Finasteride – What Are The Real Sexual Side Effects?

The concept of a nocebo (a psychogenic effect of a drug) has been discussed in the various studies which focus on the sexual side effects of the drug Propecia (finasteride). The significant ‘hype’ on the internet, has driven many people to the conclusions that if they take this drug, they will become impotent. As doctors, we must discuss potential and known side effects of any drug we prescribe, but what happens following the disclosures, the patients read the internet and find many, many panicky young men who report sexual impotence from taking the drug. Is it real? We don’t know but what we do know is that many studies from other countries have not shown the high sexual side effects reported in the United States. So is this a Nocebo effect resulting from panicky people who read bulletin boards and web sites which focus on this issue.

Dr. Robert Haber, a very respected hair transplant surgeon from Ohio, started to test the concept of a nocebo effect and the general side effects on sex drive and sexual performance using patients who came into his office. This is an early report of his initial findings.

“While I also doubt the existence of PFS, about a year ago I started asking my finasteride patients to complete an anonymous sexual dysfunction survey. For years I frankly addressed the topic at every visit, and my impression was that the incidence of sexual dysfunction in my patients was similar to the 1-2% reported in the studies. I wondered if a more objective survey would reveal anything different. Many of you may recall that I suggested we all gather this data in our offices, but there was little interest.

I have data now on over 500 patients, and much to my surprise, the overall incidence of sexual dysfunction in my finasteride patients is 25%. When I started seeing this number develop, I started giving a similar survey to all male patients not on finasteride as well. I have a much smaller sample size thus far, but the overall incidence of sexual dysfunction in my non-finasteride patients is 24%!

So I think that when we trust our direct face to face questioning of our patients regarding sexual dysfunction, [we should ask our selves if the patients are fooling themselves] that they are being honest. Apparently, sexual dysfunction is very common overall, so its not at all surprising that patients with this problem will try to blame something, and when there is a target like finasteride, both physician “experts” and lawyers will capitalize on the opportunity.”

in this article back in 2005 you mention that the pseudohermaphrodites had a diet high in finasteride? I am kinda confused here. so pseudohermaphrodites ate finasteride due to them having low DHT?

I think you misread the post. A diet that was high in a finasteride-like substance caused pseudohermaphrodite characteristics in children of a mountain village in the Dominican Republic. These people did not know of finasteride. It was just a part of their diet. In the end, this was how finasteride was discovered. That is why finasteride is not to be taken by women who are pregnant and especially in the first trimester of pregnancy when the sex of the fetus establishes the gonads.

Please note the taking finasteride itself does NOT cause pseudohermaphrodites. These tidbits of information can sometimes be misconstrued into a wild tangent and taken out of context.

Finasteride vs Hair Cloning

Hi, I’m 21 years old and I started thinning about a year ago, primarily on the temples and in the front. I started taking 1 mg of finasteride daily about 8 months ago , and I’ve been using 5% minoxidil. My temples grew back 100% and but I’ve noticed the front is still thinning and I’ve been losing alot of hair in the shower. Is it fair to say that the finasteride isn’t working for me? I don’t know whether to let nature take it’s course and just save my money. Is there a chance that finatseride will start to kick in ? Also do you have any news on the new buzz going around about hair cloning? Is there a possibility that it will be available in the near future?

You should give Propecia at least 18-24 months to peak out its benefit. When you say it is not working, I assume that you mean you are still losing some hair. The normal hair loss rate is about 100-150 hairs/day and with that comes another new 100-150 hairs that start growing. Could it be that Propecia reduced the rate of loss and because it is still negative, you are assuming that it is not working? Whatever you do, I strongly feel that you should stay on the drug for the full 2 years. As you have indicated that you are in Los Angeles, you should be under a doctor’s management here. So if tissue levels remain for 6 days, would topical application every 4/5 days be theoretically still somewhat effective?

Some amount of finasteride stays in the tissues for more than two weeks after stopping the pill. Daily dosing is the ideal way to build appropriate tissue levels.

Finasteride then dutasteride and my hairline is still receding

I’ve seen a lot of your messages on reddit being very helpful for people combating baldness. If you have a moment I hope you could help me with a small question. In my mid 20’s I noticed my hairline begin to recede and went on Finastride. It seemed to work great. My hair recession slowed down significantly. However, when I turned 34. I began to notice my hair line was receding again. I’m aware that drugs like Finasride, more or less, just slow the process down. But I was alarmed at how quickly my hair seemed to be receding all of a sudden. It was like the drug simply stopped working. I panicked and with some research I found that Dutasteride was an alternative that’s been shown to be slightly more affective. So I switched, from 5MG of Fin(quartered) to 0.5 of Dutasteride. That was approximately 10 months ago and my hair continues to recede and thin, at what feels like an alarming pace.

