“2 case reports where notable improvement in AA was observed after FMT was performed for recurrent CDI. Further study of gut microbiota in patients with autoimmune alopecia may elucidate disease mechanisms and provide evidence to support clinical trials of FMT in this population for whom treatment options are currently limited.”

Should I do SMP or a hair transplant?

First you must ask yourself if it is hair that you want. If the answer is yes, then you must get an objective assessment with good numbers on (1) your donor density and (2) your hair mass index. These two factors will determine if you have enough hair to cover your entire head. Keeping in mind that if it is remotely reasonable, then you might be able to combine Scalp Micropigmentation (SMP) with a hair transplant to get a full head of real hair (see here: https://newhair.com/wp-content/uploads/2018/11/Combining_Follicular_Unit_Extraction_and_Scalp.98621.pdf ). But if you decide to have SMP, you could do it first ( https://scalpmicropigmentation.com/), see how you like it. If that turns out that you rather have a hair transplant and if the analysis of your donor area points to a good possibility of having a hair transplant as I discussed above, you can then go the transplanted route. Having SMP does not preclude a hair transplant.

Female Hair Loss After Knee Surgery

I am a 26 year old female, just had knee surgery last year june 2005. I have been experiencing a tremendous amount of hair loss since then. I’ve read that it could be because of the anethesia, so, my question is how long after will this continue.

One of the manifestations (or presentations) of female genetic hair loss is that some women have hair that sheds easily with stress and then comes back. You will have to wait this out.

Should I Transplant Behind the Balding Area Where I Might Lose Hair Later?

I’ve been looking around on internet to try to learn as much as I can about hair transplants, and I know quite a bit now, but there is one question that I can’t seem to get answered. I have a slowly receeding hairline, not very bad but definitley too much for 27 years old. I have healthy hair on the back and sides of my head but on top it’s a little thin and I think I may eventually loose it all on the top like my grandfather. I’ve tried rogaine and propecia but no dice.

My question is, would it not be a good idea to get a transplant seeing as I might lose hair behind the transplant later? Or is it possible to progressively do other transplants?

I do not believe that a Norwood class 2 pattern is worth transplanting. If you are evolving to a mature hairline, stay on Propecia and see what happens. If the hairline is actually moved up beyond the mature hairline into a class 3, you can transplant it.

I wouldn’t allow any doctor to transplant into an area that has not fallen out under the excuse that there’s anticipated hair loss (behind the leading edge of your loss), as this will only accelerate the thinning. Some doctors do this, and it isn’t in your best interest, only serving to put your money in his pocket.

Female Hair Transplant Without a Line Scar?

Hi I am a 20 yr old female wondering about a hairline transplant. A previous question on your site about evening out a widows peak was the most similar to my request. I have had a high hairline on the sides for several years and a recent year on medications has now thinned out the hair on either side near the temple. I have always hated my hairline which is ill suited to my face shape and want to at least fill in the hairline around my temples. I am assuming hair grafts would be the correct procedure and want to know more about them.

HAving watched documentation of several female hairline transplants I wonder : Is the donor area for the grafts always left with a long scar? (example had one across the back of her head) CAn this be done without any scarring or difference to the doner area? and (of course) what would be the cost for this procedure?

There are 2 techniques for hair transplantation — the strip method (with the linear scar) and FUE (where you are left with small punctuate scars in the donor area). There’s no surgical procedure without some level of scarring, though trying to minimize the scarring is the best we can do. Everyone scars differently, and closure techniques for the strip method have resulted in extremely thin linear scars, and the length of the scar depends on the amount of grafts required. Costs also depend on how many grafts are needed, with FUE fees being higher due to the more time-intensive nature of the procedure. You can learn more about FUE at the following pages:

Shoulder and Back Pain from Propecia?

Hi doctor , i have been taking propecia almost 8 months . I am 28 years old and do not have thinning anywehere but some receeding from the front sides. I was taking to slow down my receeding. Two weeks ago i decided to take two pill per day and i took it 5 days like that. And when i didnt see any difference i stop taking two pills and went back to one pill. Just two days after i went back to one pill .I started to realize that i have a pain on my upper back and shoulders and my shoulders started to move forward like a body builder. I stop taking propecia one week ago to see if it cleans whatever damage i had . Is it possible that something like this can happen in a short time. Now i have shoulder pain and upper back pain time to time during the day. Is the hormones has something to do with it.Thanks.

I have never heard about Propecia causing shoulder, back, and joint pain. I do not believe that blocking the conversion of testosterone to DHT will cause these muscular and joint problems. Have you been carrying some heavy loads or started new exercises which could cause your shoulder or back pain? Look for other causes for the problem, but get to see your doctor and do not treat yourself.

Female thinning (photo)

I have been getting thinner and thinner. I am a woman age, 34. Any suggestions?

Women with progressive thinning require a trichoscopic examination where the doctor magnifies the hairs on the scalp and looks for miniaturization. Some women just develop premature Age Related Thinning where there will be no miniaturized hairs on the scalp. In ART, the best treatment is to use thickeners iin your shampoo and conditioner which will increase the thickness of the hair shafts as it is forced to absorb water. An alternative is to use either topical concealers (like Tokkik) ot Scalp Micropigmentation (a tattoo of the scalp make to look like cut hairs). If your hairs are miniaturized in some areas, you need a hair specialist to assess that for you.

Since I Have Been on Finasteride, I Lack Orgasms.

My ejaculate hasn’t reduced at all really. The only symptom I have is lack of orgasm. I’m 100% sure it’s not psychosomatic as I had a wet dream a few days ago and came in my sleep, and it didn’t wake me up because I literally didn’t feel anything.

You may be right. If you have an ejaculate and no orgasm, it must be psychological as the ejaculation almost always accompanies orgasms. If it occurs during sleep, it is called nocturnal emissions, a common occurrence in young men.

Finasteride, DHT and Apoptosis

I mean, they’re not really wrong. The only way to stop balding is to never go through puberty. Futhermore, hair loss happens with age, as we slowly slink towards the grave. Minoxidil is a growth stimulant that doesn’t address the mechanisms for why we bald, it simply provides a cosmetic effect, which is what most people care about. Finasteride helps reduce the amount of DHT below a certain threshold that would cause hair loss otherwise, but as you age, the body may become more sensitive to DHT. These both work, and most people are happy enough to go on them, but we’re still balding. Some people have wonderful, Disneyland results, but they’re still losing their hair, and it would go as quickly as it came if they were to cease treatment. It sounds like your derm is just managing your expectations, as we do here; It’s easier to keep what you have than gain anything back.

I always say that when you are 17-18, something happens and the hair loss starts like the gate of a race track. We know that there is a process called Apoptosis (cell death) which occurs with hair follicles that are genetically programmed to shut down. Each hair follicle has a number of cycles that they will have before ‘dying’ and as the typical male has a 3 year growth cycle, if the hair is destined to not come back after 6 cycles, that is 18 years, 7 cycles is 21 years, 8 cycles is 24 years. We see much of the hair loss in those under 24, so it appears that 8 cycles is a magic number. Finasteride, extends the anagen (growth) phase in hair so these loss cycles slow down while on the drug.

Sleep and Hair Loss

Does sleep (or lack of) have a big impact on hair and worsen MPB?

We know that people who do not get adequate sleep are stressed. Stressed people lose their hair (common in women) and accelerate hair loss that is part of the genetic male patterned baldness process.
