Propecia and Liver Disease

I have been taking propecia for about 6 years. I also have Primary Scholerosing Cholangitis (PSC), a liver disease. Do you know of any studies or have any other knowledge about the impact of propecia on the liver? Thank you.

There are warnings on the use of Propecia for those people with liver disease, however, it is not clear if the drug can negatively impact the liver directly in those with active liver disease. What I suspect is that the metabolism of the drug may be affected by active liver dysfunction. I will ask Merck, the makers of Propecia, about it and get clarification from them, getting back to you in the very near future. They are usually responsive to my questions.

Diffuse Hair Loss

(female) Dear Drs,

I was told that I most likely am suffering from Diffuse Areata, the thing that directed my doctor to this diagnosis is diffuse all over hair loss, brow and lash shed and short tapered hairs shedding out. I can find nothing on the subject, nothing on the net and my doctor admitted he did not understand the disorder.

Could you provide me with information regarding the diffuse form leaving no bald spots but diffuse loss all over the head. What is the general prognosis etc?

First, I want to tell you that clinical medical science is a descriptive science in many situation. This is important because what we see is what we are reporting when we diagnose diffuse alopecia areata. What doctors are saying is that there is a diffuse form of the disease that mimics classic alopecia areata, except that it is found all over the head. Some doctors question if this is the same disease, but because it looks like the same entity when the skin is biopsied as localized alopecia areata, the conclusion is that the diffuse variant is different in ways that are not understood. Most people believe that it is an autoimmune disease and we see evidence in what we call chronic and diffuse alopecia areata of the microscopic changes seen in autoimmune diseases.

Propecia and Testicular Pain

I have had pain in my testicles after I started Propecia. Is there a relationships?

There have been occasional reports of testicular pain on Propecia (finasteride). I have had thousands of patients using this medication and this has never crossed my path as a patient complaint, but some other doctors in the community have reported this as a suspected rare side effect. One doctor reported that the pain came on with the drug and disappeared when the drug was stopped, suggesting a relationship. It could have been coincidental. Most causes of testicular pain are caused by athletic stresses and for the young men, it may have something to do with puberty.

I would stop Propecia, wait a month and start it up again. If it happens a second time, then assume that this is a side effect of this drug for you.

Disappointed with My Hair Transplant from 8 Months Ago

Hello. I had an FUT procedure done at [name removed] about 8 months ago. I see visible, what look like holes on my recipient area from overhead lighting, almost like scarring. Obviously, I wasn’t expecting this as I had heard that new techniques show no visible recipient scarring or mini holes. Will this EVER heal in time, because now i am freaking out about it. If so, how long does this take to go away?

In addition, my hair is darker on the sides than the top so the recipient hair does not match in color. Will this change in time? My doctor tells me the sun will change it and it will eventually grow in lighter at the roots but I don’t know if I trust anything they say anymore.

I wish I had never done this and don’t know where to go from here. Any answers and direction would be very helpful because I am freaked out now.

Generally by 8 to 12 months most of your hair transplants should have grown and you should not have the visible holes or scarring that you are describing. As you do, then this is probably permanent. Sometimes certain patients are more prone to scarring than others, but that is not what you seem to be describing and it is very, very, very rare in the recipient area. With respect to hair color, your hair color should generally match the hair on the back and sides of your head.

Obviously it is very difficult and practically impossible to give personalized advice this way. Propecia Doesn’t Prevent Hair Loss, But Just Slows It Down?

I read somewhere that propecia doesnt prevent baldness it just slows down the process.I am using propecia since i was 19, so should i expect to get bald sooner or later one day even though i continue to use propecia? If yes,on an average at what age will get bald?

Everyone is different and reacts differently to medication. Some people have a strong and aggressive hair loss pattern and Propecia will slow it down, but not by much. Other people have a not-so-strong tendency to bald and it may slow it down for years and years. Others will reverse the balding process. I have a number of patients who took it for crown balding when the drug was first released and the hair loss completely reversed and has stayed normal since 1998 (no evident balding to the naked eye, although it can be seen with some miniaturized hair when looked at under high magnification).

Do Black Women Have Less Hair Loss Treatment Options?