So here is my question. Have I made a huge mistake in switching? Will I lose all the hair that I kept on Finastride even though I’ve switched to Dutasteride? I thought that if Dutasteride worked it would keep the hair that Fin was holding back. Or perhaps there was no slowing these effects any longer, whether it was Dut or Fin? I don’t know what to do.

Dutasteride and finasteride can be interchanged without negative effects. What you are describing is the inevitable changes in progressive hair loss and I doubt with the history you gave, that any medication will help you at this stage. If your hairline bothers you, a hair transplants is the only option.

Finasteride the very good and the very bad in my life

I’ve been on finasteride for 3.5 years. Without it l would be fully bald by now (25) and it’s allowed me to keep a good amount of hair. Generally doesn’t look like l am suffering from hair loss. A bit thinner on the top but decent overall thickness.

I’ve had enough of taking the drug. I have come off it several times during those 3.5 years and l feel remarkably better off it. I feel more significantly more virile and energetic. When l take Fin my libido drops, brain is foggier and l just don’t feel as good. My face is rounder and less defined as well.

Are there alternatives to Fin that work?

One of the options is to shave your head and get Scalpmicropigmentation (see: https://scalpmicropigmentation.com) which can take on the appearance of a full head of shaved hair and this can be used as a back-stop in the case the other treatments fail to maintain the full look. Oral minoxidil may work in your situation but you will have to be managed by a good doctor. A hair transplant doesn’t require medications, so there are a few choices available to you. With your reported side effects from finasteride, if I were your doctor, I would advise you to get off of the medication.

Why Does Finasteride Take so Long to Get Results?

If the hair received immediate benefits, it still takes time to grow out at 1/2 inch per month. It just takes time to see the impact on the growth of the hair follicle. We see that its greatest benefit appears in a year, with more in the second year on Finasteride. That is an observation that doctors have made over many years of experience.

Finasteride Substitute Suggested By Patient – Your Feedback Is Important

You performed hair surgery on me in 2009. I thought you might be interested in something I’ve discovered.

Since 2009, I have been taking Finasteride. It was very effective in stopping hair loss, and even regrowing some of the more recent hair losses. The problem was that after several years of taking the drug, my system seemed to become less responsive to it, and regrown hair started falling out again. I increased the dosage from ¼ pill to ½ pill. That was effective at stopping additional losses and some regrowth. Then, gradually, the new, higher dosage became less effective, as well. I then increased the dosage to a full pill per day. The same phenomenon of hair regrowth happened again. You advised me (see below) to not exceed the 1 pill per day dosage. The situation seemed stable, although not as much hair regrowth as when the drug was still relatively new to my system.

I started taking this supplement in addition to the Finasteride in February, 2016. Over the next several months, I noticed the hair started regrowing thicker than it had ever done previously, even when the Finasteride was new to my system. Then, in July 2016, I decided to discontinue the Finasteride to see what would happen. Since discontinuing the Finasteride, my hair growth has stabilized, and no significant hair re-loss has occurred. That has now been nearly 5 months of stable hair growth without Finasteride (Restore FX only).

I have also noticed that since discontinuing Finasteride while still taking Restore FX, the adverse effect on sexual performance (e.g. decreased semen flow) has diminished.

My finasteride story for the past 6 years

I read a lot in forums that if loses its effectiveness after 10 years. I’ve been on the drug since I was 19 at the first signs of miniaturizing. I may have confused the miniaturizing with a mature hairline. I’ve now been on the drug for 6 years and I’ve kept exactly everything I ever had, I had a Norwood 1 with small amount of thinning.

Maybe you were never balding and misjudged your situation. If I were to have seen you years ago, I would have done a HAIRCHECK test and if I didn’t detect hair loss with this accurate instrument, I would never have prescribed the drug to you.

Finasteride Stopped Working and My Doctor is Too Busy To See Me

Good Afternoon!
My doctor prescribed Propecia 1 mg per day, 3mg per day of Biotin Forte and wash my hair 3 times a week with Ketoconazole shampoo in 11-6-03, follow up was one yr later 04. I started generic Finasteride 5 mg to save $ 6 months ago. I break the tablets, everything was great until June of 2011. I have had recent rapid hair loss on the crown & back of head.

My doctor is too busy, takes months to get appt. Any referrals? Suggestions maybe closer to AZ? No side effects except I have a sore left Testicle for about 3 yrs now, my doc cant figure out why its sore? Im now 51. Thx!

I think that if your doctor does not have the time for you, you need to find a doctor who does. I don’t have a referral for you in Arizona, but you can check out the physician search at ISHRS.org.

As for the recent hair loss starting after switching from name brand Propecia to generic finasteride — I really don’t know. It should be the same medication, just different doses. Are you breaking the 5mg tablets into 4 equal parts? Did you get the generic from a legit pharmacy? What was the maker of the generic? Have you talked to your pharmacist about perhaps trying a different generic brand?
