow up with your doctor to make sure there were no other damages to your body.

With respect to hair loss, growth hormones can certainly precipitate genetic male pattern baldness. I cannot say if the hairs would grow back, but if it was hormone/genetic related, the hair that was lost will probably not grow back.

Propecia and HGH?

Just wondering, im 30 years old hair starting to thin, i’m on propecia and curious about taking HGH. I want to get a bit bigger, but will this cause my hair to fall out? and is the safe to take this many drugs at once?

Human growth hormone (HGH) can cause hair loss, though one could speculate that Propecia might reduce the possibility of loss. Talk to your prescribing doctor about your plan.

Different Hair Characteristics in Transplanted Region

I had my transplant at NHI 9 montys (2000 grafts to the front). My hair is growing very fast, and looks awesome, except that it is curly, and the rest of my hair is absolutely bone straight. I am growing my new “full” head of hair long and “surfery” looking, but it looks so different with a curly, puffy front, and then the rest is long, flowing and stick straight. Will the front ever be straight? What can be done? I use a hot flat iron every day to try and straighten it.. I know totally why people with curly hair hate humidity and rain…instant frizz. Help.

Hair transplants rarely radically change the character of the hair, but it can happen. In about half of the people when this change occurs, the hair will straighten a bit after going through a hair cycle. I remember one person who had a very radical curly change to his hair after the first surgery. I personally did not like it at all, but he loved it. When he came in for his second transplant, the same thing happened again. It remained very curly and he could not have been happier. Much to my surprise, it lasted through a hair cycle, possibly a bit less curly.

Some people who get changes in hair characteristics, go to a slightly more wavy hair and this change is appreciated in almost every person because it produces ‘body’ for styling that they never had when their hair was very straight. This often reverses in a hair cycle (about 2-6 years), so these benefits may not last.

With regard to your situation, you need to see me personally so that I can review your change in hair character. In addition, unfortunately, you will have to learn to control the hair with relaxers that will allow you to gain better better control of styling. Propecia and Sperm Morphology

I have recently had a sperm analysis done and have seen that I have abnormal sperm morphology, pretty much close to 100%. I have been taking Propecia for close to 9 years. Does Propecia increase the effects of abnormal sperm morphology?

There is a report in Fertil Steril. 2013 Dec;100(6):1542-6. doi: 10.1016/j.fertnstert.2013.07.2000. Epub 2013 Sep 4. that state Propecia (finasteride) can cause reduced sperm counts in some men. It goes on to state: “In this population, counts improved dramatically for the majority of men after finasteride discontinuation. The hormone parameters, sperm motility, and sperm morphology were unchanged after cessation. Finasteride should be discontinued in subfertile men with oligospermia, and used with caution in men who desire fertility.

In your case, we do not know if your sperm morphology was abnormal even before taking Propecia. It would be difficult to establish Propecia may have cause it. I simply do not know. Your question is better left to your urologist or reproductive specialist.

Direction of Graft Growth

Hi Dr,

Can you explain how accurately a surgeon such as yourself can transplant grafts so that hairs grow in the same direction?

I have seen quite a few celebrity transplants where the hair still appears thin and the hair once grown out always looks messy because the direction of the grafts is not quite right.

How are you able to ensure that this doesn’t happen?

As someone with naturally fine and very straight hair then i imagine it would be essential that any transplant i had done should be performed by a top surgeon who is able to make sure that the angle of the transplanted graft is accurate. If i had thicker and naturally curly hair then i suppose this wouldn’t matter so much and is probably a part of the reason why those with curly make better candidates?

Doctors have direct control of how the transplanted hair grows and how the transplanted hairs are distributed to give the most natural look. This is a very important point that most prospective patients do not take into account when searching for the right medical group for a hair transplant procedure.

You are correct in stating the fine straight hairs need careful attention to detail when undergoing a hair transplant procedure, but careful attention to detail is also a must for coarse hair, because each hair can have many times the bulk of a fine hair. A coarse hair put in the wrong direction will be more easily detected than a fine hair put in the wrong direction. Some surgeons, when placing the hairline, place the frontal hairs radially (like bicycle spokes). Is it possible that surgeons who do this do not understand the normal direction of the hair growth, just are ignorant of the facts? Sometimes I think so.

To be sure that these poor placement doesn’t happen to you, you have to do your own research and due diligence. There are many good doctors with great artistic skills. The best way to determine who these doctors are is to ask to meet some of their patients. My patients love showing off their results in the privacy of our office, so we set up circumstances once a month to allow this to happen. To me, this is just plain good medical education.
