I have seen thinning and hair loss in the nape of the neck with Finasteride. I have also seen reports of loss of body hair while on the drug.

Hi Doc, Does the stiches which takes place at donor area hurt after HT? any headaches does it has to be protected from Sun etc for a long time

Stitches are something you feel, but I would not say that they hurt. I am talking from my experience with stitches after my two hair transplants. The back hair covers the stitched area so you do not have to worry about the sun.

Hey Doc- I had a quick question. Have you seen any patients have hair loss worsened by Fin? I took fin from roman for maybe a week and had such extreme shedding immediately that I stopped. Shedding continued for 8 weeks or so and stopped but I went from a NW2 to a NW4 in that time. Do you think this will grow back? I tried Min, but the sides were bad for me and made it extremely uncomfortable. Thanks!
I have heard about shedding, but I haven’t seen it myself in my patient population so it must be rare. I gather that those who have it should stay on the medication as their is a delayed response but the response usually does happen in a couple of months.

I have been on finasteride for more than 10 years, then I decided just recently to stop it. I lost basically everything in 3 months when I stopped min/fin. I started back 30 days ago is it possible to regain everything?
It is possible that some or all of the hair is still there. Good luck with a potential reversal. Let me know in 3 months so I know. I always want to be educated by the men who are balding and dealing with such problems.
Hi, I’m 21 years old and I started thinning about a year ago, primarily on the temples and in the front. I started taking 1 mg of finasteride daily about 8 months ago , and I’ve been using 5% minoxidil. My temples grew back 100% and but I’ve noticed the front is still thinning and I’ve been losing alot of hair in the shower. Is it fair to say that the finasteride isn’t working for me? I don’t know whether to let nature take it’s course and just save my money. Is there a chance that finatseride will start to kick in ? Also do you have any news on the new buzz going around about hair cloning? Is there a possibility that it will be available in the near future?
You should give Propecia at least 18-24 months to peak out its benefit. When you say it is not working, I assume that you mean you are still losing some hair. The normal hair loss rate is about 100-150 hairs/day and with that comes another new 100-150 hairs that start growing. Could it be that Propecia reduced the rate of loss and because it is still negative, you are assuming that it is not working? Whatever you do, I strongly feel that you should stay on the drug for the full 2 years. As you have indicated that you are in Los Angeles, you should be under a doctor’s management here. Hair cloning is not coming down the short-term highway. Maybe we will see hair cloning in 5-10 years in the research stage, but for someone of your age, you have to surround your problem well before that process is on the short term horizon. i’ve been taking propecia for 1 year and a half with no success. But it seems that for me it’ll work in small stretches then when im feeling stressed, ill have a strange feeling in my throat, and any hair ive maintained in a short period of time will fall out. it’s very visible. This is obviously uncommon, does it make any sense? if so is there any prevention for something like this?

Stress may be central to your problems, but all of what you think you are seeing may not be what is actually there. Work on the stress with some stress reduction process and stay on the Propecia if you really have male patterned balding. This is a wonderful drug to slow, stop, or reverse hair loss in men, particularly for those under the age of 30.

I am 34 years old and I have a receding hairline. Now, I am getting a bald spot on the top of my head. What should be my first course of action to save my hair?
The first course of action should be to see a competent doctor who understands the male pattern balding (MPB) process. He will examine the hair on different parts of your scalp and look for the amount of miniaturization that you are showing in various locations. The things he will identify will not be the level that you can see with the naked eye as balding occurs slowly and insidiously and can not be picked up without high powered examination of the hair. Based upon a proper examination, he should develop a master plan for you that will cover your worst case scenario and probably include the use of medications for MPB if that is your diagnosis.

The effects on scalp and serum dihydrotestosterone (DHT) of different doses of a novel topical solution of 0.25% finasteride (P-3074), a type 2 5?-reductase, were investigated in men with androgenetic alopecia.
Two randomized, parallel-group studies were conducted. Study I: 18 men received 1 mL (2.275 mg) P-3074, applied to the scalp once a day (o.d.) or twice a day (b.i.d), or 1 mg oral tablet o.d. for 1 week. Study II: 32 men received P-3074 at the dose of 100 (0.2275 mg), 200 (0.455 mg), 300 (0.6285 mg), or 400 (0.91 mg) ?L or the vehicle o.d. for 1 week. Scalp and serum DHT and serum testosterone were evaluated at baseline and treatment end.
Change from baseline in scalp DHT was -70% for P-3074 o.d. and approx. -50% for P-3074 b.i.d. and the tablet. Serum DHT decreased by 60 – 70%. The doses of 100 and 200 ?L P-3074 resulted in a -47/-52% scalp DHT reduction, similar to the 300 and 400 ?L doses (i.e., -37/-54%). A -5.6% inhibition was observed for the vehicle. Serum DHT was reduced by only -24/-26% with 100 and 200 ?L P-3074 and by -44/-48% with 300 and 400 ?L P-3074. No relevant changes occurred for serum testosterone.
The novel finasteride 0.25% solution applied o.d. at the doses of 100 and 200 ?L results in an appropriate inhibition of scalp DHT potentially minimizing the untoward sexual side-effects linked to a systemic DHT reduction.

What can you tell us about FlutaGel?
Flutagel is a gel that contains 1% flutamide as the active ingredient, a drug that is used to treat prostate cancer. It acts by inhibiting the uptake and/or binding of dihydrotestosterone (DHT) to the target cell receptor. To date, no clinical studies have been done to prove its efficacy in MPB. In theory it sounds great, but this does not always translate into the clinical arena. It needs to be studied further to evaluate its safety and effectiveness.
