You have a Class 4A pattern of balding with a persistent frontal forelock. The balding is behind the forelock, and the forelock looks strong. Forelocks that persist after the age of 35 often stay, and there is a strong family history in balding families with strong forelocks. For instance, a man can lose almost all of his hair and still keep his forelock. Since you reported that you are 42 years old, I think this is the case with you, but I would need to examine your forelock to see just how permanent it is and to see if there is any miniaturization in it. Weak forelocks show miniaturization, while strong ones do not. I would transplant the area around your forelock to complete the frontal hairline if your forelock is strong in addition to the area behind the forelock.

42 celebrities who had either hair transplants or hair systems
It is always interesting to know that you are not alone when you are worried about hair loss. You are fortunate that you are in 2020 when the art of the hair transplants is as good as the natural hair growing normally. Had you done it 30 years ago, you would not have been so lucky like Joe Biden (had plugs that were fixed) or Donald Trump (who had a scalp reduction and now is probably bald with a good hair system).
See if you can tell who had hair transplants and who has a hair system from this list: https://www.buzzfeed.com/kimberleydadds/42-celebrity-men-who-are-less-bald-than-they-used-to-be
Prolactin levels and hair loss
Can Prolactin in a Male, Like a Spike over 6 months caused by a medication cause some loss? I’m curious. I still have a full head of hair, but was wondering about that.
Elevated prolactin levels in association with diffuse or androgenetic hair loss can be neglected as causative for the hair loss, because there is no evidence that they have an influence to the pattern
4000 grafts resulting in donor site depletion (photo)
4000 grafts taken from the donor area pushes the donor area’s ability to support such a large procedure when the donor density is lower than average. I am afraid that too many men who push their doctors or go to clinics where very large FUE sessions are done regardless of their donor density, will find themselves bald in the back and side of the head, like this man with new balding in the back of the head.

4000 FUE grafts and now balding in the back of the head
Some doctors just push numbers to make money when they do FUE grafting. I believe that this is such a case I picked up on the internet which shows some patient victimized by too many FUE grafts. The only options to treat this is with Scalp Micropigmentation, a tattoo for the donor area something we do all the time. Overharvesting to make money, is as immoral as a doctor can get. This was a woman who from the pictures, did not have much hair loss and fully 80% of women have miniaturization in the donor area which will absolutely cause this problem, This picture is one months following the FUE surgery and I suspect that there will be little growth and she will be bald in the back of her head.

I Had 4000 Fue Grafts 6 Days Ago, and I Don’t like the Hairline Given to Me. What Can I Do About It Now?
Usually, the grafts can’t be moved at 6 days. I know because I tried to do it, and they come apart as they are pulled out. Now, you should wait until the hair has grown, then fix what you don’t like after the hair has grown in.

40 year old with hairline regrowth (photos)
This 40-year-old male reversed his hairline recession with minoxidil 6%, 0.3% finasteride topical solution as well as microneedling. I suspect that without the microneedling, he would have had no response. Microneedling is a fantastic treatment for almost any age man with balding. The problem with microneedling is “Will it last” and if so, how long without continuing the process.
I am 40 and stopped finasteride 2 years ago, thinking about going back. Any thoughts?
As you get older your testosterone drops along with the DHT and then the finasteride is often less effective as you get older. What hair you have has stood the test of time which is an indication that much of it might stay; however without seeing you, I can’t make such a judgment.
