Men who take testosterone and then lose their hair

I was on Androgel since august 2018 after being diagnosed at 208 nG/dL. The reason I got myself diagnosed in the first place was because of my high pitched voice and lack of (muscle) development. While I was on TRT, I did feel slightly stronger and my biceps did get bulkier and feel significantly firmer to the touch (and I don’t work out). However, around the turn of 2019 I noticed my hair was noticeably thinner. I had always had a dense forest of hair on my head ever since I remember. That changed only after I started TRT. I quit Androgel cold turkey in January, but the damage was done. My hair kept deteriorating so I got a DHT test: 747.67 pg/ml. (Normal range: 250 – 990)

I then got on Dutasteride for a month, but taking it didn’t seem to help so I tested my DHT again while I was on it: 1110.8 pg/ml.

I have since stuck to Finasteride, and my DHT levels dropped to 590 pg/ml then rose to 990 pgml. My ferretin was measured at 48 ng/ml on a range of 30 to 440.

My hair is worse than ever. I have a noticeable bald spot on my crown and my hairline has receded noticeably. My hair now parts naturally in a very noticeable path, looks like severe diffuse thinning. I started fin in march and mine in June, but my hair is worse than ever and sheds more than 100 hairs daily. Hair is frizzy and curly whereas it was straight before trt. Shed hairs are much thinner towards the bulb.

I wish I never took TRT. My hair was fine before TRT and that was not the only thing Androgel affected. I wish I could rewind time back to last year and convince my younger self not to make this irreversible decision.

Unfortunately, many men who go on Testosterone therapy lose their hair if they have the genes for balding. Many older man who had minimal hair loss and then take T therapy, start losing their hair big-time

Asymmetrical beard and hairline

I seem to have less hair on the right side of my face in relation to the left side, but yet nobody else in my family has this issue. What is usually the cause of this and is a hair transplant the only solution?

Some men develop asymmetrical beard growth. If you are just starting growing your beard in the past year, don’t worry. Also the transition from a juvenile hairline to a mature hairline is also often asymmetrical as well.

This article was published however, I am not impressed that this will cross-over into humans. You judge for yourself.

Avodart – An Update

I would like to know what you found out in regards to Avodart at the hairloss convention. Please get back to me when you get a chance.

There is a strong suggestion that Avodart will work better than Propecia from what I heard at the meeting. There are, however, many questions not answered such as:

  1. What is the appropriate dose considering that there is a half life of 6 weeks for the medication in your body? Nobody seems to have a good answer to this question.
  2. Some think that Avodart may stay in the tissues for over 6 months, so what effect could this have on your system?
  3. What is the long term safety of Avodart taken by young man? All of the documented studies are on men over 50 (old-‘farts’ like me)

These are the reasons that most doctors do not want to prescribe Avodart ‘off label’ (not FDA approved for hair loss treatment). From a medical-legal point of view, as well as a licensing issue, I can not really prescribe it. Also, my expertise is in surgery, not medications (although I try to use medications I command and know forward and backward). There are some dermatologists, internists, urologists and family doctors that would prescribe this medication, and if you want to go that route, you should try to find one of them to prescribe it for you. With an off-label use, the issues of safety are risks that you must take.

The preliminary studies that were presented indicate that it works to grow hair when Propecia fails to do so. Still, Propecia should be the first line for the medical treatment of hair loss. What does concern many doctors, however, is that there may be a Vioxx type of hidden side effect in the young man that is not known. Will it produce cancers or heart disease or some other problem that is not yet identified? This is the reason we look to the FDA, to define safety and effectiveness for us. So far, the safety is not clear and the effectiveness appears to be coming into view through unofficial channels.

Microneedling beyond normal hairline

There is some good teaching in your photos. This is an outstanding regrowth of hair from microneedling and minoxidil. I want to call your attention to the corner area which received microneeding beyond (below) the natural adult mature hairline, which did not bring back the hair to the full extent of the microneddling. It is important to note that microneeding in areas where your juvenile hairline is lost, microneedling this area will not bring your juvenile hairline back. For those who are microneedling, please limit it to the mature hairline you want to return to.

I Had a Bad Hairline Lowering Hair Transplant

My forehead came down about 1cm with 1500 grafts. I am unhappy with the unnatural look, and my skin is elevated and white. I would like to reduce my forehead surgically and reverse the transplant.

This is not an easy assessment without seeing, at the least, a photo. You said the hairline was only lowered about 1cm, so I need to know what it looks like. Also, your comments about the elevated grafts after growth and the white color of the skin is concerning. If the entire area in front of your normal hairline is no more than 2.5 cm, then a hairline lowering procedure may be able to remove the entire transplanted area. You should see a surgeon that offers hairline advancement surgery, and this could be a good solution for you.

Microneedling question

Hello Doctor, may I ask if microneedling could induce hair regrowth in an area of the hairline that is completely bald?

I believe that it may be associated more with how long it has been bald there. If you are under 26 years of age, then the balding area might still have active stem cells but if you are 60, the stem cells might have died off by now.

Bald Scalp Lacks Progenitor Cells

Interesting article :

Bald scalp in men with androgenetic alopecia retains hair follicle stem cells but lacks CD200-rich and CD34-positive hair follicle progenitor cells. This is exciting because this means that the critical elements for creating the mature hair follicle is in the scalp and once the progenitor cell defect is solved, we should produce gobs of good hair bringing back what was lost.

Miniaturization Study and What Is It Supposed To Look Like? Am I Balding?

You said before it’s normal for non-balding men to have around 10% miniaturization on any part of their scalp.

Could take a look at these photos to help those of us who perform our own miniaturization mapping what that looks like.

Is it normal for hairs to be different thickness like this? Are the lighter colored hair in an earlier stage of growth compared to the darker thicker hairs? Could the lighter hair actually be greying in process (I already have a few grey hairs in odd places)?

I am 25 years old, with Norwood 2 trying to figure out if my hairline is maturing or I am going bald.

In general it is normal to have what appears to be 20% of miniaturized hairs on a normal person’s scalp. Some of these finer hair are actually vellus hairs but from a microscopic view they are shorter than regular normal hairs but also appear miniaturized. There can be variations and ambiguous interpretation when the hairs are naturally fine in texture or there are different colored hairs in the mix (grey, blond, brown). The photos show similar hair thickness on the left and right and it may be that your naturally fine can be differentiated from normal hair thickness in you. Hairs on top look normal as well. I didn’t see much miniaturization on any of the photos.

One other way to diagnose very early balding that can not be seen when hair loss is less than 30%, is to have your doctor perform a bulk measurement with an instrument called HAIRCHECK. This instrument will tell you if your hair bulk has been reduced and 99% of the time any reduction is due to genetic balding in the male.

Finally, we don’t diagnose hair line patterns here on BaldingBlog. If you are worried about hair loss please see a doctor.
