Has Miniaturization Mapping Been Written About in Peer-Reviewed Publications?

First, let me thank you for the valuable resource your blog provides. I understand that it is in men’s nature to become concerned about hair loss as we age. I also understand that you have answered this question, or at least very similar questions many times. I’ll ask anyway, because you seem like a good guy.

I am a 27 year old white man. I have severe depression and OCD. I recently (past couple of months) started obsessing about my hair. I’ve always been overly concerned about the way I look, and I’ve always been severely lacking in self confidence. When I tilt my head back and look up at the front of my hair line in under direct light from a couple inches away, I can see scalp going back about two inches. When I tilt my head forward, I see no scalp and my hair looks the same as it always has. There are a few long hairs growing below my hairline. I dont notice a change in my hairline when compared with photos taken when I was in my early 20’s. If I pull hard on my hair around my hairline, nothing comes out. Same with the crown. My mom’s dad died with a full head of hair in his 90’s. My dad thinned over the course of his life, but didn’t really go “bald” until he was in his early 50’s. He’s now maybe a norwood 3v or 4. His dad had the same pattern, except it didn’t kick in until later in life. My mom’s brother went bald young. My maternal great grandfather had Homer Simpson’s hair by the time he was 30 or so. My dad’s brother still has a good head of hair in his 50’s. What do you think (and I know you can’t really tell without a miniaturization study)… which leads me to my next question.

Is a miniaturization mapping something that is common among dermatologists? Do most of them (in your experience) properly perform and understand the procedure? Is this something that has been written about in peer-reviewed publications? Will a dermatologist that does not specialize in hair loss be able to correctly diagnose what is going on with my hair (if anything)? Thank you very much for your time.

If you know you have OCD and are starting to obsess about your hair, I think you answered your own question about what my thoughts are. You’re seeing possible thinning when your head is tilted and you’re under direct light. The hair will generally appear thinner under harsh lighting.

I wrote about this when I applied for a US patent in 1992. I published it in various articles, not suggesting mapping the head, but showing the connection between balding patterns and miniaturization. The rest is not rocket science. It is not exciting to map out the scalp for miniaturization and actually if the doctor is knowledgeable, then a good sweep of the scalp will give the needed information for a well trained physician.

Many doctors now have video imaging systems in their office, like slightly more expensive versions of the USB video microscope mentioned in the Mapping Your Own Scalp series. For around $100, you can buy this microscope for yourself and get the mapping started. One of the biggest reasons that I created BaldingForum.com was because of comments like yours — that forum gives site visitors the chance to empower themselves by mapping their own hair and making their own diagnosis, following the process over time with aging, drugs, etc. You can share your results with others and get feedback from other site members, not just from me (time is always a problem for me, because I can’t just get to everyone). Has GSK Abandoned Plans to Market Avodart for Hair Loss?

what does it mean that they won’t pursue further studies because of market research analysis?

Some doctors in the know have told me for some time that GlaxoSmithKline (GSK) had no real intention to get into the hair business with Avodart, but their recent study may have reflected their playing the options for the future. According to the ClinicalTrials.gov site, GSK is still recruiting participants in a study of Korean patients to be concluded in July 2013… so I really don’t know what they’re doing.

As I am not an insider, I really can only speculate like everyone else.

Has Finasteride Made My Penis Small?!

Hi Doctors,

I am a 23 year old male. I started taking Dr. Reddy’s 5mg Finasteride about a month ago in 1.5 mg doses. I have noticed that it has made my penis decrease in size in it’s flacid state (it’s like right after coming out of cold water), as well as a tightening of the scrotum.

I stopped for about a week and the problem went away. I tried to start taking it again today and the same problems occur.

I have heard of people feeling this tightness in the scrotum, but not the sympton of decrease in penis size. Have you heard of this before? Should I discontinue my use of the finasteride.

I highly doubt a small penis size is a reaction from finasteride or any drug for that matter. If stopping the drug solves the problem, then you made a decision between your penis and your hair. Trade-offs in life are always made. I’d say the penis is more important than the hair, but I really suggest you see a physician in person for an exam if you are concerned.

Has anyone tried being bald?

