How do you know that Finasteride reverses miniaturization?

How do you know this? Any evidence you can provide? Anecdotally on Reddit and hair forums – you read hundreds of posts of people “sprouting new mini hairs” (ie at the hairline), after starting finasteride, minoxidil, and more recently – dermarolling. These hairs may not necessarily be fully miniaturized – but they certainly are thinner than other terminal hairs.

I have seen patients with significant miniaturization placed on finasteride and then upon re-examining them, I saw less miniaturization as hair seem to become thicker in the same areas I examined.

Adrenaline and Hair Loss

I hear caffeine from coffee i.e. increased adrenaline and you’ve consistently said adrenaline can contribute to hair loss. What’s up with this?

I am not aware of this. I drink a triple espresso every day. Coffee drinkers are not at any higher risk for hair loss than non-coffee drinkers. Genetic balding is the cause of most hair loss.

I wonder what his age was in the balding photo. Most men who develop a Class 7 pattern have it before their 30, mostly before they are 26 years old. The picture on the left must be his high school picture. I have little doubt that men like Larry David confronted with extreme balding at an early age, are impacted in such a way that it changes their lives. Maybe he owes his talent development and his career to his balding, a positive spin on balding for him.

After 13 Years on Propecia, Could I Be Developing a Facial Rash From It?

Hi, I am 43 year old and been taking propecia 1 mg per day and 5 % minoxidil. I have taken these for the last 13 years. The results have been amazing, probably a little less hair on top and sides now. As far as I am aware you didn’t/don’t think I have had any side effects. Well I have a slight none itchy rash above my upper lip. I have got very worried about using the propecia so I want to give this drug up. How would I scale down the drug and over what time period please. I have already halved the tablet each day. I do hope you can help, there is just so much worrying data on net saying about terrible side effects after finishing off propecia. Thanks in advance

The dosage of any medication has little to do with the allergic response. If you are allergic to it, cutting the finasteride pills in half will not solve your allergy issue. It’s possible that either Propecia or minoxidil could cause allergic reactions like you described, but you could and probably should see a good doctor for the discussion.

Stopping the pills or the minoxidil for a couple of weeks will have minimal impact on the hair, but will address the allergy issue if that is the cause.

After Taking Finasteride, I’ve Shed Short Hairs Near My Sideburns

Hello, I am 21 years old and have been on Finasteride for three months now. Since that time, I believe I have seen benefits from the medication (I no longer pull out any ‘stray’ hairs whenever I run my hands through my hair). However, I have been shedding some short, thin hairs on the sides of my head, specifically above my sideburns. The hair on my sides has always been a bit thinner than the rest of my hair, so is it possible that the medication is reacting harder to regions that are genetically weaker than elsewhere on my head?

I haven’t heard that before. Anything is possible, but likely not probable. Perhaps there are other issues and you do not have the correct diagnosis of androgenic alopecia.

There is also a rare condition known as triangle alopecia where men lose hair on the side of the scalp in the shape of a triangle at or near the temple peaks on the side of the head. This condition is usually genetic and may impact only one side. Generally Propecia does not work for triangle alopecia, and if this is what you have, transplants work well for it. This is all guessing on my part. You should follow up with your doctor.

Lichen Pilinoplaris

Hey, Dr. Rassman. Thank you for checking in. Unfortunately (or, perhaps, fortunately, in that I have an answer) I was diagnosed today with lichen planopilaris. It seems worst case in a way, but again, I’m relieved to have an answer. Starting injections soon. This is becoming more and more common in younger men and men of all ages. We used to think that this was a woman’s scalp disease and now we know better. You must get a good doctor when you treat hair loss, as this man struggled to get the proper diagnosis. If he was to be transplanted by some over-zealous hair transplant surgeon, it would have failed because the disease that took his hair away would have done the same thing to his hair transplants.

Alcohol and Finasteride Taken Together

You’ve mentioned in the past that alcohol and finasteride is safe.

But…is it as effective? I know some other drugs are less effective if you have alcohol in your system…something to do with the liver or kidneys working overtime to filter out the alcohol, and being unable to really focus on absorbing all the finasteride.

WineIt really depends how much alcohol you drink. If you drink to a point of a drunken stupor every day or have cirrhosis of your liver from all the drinking, or have dietary deficiencies associated with your drinking, I would think many things are not safe.

From a biochemical point of view, both alcohol and Propecia (finasteride) are metabolized in the liver by an enzyme called cytochrome P450 (Cyt-P450), so there is definitely an interaction (as with many other drugs). If you have one or two glasses of wine with dinner, I doubt it would be a problem.

Lockdown! Should one take Vitamin D?

It looks like a lot of us are going to be going into lockdown for the foreseeable future, which means a lot of us aren’t going to be getting enough Vitamin D. Now, Vitamin D does have a proven link to hair growth. It’s not going to stop AGA but it’ll help keep some of the thickness and generally just make us feel healthier in these times. Stay safe

If you live in cold climates, have to stay indoors for a long time, don’t have a tan, then taking daily Vitamin D supplements may be a good idea for your hair. Vitamin D only works in the presence of a Vitamin D deficiency.

I have been transplanted twice. Both times, this bald spot, which was transplanted, didn’t seem to take the grafts and grow out like the rest of the hair.

You most likely have Alopecia Areata. This condition will cause the hair to fail to grow in an active area of the disease. I recommend that you see a dermatologist. He will look at it via microscopic examination and possibly find ‘exclamation hairs’. He will also perform a biopsy to confirm the diagnosis. This is a critical step for you because this disease can get worse.
