I am a female. My dermatologist recently told me that any kind of hair color should not be used when experiencing hair loss/thinning hair. I’m dealing with thinning hair on my crown and right side of my scalp. I’ve recently started using Rogaine and my derm advised I could use whatever shampoo I wanted, ( I was very surprised at this). I’ve been using semi-perm color on my hair for years and on occasion using perm color. All in the salon ?

Yes, hair coloring impacts the hair on top of your head. Genetic hair loss is below the scalp level so the two are unrelated. Don’t damage your hair with dyes, which can happen below the skin when it is used wrong.

Balding in the back of the head can only come from an overly aggressive surgery so anyone that looks at them knows that they had a hair transplant because balding in the back of the head is not anything found in nature. Cole’s FIT and BHT Procedures

What are your opinions on the techniques and procedures of Dr. John Cole, such as his FIT procedures which he acknowledges was developed from your FUE. However, the FIT is allegedly an improvement. Also, what do think about his claims regarding Body Hair Transplants. I know you have made previous comments regarding BHT’s. However, I am asking specifically about Dr. Coles procedures regarding BHT.

FUE and FIT is exactly the same procedure, with some technique variations. There are a number of technical ways to do it and FIT is a proprietary brand name developed by Dr. Cole to show some unique identification for him. Body hair transplants are new with not much validation from other doctors. There are only a few patients with any history to the process and we do not know much about success rates in, let’s say, 100 patients overall. I am not surprised that they may work, but body hair is limited in numbers, usually growing as a single hair unit although there are some two-hair groups. The hair has a different character than scalp hair, but Dr. Cole’s claims have no real science with them, although I am not saying that it is not true, just not proven scientifically. I would not jump into it just yet. It is, in my opinion, still an experimental procedure.

A number of times we wrote about the use of the Cold Cap for people who have cancer and did not want to lose their hair during chemotherapy. The stories I followed included a relative of mine who was able to hold on to 80% of her hair under chemotherapy for breast cancer. Another patient held on to his hair with chemotherapy for lung cancer. The value of this technology is absolutely clear and now the FDA has formally cleared the technology for clinical use. It was interesting to hear the stories from some of the patients I followed. Their doctors were somewhat resistant to this technology but because the patients insisted on it, they allow its use in their Chemotherapy Centers. Keeping ones hair during chemotherapy gives dignity to many such patients and helps reduce the stigma associated with these horrible diseases.

Cognitive Impairment from Finasteride

I’ve done my due diligence on the side effects of Finasteride, but I’m really worried about the potential side effects of cognitive impairment. As someone who is really susceptible to the side effects of drugs, I’m afraid to take Finasteride as I have just started medical school and I cannot imagine a worse time in my life to be experiencing some kind of cognitive impairment. Have you had patients complain of cognitive impairment? If you were in my situation, would you even take that risk?

Yes, I had a few patients who reported some cognitive impairment, more however stated they just did not feel good on the drug. The decision to take finasteride is a personal decision. I advised my son to take it, and prior to having his first child, he took it for 15 years and the only thing he noticed was an increase in his sex drive. Unusual, but that happens probably way more than cognitive impairment. He started on the drug when he went into Orthopedics.

If you have cobblestonning it may be related to the amount of skin that you had on the recipient grafts. Also you will find that this skin will be very sensitive to sun. I always trim off the skin when I transplant the beard or scalp so that I avoid cobblestonning and a significant skin disc which easily develops sun burn for the first year or two.

Close FUE Scars With Sutures?

Would FUE donor-area wounds closed by suture allow for relatively scar-free surgery, or would the procedure be simply too impractical and/or prohibitively expensive to carry out?

It would be prohibitively expensive and time consuming. Many people with densities of 2 hair/mm square or better will not scar significantly if the punch that the surgeon uses is 1mm or less. Some surgeons use larger punches and if the person has a low donor density, the scars will be evident as these two things are combined.