I know many doctors who are in the hair restoration field and do not have hair transplants or SMP and seem to be happy with their balding patterns. I have come to know them and they tell me (1) They are married and they and their wives don’t care, (2) the balding look is good for them and they like to keep their hair very short, (3) when they entered the field, the quality of the work in the 1980s and early 1990s didn’t meet their sense of quality that they would require.

I find the last comment concerning for those doctors who told me this, that they would do a surgery on their patients that was not good enough for them. What does that tell you about their moral compass?

Many men look good with a bald look. Jeff Bezos, for example, looks great with his balding and his hair cut short. Actors like Gene Hackman who has one of the best balding patterns to fix, has established a character for his balding pattern that brings on many roles in many movies. He is just one example of men who are satisfied with their hair loss fate (I never spoke to him so I am just assuming that he doesn’t care about his hair).

Will a Hair Transplant Work Well Without Being on Minoxidil and/or Finasteride?

Your age has a lot to do with the drug decision. IF you are over 35, drugs are less important to hold on to your existing hair with a hair transplant. If you are 26, the existing hair is under threat of loss if your pattern is more advanced and you have not lost the hair that you are going to lose yet. These younger people do better on finasteride with a hair transplant because it holds on to the existing hair and prevents shock loss, which is a side effect common in men in their 20s who get a hair transplant.

Harvesting techniques and their value

We need to talk about good medical practices. Many surgeons believe that FUE works well provided that in a person with an medium hair thickness and an average donor density, has no more than 3500 FUE grafts removed to avoide a see-through donor area. 3500 grafts for such a person will cover Class 3, 3a, 4a, 5a patterns of balding relatively easily. Once you get into class 4, 5, 6 and 7 patterns, then an FUT may be a better answer either as a sole harvesting technique or combined with FUE. As the hair thickness gets finer (less and less hair bulk/mass), the maximum number of FUE grafts drops because by maintaining the same harvesting number in a person with fine hair, the see-through donor area may become a problem. With coarser hair (more hair bulk/mass) and an average donor density, more FUE grafts can be safely harvested. As the original donor density drops, the see-through donor area risk rises so FUT might be a better choice as a see-through donor area is a real risk for low density donor areas in many men. High original donor densities with medium-coarse or coarser hair are good candidates for FUE in the more advanced Norwood Patterns. If you combined higher than normal donor density with a coarser hair, this person can easily have most of his head transplanted with FUE alone. I have never seen an FUE result in a Class 7 patient with fine hair or one with significantly reduced original donor density and not have a depleted donor area (see-through donor area). I believe that there is a role today for surgeons to be skilled in both FUE and FUT and based upon the above factors and the surgeons preferences, the right harvesting method for the right reasons will be selected.

The doctor should discuss a plan when he/she intends to harvest hair outside of the donor area, as shown in this picture. A procedure with 4500 grafts and the harvesting FUE patterns shown demonstrate the problem. If his balding pattern increases, many transplanted hairs will fall out because many hairs/grafts were taken outside the safe donor area. FYI: The safe donor area is a 2 1/2 – 3-inch high area from the base of the skull around the side of the head above the ears to the temple peaks.

Harvesting More Grafts with FUE

I’ve had two procedures with your group and I just read your posting about the progressive nature of hair loss. My hair is really thin now and getting worse.

With the new procedure FUE, is it possible to extract more grafts than the old method all things being equal? If so, could you please explain.

To answer this question would be mostly speculation, as each case is different. If you already had strip harvesting, then at some point if you needed more hair, FUE can harvest hair above and below the point where the strip was harvested. So, yes there should be more hair that can be harvested in combination with the strip harvesting you had. Also, make sure you are on Propecia to slow down or stop the hair loss. If you are not on this drug, you should be.

Harvesting Hair for Transplantation From Other Areas with FUE?

Is FUE able to harvest in regions that were considered unaccessible with strips? I think about the temple areas and also the region called the “pigtail”, which in my case seems very dense.

Yes, harvesting the areas you are talking about can work. It is very difficult to do and the yield will probably be less, because hair densities tend to drop on the sides of the head.

This is another patient who had his donor area over-harvested with a see-through look that he did not expect. He was never informed that this could happen. He wants something done about it and I recommended Scalp Micropigmentation for him (see: https://scalpmicropigmentation.com/scar-covering/#!) which should easily handle the appearance of the see-through donor area. His doctor should have told him that this could happen.
