Assuming that you are a Norwood Class 3 at this time and do not have a crown component, it is likely that you will not advance to a significant Norwood Class compared to most people your age. If you take the drug Finasteride, it will not advance the balding pattern. With these factors, it is possible to restore the balding area to a “full level,” provided that your donor area has at least average donor density (which could be a limiting factor). You are asking to restore your recipient site density to near normal in order to cut your hair very short; I think this is worth discussing with your hair transplant surgeon.

Are hair systems really unnoticeable? I am considering gluing the system on, is this the best way to apply ? What’s the best shampoo/ conditioner to use? Do you think that I should just rock the bald? (Devil’s advocate) Any tips/advice? I am planning on going scuba diving in September, will I be able to do this with the system in place ? For those that have used a system before, what was the first time after like? Has it ever came off at an inopportune time? How long do the pieces actually last? I’m hoping for it to last about a year, is this a reasonable expectation?

Hair systems or wigs are expensive over the long haul, running in the thousands of dollars forever. They are usually detectable and hard to keep in place unless they are heavily glued and with long term use they tend to develop an odor from your sweat. You can’t scuba dive or swim freely and you always look at people’s reaction trying to see if they can detect that you have a wig on. They do have challenges when you are with a woman who might want to grab you by your hair, not good. The average hair system last 2-3 years and then must be replaced. The less expensive ones last a year or so before they fall apart. The advantage of a hair system is it almost looks like real hair but most people can tell it is a wig.

I have heard you say that hair transplants are not an option for young men under 25. If you are confident about getting good results for NW7 people, why are young men who are heading to a Class 7 pattern not good candidates?

The supply of donor hair in a Class 7 pattern is only 20% of the total original hair. Only half of it can be used. That means that 10% of the original hair will have to cover the balding area (80% of your scalp). Unless you have great donor density, the math does not work out. If you combine it with Scalp Micropigmentation, the reduction of contrast can make it work.

So, let me contradict myself a bit, as there are few of Class 7 patients who had hair transplants (https://baldingblog.com/2007/05/25/difference-between-norwood-class-7-patients-with-photos/). The first one had 10,000 grafts and a very high donor density, so his results were fairly good. The last patient had very fine hair, about 5,000 grafts, and looked good from the front. He used a comb-back which he didn’t see in the mirror.

After everything has settled down, you should know if this is a problem. I always trim the the skin from FUE grafts to prevent this from happening. At the fifth month, if this is still a problem, see your surgeon.

From the recent issue of the Hair Transplant Forum and also an article in the recent issue of the Journal of the American Medical Association, there is a report that CBD may work on preventing hair loss similarly effective but by a different action to Finasteride. There is no mechanism suggested for this finding at this time.

The evidence is now mounting that a virus, identified as Adenovirus-36, is responsible for obesity. The evidence is seen when antibodies are picked up in 30% of obese people compared with normal people who had a 5% incidence of the antibody. The virus has been found in flocks of chicken in India that made the chickens grow fat. When the virus was injected in mice and monkeys, they gained weight. A study was performed on women with breast cancer, and it found that 81% of obese women with breast cancer had the antibodies to the virus. We have long believed that obese women have a higher incidence of breast cancer. When the virus was exposed to breast cancer cells in a dish, they multiplied twice as fast as normal cells. Now, with that said, there does not seem to be any funding to find out if there is a connection, but the connection to cancer is not being overlooked.

First, thank you Doctor for your years of good advice offered through your site and elsewhere. I am light skinned, 65 and have been on Finesteride for 10 years, had a FUE procedure with an outstanding surgeon in Thailand 9 years ago, and found that I was among the 5% who never shed (perhaps due to blood pressure on the high side?). The untransplanted region in the rear of the crown seems to have thinned over time, but not profoundly and I have considered the procedure a success, until now. Recently I believe that the right side of my frontal hairline may be undergoing some sort of change, where a patch of maybe 15mm by 10mm seems to have hair that is both thinning and maybe only 80mm long. The symmetrical area on the opposite side of the hairline is unchanged the hair long and as full as transplants typically are (i.e., not especially, but cosmetically impressive). Apart from any comments you might care to make concerning the possible cause, could there be reason to believe that this situation could recur even after an second FUE? Thanks so much.
