Tug-of-War between finasteride and genetic hair loss metaphor

If I remember correctly Dr Rassman u/wrassman said that in his experience that people who are younger (under 25) tend to respond way better to finasteride, but obviously thats anecdotal and everyone’s body responds differently

Yes. I have seen young men who were becoming a Class 7 at 19 years of age, and they responded poorly to finasteride but it did slow down their loss rate. I always use the metaphor of a Tug-of-War between 5 alpha reductase blockers and the aggressiveness of the genetics of balding

Hair Raiser

I was just wondering if you had any information on Billy Jealousy’s new product, Hair Raiser. It claims to regrow lost hair and recently won an FHM grooming award.

It is said to contain Methyl nicotinate and 2 DHT blockers (Swertia and Licorice Extract). Their website says “Hair Raiser takes about 4 months of every day use to experience the beginning growth of terminal hair. Regrowth will not be experienced if hair follicle has been dead for more than 10 years. Hair Raiser was created by world renowned bio-chemist C. Sanders.“

I am not familiar C. Sanders, the bio-chemist — but I am familiar with Colonel Sanders, the purveyor of original recipe and extra crispy fried chicken. Kidding aside, if you have been following this blog, it has always been stressed that it is a “buyer beware” market. Anyone can claim effects of a cream/lotion/potion. What strikes me is that there is a 1989 U.S. Supreme Court decision preventing these products being advertised or sold in the U.S., yet these types of claims are made and products are sold every day. There are only 2 FDA approved medications to treat male pattern baldness: (1) topical minoxidil (such as Rogaine) and (2) oral finasteride (Propecia). Since Propecia is still protected under patent, I suspect this Hair Raiser product contains Minoxidil as one of the ingredients, if it actually does do anything for your lost hair.

Two normal hairlines

These men clearly are using some medication to get those hairlines. Do you agree?

Half of the male population is not balding and some of them have hairlines like these men without medications. Just great genetics!

Hair thickened over 1 year

A little over a year of being on 1mg finasteride pills and using 5% minoxidil once daily. Not much hairline progress since my last update, but my hair has thickened significantly. Extremely happy with these results.

It has thickened because you took finasteride and minoxidil and probably reversed the miniaturization that might have progressed to hair loss. Nice!

Undetectable Hair Transplant

Dear Dr. Rassman,
I will like to know if you can you tell that someone had a hair transplant by looking at it?

Hair grows naturally in units of 1, 2, and 3 hairs each. The frontal hairline always has single hairs and a transition zone of these single hairs starts at the forehead (more widely spaced) and they then get closer together as you look further back into the actual denser hair. Within ¼ of an inch, these single hairs get noticeably denser and then groups of 2 hairs start appearing. I say this because to make a hair transplant look like God’s work, God’s design must be copied. The single hair leading edge of a non-transplanted person is erratic and disorderly. It is not heavy, so the idea that there is a ‘line’ with a hairline is a misconception. The transition zone makes the hairline imperceptible. If the pattern is created by the transplant surgeon with enough hair to mimic the normal transition zone, an observer should not be able to see a hairline nor a hair transplant. At our open house events (which we hold every month at each office) the people are amazed that they can not pick out those who had hair transplants from those that did not.

So simply answered, in 95% of all cases, a hair transplant should not be detectable as a hair transplant if there is enough hair to address the needs of the patient. Look at this video of a completely transplanted frontal hairline worked with a comb. Please note that this patient has a high contrast black hair against a light skin color (the most demanding combination). If there is anything abnormal looking about a hair transplant, it will always show up in someone with black hair and light skin.

Am I a Hair Transplant Candidate?

What is your age? Your hairline is clearly high. I would love to see pictures with your eyebrows lifted so the creases on your forehead show. This will tell me where your hairline belongs and if it has receded. Then, maybe knowing your age and seeing those pictures, I can give you a suggestion. If you are over 25, you would make a good hair transplant candidate to lower the hairline which will better balance your facial features.

Hair line lowering surgery for men

I suffer from MPB, which I’ve successfully been treating with RU58841 plus DHT-blocking shampoo+conditioner and obviously all the must-have daily supplements . My MPB has stopped, I shed zero hair, maybe 5 hairs every time I wash my hair. My hair’s very thick and dense. But my hair/hairline is still disgusting and I can’t style my hair the way I want to style it.In any way . The only difference between those photos and the current me is that my hair’s a bit longer now, hairline’s the same sadly and my eyebrows are now thick due to minoxidl on eyebrows.

A hair transplant clinic I’ve recently been to in person refused to give me a HT just for the temples, to fill them in, because apparently to them they wouldn’t be able to achieve the same density on the temples with the transplanted hair as my native hair so it’d look fake according to them. But I desperately want and need to not only reduce my forehead’s size which is absolutely disgusting to look at, but also to get my temples back. I’m still fucking ugly, and the first step to fix this problem would be to fix my forehead/hairline issue, I swear every time I look at my huge forehead and slick bald temples I feel throwing up on the spot.

So do I get a HT or a Forehead Reduction Surgery (aka hairline lowering surgery)?

In my opinion, young men should never get a hairline advancement surgery because young men don’t know where their hair is going to be lost. If the hair is lost behind where the advancement is, they are stuck with a surgical scar that will become progressively obvious. For many young women, hairline advancement surgeries work nicely because they don’t go on to recession of the hairline as young men do.

Using Body Hair to Transplant Into Scalp Donor Scar?

I know you aren’t a fan of body hair transplantation because of the different growth cycles it has compared to the hair on your head and just a general difference in the type of hair but do you think it would work when used in a donor scar from a hair transplantation? I just figure it would work pretty well when you’re just trying to cover up a smaller area like that and aren’t as concerned how the hair looks just so long as it covers.

I would agree with you on this, although Body hair Transplanation in general is not ideal for Hair Restoration surgery, it would probably work ok to give some coverage in a Donor Scar.

It may work, but as the body hair is not as thick as regular scalp hair, the coverage per hair shaft will be less than that of scalp hair. Do you have enough hair for FUE from the scalp which may be better? A stronger hair can be found in the pubic and beard areas and both of these areas can be used as donor hair sources as well.

Hair Transplant Results After 4761 Grafts (with Photos)

This patient has had three procedures with a total number of 4761 grafts. The last procedure he had was a little over a year and a half ago, and the after photo was taken this week. He’s grown his hair long for the “rock and roll” look, and the results speak for themselves.

I realize that the quality of the before photos isn’t all that great, but they should still be clear enough to see what the transplants did for his hairline and the hair behind the hairline. The transplants were placed almost to mid-head and when he combs it in almost any direction, he has a full head of hair. The photos were taken previous to his first surgery 8 years ago and our digital camera back then was hit and miss, so I do apologize if the quality of those photos is lackluster.
