I have a question for you.

I’m taking finasteride 1mg now for two years and my hair improved a lot.
Now with exams and very busy schedule I forgot to make an appointment with my docor. I now have one in three weeks. But now i’m afraid i’ll lose my hair again because i’m out of finasteride for three weeks.

Is it possible i can lose a lot of hair in three weeks?

It is possible that you can lose hair after stopping Propecia for three weeks. It generally takes longer for “catch up” hair loss to occur. Catch up hair loss is basically the loss of your native hair that benefited from the use of Propecia.

Can Zinc Cause Hair Loss From Increased Testosterone?

I get oily skin and increased libido while on zinc 50mg/day. As Zinc increases testosterone so I fear that it may increase the progression rate of my baldness. Considering that I’m also on Finasteride, will the use of zinc negatively affect my hair?

The short answer to your question is that zinc does not necessarily negatively affect hair loss. Some studies show zinc supplements increase testosterone levels.
There are other studies that show zinc supplements do NOT affect testosterone levels.

For a normal healthy individual with genetic male pattern balding (MPB) taking over the counter supplements such as zinc would not negatively or positively affect their hair loss. It is always a good idea to follow up with your doctor if you have concerns.

Im curious when the testosterone increases from finasteride if it can speed up hairloss in the frontal area? So propecia could in theory speed up frontal hairloss while slowing down vertex/crown hairloss? Propecia also permanently increases receptor areas for dht? So in reality propecia could make hairloss worse it is possible?

I do not believe you analysis is correct. DHT blocks the 5 alpha reductase enzyme production which causes the hair loss in genetically prone men.

I am finding that there is a trickle of these types of questions. Propecia (finasteride) is prescribed for slowing down genetic androgenic hair loss. It does not cause more hair loss. I do not know why there is a belief that testosterone causes frontal hair loss. On men who take finasteride, they still produce healthy amounts of testosterone. Sometime, the testosterone levels increase when a person is on the drug finasteride.

Catch-Up Hair Loss, Finasteride, and Hair Cell Suicide

Hi, Dr. Rassman.

You’ve written a number of times about the “catch-up” loss that happens when a patient discontinues finasteride/propecia. I know you didn’t design the drug, but I was wondering if you know how this catch-up process works.

What I have a hard time understanding is this: If androgenic alopecia is a gradual process of miniaturization, why doesn’t that process-when you stop the drug-just pick up where it left off at the start of therapy? It seems like something must be getting worse “behind the scenes” during the course of therapy. Is it known what that something is?

Propecia (finasteride 1mg) sustains scalp hairs that are genetically susceptible to falling out. It does this by a process of competitive inhibition. When the drug is no longer available, the DHT is manufactured by the body and it ‘attacks’ the hair follicle which is like turning off the ‘life’ switch of the dermal papilla cells (which manufactures the hair follicles).

Once you stop taking the medication, you will not lose all the hair at once, but the ‘attack’ will occur over a few months. The hairs that were being supported by finasteride will go back to its pre-destined state, which is something called apoptosis (these dermal papilla cells that generate the hair follicle commit suicide — just die off).

It is not unusual for the hairline to mature and lose the frontal 3/4 inch upward. I call this a maturing hairline but it is not always uniform and it can start in the center anytime between the ages of 18-29 years. This can also be a sign of genetic balding but your hairline looks flat and low. You should see a doctor to determine just where this is coming from. The doctor will perform both a HAIRCHECK test of your entire scalp and miniaturization studies of your frontal hairline.

Oily Skin or Acne a Factor in Male Pattern Baldness?

Is having really oily skin or acne a risk factor for Male Pattern Baldness? My dermatologist didn’t see any miniturization. I’m not shedding. I’m 22 years old and there’s no visible recession. I still have a teenage hairline. But I do have really oily skin. My dad is a Norwood Class 7. I figure if I was going to be a class 7 I’d have recession by now. What do you think?

Acne is common in young men, and really, it’s common with any man with oily skin. That being said, there may be an association, but not a cause and effect (balding does not produce acne). Likewise, oily skin and acne will not cause balding. The acne problem is more common in the young patient, when hormones seem more volatile. I am sure you remember the days in high school when the zits seem to pop up at the worst times. In these situations, the appearance of the beard in the young man compounds the problem.

Chemical burns to scalp from dyes or hair straighteners

Is there a way to check whether the hair loss is from chemical burns as opposed to a condition like alopecia?

If there is an event where a chemical burn could have happened, then the burn may limit some or all of the hair growth in the area where the burn happened. I generally say if hair has not grown in 8 months since the incident, then the chemical burn is permanent. I had one bout of Alopecia Areata and I have DUPA now, can I have a Hair Transplant?

I had one small spot of alopecia areata on the back of my head 3 years ago at 32 years old, it resolved with a steroid shot. I do have it on my face and it resolves periodically without treatment. Right now I have DPA but almost my hair is intact, just generally thin throughout the NW6 pattern. Can’t tolerate fin.

DUPA, if properly diagnoses, is a contraindication for hair transplants as the hairs are highly miniaturized and may not grow when moved to the balding areas.