I interesting in learning what can be done for Black Women who are lossing their hair. And the physcians are saying it from the chemicals that we use. Ex. perms. they say the only treatment is rogaine and injections which will also cause complete baldness if we can no longer afford that treatment while they give the white women hormone replacement. Can you help and give better advise

Black hair permChemicals (like relaxers) can cause damage, but many black women will also see traction alopecia, which is permanent hair loss caused by the pulling of tight braids, many times from when they were very young. For women, the only FDA approved hair loss treatment is Rogaine (minoxidil). Hormone replacement likely won’t regrow your hair and I’m not sure which injections your doctor is recommending.

This isn’t an issue of race so much as gender. For men, the great majority of cases of hair loss are from genetics and there’s an oral pill for that. For women, there’s a large list of possible causes of hair loss and no truly great solutions. So it’s not that black women have less options, but rather, all women have limited treatment options.

I have been on Propecia for a month and I noticed pain in my nipples

Breast pain and nipple pain has been reported with people using Propecia (finasteide). If this continues, you should see a doctor if that is why you are asking this question. Some people develop small painful breast lumps so an examination of a doctor is a good idea. When this happens (it is rare), the drug often needs to be stopped.

Do Hairy People Respond Better to Finasteride?

Yes, usually hairy people who take finasteride are early in the hair loss process, so that is when finasteride is most effective. At the end of the hair loss process, large balding areas rarely respond to finasteride. Everything else is in-between.

Propecia Sufferers Are Misdirected and Belittled

In a recent blog post you wrote about sexual side effects associated with propecia use, “These side effects are reversible withing one to two weeks after you stop the medication.” You then went on to say that Dr. Irwig’s study in Journal of Sexual Medicine was not a scientific study.

When you read through the FDA hearing on Propecia (FDA link) Keith Kaufman of Merck talks about the 3.8 percent of finasteride users experiencing side effects versus 2.1 percent of the placebo. He then goes onto say that these patients then completed a validated sexual function questionare. I am not a doctor but Merck basically based these figures on self reporting, should those numbers not be considered scientific?

In July the Dr. Irwin Goldstein the Editor-in-Chief of the Journal of Sexual Medicine wrote “I think of the frequent phone calls I receive from distressed men with varying degrees of hair loss who have used 5 alpha reductase inhibitors and now have newly manifested sexual and cognitive complaints that often persist despite discontinuation of the 5 alpha reductase inhibitor. Often such 5 alpha reductase inhibitor users have sought help only to be belittled, betrayed, misdirected, and sometimes misinformed. In general, these patients feel deceived becuase of the lack of information warning them of the potential side effects.”

I am one of the individuals suffering from persistent sexual dysfunction after stopping propecia and it appears pretty certain to me that in a small subset of men the persistent sexual dysfunction is due to the medication.

I am not going to get roped into an anti-Propecia agenda. I will leave that up to those who regularly comment on almost every post where the word “Propecia” appears.

My heart goes out to you if you actually have long-lasting sexual side effects, but the questions of validity ring true to your cause of sexual dysfunction. I am not in a position to judge your problem or to put the pieces together for you. That must be between you and your doctors. I am a doctor, but the posts here are just my opinions on my blog. We attempted to obtain followup on all patients reporting sexually-related adverse experiences. Of the 36 patients on finasteride reporting these adverse experiences, 21 reported resolution of the adverse event while continuing finasteride therapy. Seven reported resolution following discontinuation from the study, and seven had persistence of the adverse event while continuing in the study on finasteride therapy. The pattern for patients on placebo was essentially similar, with most patients resolving either on or off drug, four patients reported persistence of the adverse event while remaining on placebo therapy.”

Do I have to stop losing hair to get a hair transplant?

I have been experiencing hair loss for 6-7 years now and my density has kept on decreasing. I have never been on Fin, however, I did try out min for 2-3 months and then stopped using it. I see a lot of posts that suggest an HT only after your hair loss has stabilized. but how do you guys determine that? I lose hairs every day. Should I get on fin and see it I stop losing hair, and then process for an HT ?

You don’t have to wait until you get bald to get a hair transplant. Stabilization is not always possible, even with finasteride. The key is to be able to predict with reasonable certainty, what your long term hair loss pattern will be. Most men must be 26 to be able to make this prediction and that allows the surgeon to build a proper long-term Master Plan for your future hair loss. I have had men keep up with their balding without running out of donor hair in the 29 years I have been doing this type of surgery.
