Many people have donor densities that will not support 4000+ grafts. They will develop a see-through donor area, reflecting donor site depletion. This may be your situation, and to deal with this, you might have to keep your hair long in the back of your head, or see a doctor and get scalp micropigmentation if this bothers you.

Can you receive or do you recommend getting a hair transplant while on Accutane (isotretinoin)? My understanding is that the drug disrupts cell division and skin regeneration. Will this delay healing?
Yes, having a hair transplant while you are on Accutane it is not a good idea. You should wait a year without the drug ensure that the transplants will work and grow well. There is no point in spending money and your donor hair if you are not going to have good results.

I read a while ago that if a male keeps most of his hair by the time he is thirty, he probably won’t go completely bald. It has something to do with testosterone levels. Is this true? If so, is it also true for individuals who keep theie hair by the use of Propecia or Minoxidil?
If you have not lost any hair or have any miniaturization of the hairs on your head by the time you are 30, then it is most probable that you will not lose it. The drugs you mentioned only slow down or reverse hair loss and they will not impact hair that is normal. Hair loss has more to do with DHT than testosterone, but if there is more testosterone, then there could be more DHT. Drugs like Propecia (finasteride), block the effect of DHT and it is the DHT that causes balding when the defective genes are in the person.

I’ve seen all of the questions about l-arginine, and I even asked one before, but I was only referred to another answer which didn’t answer my particular question. I’ve heard that l-arginine increases growth hormone. This doesn’t sound too good. Does an increase in growth hormone mean hair loss is a possible result?

There are reports that L-arginine increases growth hormone secretion, but it is unproven. Throughout the internet, L-arginine is a suggested treatment for hair loss by promoting hair growth, but if this is true and it does increase growth hormone, I would worry about growth hormone induced hair loss. These two statements appear contradictory, but it does reflect complex interactions that are presented throughout the internet.

I started taking propecia about a month ago. Im starting to see gains and am very happy with the results thus far. Prior to starting propecia, i had gone for a few lasic hair removal sessions for my back and arms. I have a few more sessions for lasic, but i wanted to make sure that the two processes (for hair loss and hair gain) won’t fight and i can continue with both. Thanks for the great blog. it really helped me decided that Propecia was the right choice for me!
There is no problem with performing laser hair removal at the same time while you are taking Propecia. What the laser does is burns the hair follicle and kills it, but it is not 100% as hairs that are in the telogen phase will not be impacted and the general success rate for the ‘kill’ is usually 50% per treatment. Dr. Rassman, you won’t believe this. Maybe you will. I have had I guess 7 scalp reductions. Brandy did a lateral with that Y incision, but not a scalp lift, per se. Marzola, Stough, all did a couple of procedures. I saw my scalp under bright lights (the last thing one wants as that is very unforgiving) on a big screen last night and was shocked by the 1 inch by three inch or so bald thin area in the midline. Dr. Hitzig performed two large midline graft sessions around 1999 or 2000, fixing the scar but the density leaves something to be desired. I see you mentioned Dr. Frechet of Frechet extender fame I imagine in a response to a patient. Some sort of triple flap procedure. I have some donor left but can’t get over my lack of crown density despite a large number of grafts. I know Dr. Hitzig always grafted the midline before reducing a scalp. Do grafts just not grow after reductions, especially that many reductions? I could emply better styling but I wish I had followed the late C.P. Chamber’s advice and avoided reductions altogether. He knew what he was talking about. It sounds like there is no practical fix at any price.
Your case is complex. I have no idea what densities you have or the extent of your scar and wound defect. Most people who’ve had 7 scalp reductions are left with a slot deformity. I would like to see some good pictures of your scalp from the front, top, sides, and back. I am sure that you know that scalp reduction is no longer the standard of care today, because they do not work. I have seen men who had up to 16 scalp reduction surgeries and the last ones never really accomplished anything. I wonder about your 7th procedure and what you were trying to accomplish that the prior 6 have not.
It is best to be examined carefully by an expert and then see what your needs are. Patients should make a list of priorities and then try to work down the list. Patients who have had so many procedures never really get too far down that list, so be realistic.

Leg hair is not a great idea as only 50% is in the growth phase at any time, so it doubles the cost. Also, leg hair does not grow long and typically falls out in 5-6 months. It then goes to sleep phase and wakes up to grow for another 5-6 months before falling out again.

Increasing the medication dosage will not necessarily slow down or help stop the hair loss process. Propecia is a good medication, but it is not the magic bullet. It does not completely stop hair loss and different people have different results with the medication.
As Propecia is a prescription medication, talk to your prescribing physician about changes in dosing. If he/she decides you should, that’s between you and your physician.
