WHY Does Shedding from Hair Loss Medications Occur?

Hey, Doc. I’ve been taking Avodart for the past 3 months and have been experiencing what can only be described as a “shed”. While I’ve seen other posts and answers here regarding shedding, I’ve never seen you delve into why it actually happens. Can you enlighten? Thanks.

I do not know why it happens. It seems to happen more through the reports on this blog site than I have seen in my own practice. As I do not have experience with Avodart (dutasteride), I can only assume that any ideas I might have will be a common thread to Propecia (finasteride). It has been suggested by some doctors that shedding can be explained, because acceleration of a hair cycle occurs. Hairs in the growth phase are pushed into a telogen phase (which means that it will have some shedding). The suggestion then is that the growth phase comes in stronger. This is the idea of others, so I am just passing what I hear, not what I know as fact.

How to Fix a Pluggy Hairline (with Photos)

From time to time, I get emails from men that are incredibly embarrassed about the results they got from an old, pluggy hair transplant. They’re so embarrassed that they request I don’t even publish their emails (even if there’s nothing identifiable in there)… and that’s fair enough. So I just wanted to write a post to quickly explain the best way to fix a pluggy hairline: you need to cut out the plugs and then put transplants in the place the plugs originally were. This is generally done in two sessions (8-10 weeks apart) and we have been doing this for years. It turned out that this man’s scar (from excising the grafts) healed very well, so well that it could not be detected. I put hair wide enough in front of where the scar was so that his hairline was lowered quite a bit. We built a wide transition zone in the leading frontal edge. Here is such a case on a man with black hair.

Before on the left, after on the right. Click the photos below to enlarge.

There’s another plug repair case at the NHI site — Dean’s Story. Be warned, some of the photos there contain surgical content, so if you have a weak stomach, you may want to avoid that link.

Why Is Hair Transplantation Not More Common?

thanx for the informative blog. i have a question, despite of such enormity of hair loss problem , why the hair transplantation is not so common.

It takes time for people to accept a new breakthrough in surgery. If you look at female breast augmentation, it took more than a decade for women to feel comfortable enough with it to step up and undergo the surgery. Most people think that hair transplants can always be seen and when they run into a person with plugs coming our of the scalp, they get the idea that this is a hair transplant and say to themselves, “This will never be me!” Today’s hair transplant can not be detected, so the good work done regularly is not something that the average person can detect unless you can remember what someone looked like before they got the hair transplant. A woman with an “A” breast size can be detected when she gets breast augmentation and one day comes out with a “C” or “D” size showing good cleavage. Only a bald man who now has hair, can be detected much like the woman with the newly larger breasts.

Why Might a Patient with 6000 Grafts Have a Thinner Appearance Than a Patient with 3000 Grafts?

Dear Dr.,
First of all thank you very much for the help you have given on this forum. I have had two HT operation 1000 FUE(1.5 years ago, worked very well) and 2000 FUT (3 months ago). My question is has to do with HT results and I would appreciate if you could answer;

  1. I saw pictures of more than 5K-6K grafts but has poor coverage/appearance compared to some 3K graft transplantation? Even, the pictures are from the same clinic. What are the reasons of this inconsistency?
  2. Most of the pictures show hairlines and first half of the top. How about the crown? Is it a common practice to leave crown bald? Is it because the budget or a bald person does not have enough donor for front and crown so has to select one?
  3. Does hair loss stop or decrease rate with the age? My lost started at age 26-27 with diffuse pattern now I am 31. A diffuse pattern goes till NW-7 or NW-6 or NW-6 with some diffuse hair still on top and crown?

Thank you very much.

Regards

Three months after a hair transplant should not produce enough hair to show any significant results. Wait a full 8 months to see the results of the second surgery.

  1. There could be a variety of reasons for that. Patient A and Patient B could have different hair color, skin color, hair characteristics, degree of balding, etc. Or it could be different techniques used (many clinics have damage problems with FUE grafts, for example). There’s just a lot of variables that could be the reason.
  2. The transplantation of hair in the crown does depend upon many factors, again including donor density, scalp laxity, thickness of the hair shafts, and the degree of balding. It may not be possible to transplant an entire head with a Norwood class 7 pattern of balding, but I have done it in patients with great hair qualities (see Patient ZU here, for example).
  3. Only about 7% of men go as far as a class 7 pattern, about 20% a class 6/7 pattern… and most of these men with the class 7 pattern have clear signs of it before the age of 30. You need to be evaluated by a doctor like myself or Dr. Pak that specializes in this field if your doctor does not answer this question for you.

Hypoandrogenic Reaction to Propecia

what are the questions one would ask a doctor if he wanted to be sure he wasnt having a hypoandrogenic reaction to propecia? what would a doctor check, or know to find this out?

im a 25 year old male and ive been on propecia for 15 months or so. Hair has thinned quite alot while on treatment, instead of second guessing myself and reading to many websites.

i just want a medical answer! thanks for the help doc!

You need to tell your doctor what you are experiencing. Testosterone levels transiently go up with Propecia as compensation to the impact of DHT blocking. Are you asking if developing breasts is a problem with Propecia? Your questions are not clear.

Wig for the rest of my life

I’m already losing hair anyway. I have no issues committing to a hair system for the rest of my life especially with how good they look and how nice it’s been having one

Everyone makes choices. A hair transplant is complete freedom from worrying about your hair or going to the wig salon every two or three weeks for the rest of your life, plus, a hair system in a 7 year period is more expensive than a hair transplant and the costs go on and on. good luck!

I Am a 21 Year Old Female and I Want a Rounded Lower Hairline

I have a very square hairline along with a large forehead. I have heard that a forehead reduction can lower the hairline but that hair transplants would be needed as well to round the hairline out. Is it possible to round out the hairline with just a forehead reduction (by taking the forehead down/in more on the sides)?

This can only be created with a hair transplant. A hairline reduction surgery can lower it but not round it. I wrote a chapter in a bood on this subject here: https://newhair.com/wp-content/uploads/2018/11/phenotype-article-published.pdf

Will an Early Class 3 Pattern of Balding Lead to Guaranteed Advanced Balding When I Get Older?

No, because hair loss is an inherited pattern. For example, if you inherited a Class 3 pattern (just the corners of the hairline), an early appearance will not lead to more balding elsewhere. This is why I like to perform a HAIRCHECK test when I see a young man because we both want some indication as to what is going to happen to him in the years to come.

How true is it that the sooner you show signs of hair loss, the sooner you will end up bald? from tressless
