Dr. Rassman,
I am experiencing what I think may be a finasteride side effect that is really alarming and embarrassing.effect My pubic hair has been thinning for over a year. I am a 48 year old male and have been using finasteride (1/4 proscar) for a about 7 years with decent maintenance results, and also I have been using minoxidil 5% for the same time period.
I told my GP of my concern at my annual physical this past spring and he had no answer, but included testosterone in my blood work and it came back as in the normal range at 575. I have noticed some reduced body over the past 7 years on the finasteride and attributed it to the reduced DHT. I thought reduced DHT increased testerone which is responsible for axillary hair growth? Could the reduced DHT actually be thinning my pubic hair as well? I don’t want to quit finasteride, but should I do so?
Thanks for your advice
DHT does historically cause the appearance of pubic, underarm, beard, ear and nose hair. The pseudo-hermaphrodites that were studied in the original research who were devoid of DHT from very high doses of finasteride in their diet since birth and therefore largely absent of DHT, had very little facial or body hair. I would expect, therefore, that some people may see reversal of some of the things that DHT causes with DHT blocking drugs, but prior to your email, I have not seen or heard of any patient who complained about it. There is logical reasons to see this side effect, although clearly not common in my experience. If you stop the drug, then the benefits may be lost as the pubic hair returns (if it does do that at all).
If finasteride actually reversed nose, ear, beard, and ear hair, I am sure that most men who have too much hair in these places would rush to take finasteride, but alas, the stampede did not happen. That says a great deal about this type of a hair ‘removal’ phenomenon.
I have noticed many 60 years old men’s hair start getting thin and become sparse, revealing some of their scalp skin underneath, but is this the same thing as male pattern baldness that a lot of guys having during their 30s to 50s? Those old men have a more sparse hair compared to their younger self but is still not balding like those 40 year old men with male pattern baldness, are these two situations related? If an old man’s body produces less male hormone than young men, then is that mean the elderly thinning/balding are usually not caused by hair follicles’ DHT sensitivity? Could this also be inherited?
Thinning in men over 60 may have nothing to do with balding. Hair in men or women that get older may see their entire hair become finer. That would be genetically induced. Balding is unlikely to appear in a 60 year old man

i’ve been using propecia for about 3 yrs now & minoxodil. for the first 2 yrs my hair stopped thinning & stabilized, but just this yr & more specifically the last 4-6 months. but heres the thing, im thinning on the sides & back corners where your usually not supposed to go bald or lose hair. this started right around the time i started using minoxodil & something from hair club for men on the sides cause before that i wasnt, so i stopped doing that, do you have any suggestions, thx for your time
You need to be evaluated by a good doctor that specializes in hair loss. Loss of hair from the sides and back is unusual in men and common in women. There are too many possibilities here to discuss and there is no substitute for a good doctor to evaluate you. You can contact a dermatologist that specializes in hair loss, or has an interest in this field. If I were to guess on your problem, I would have to make a list of possibilities and that may do more to confuse you than to help you.
I have been on Fin 1mg for last six months. I have started observing that my Hair, Armpit Hair, Pubic Hair and Facial Hair have started thinning and the density & growth has reduced. Could this be related to Fin ? I checked with my dermatologist. I was told that it was unlikely. Does anyone had a similar experiences ?
Some people do report a reduction of body hair when on finasteride. It is a known, but rare, side effect

I’m thinning, and i decided to shave it all off. A big thing for me is the fact that once its all shaved off, there are follicles everywhere and thus it looks like i shaved out of style and not necessity. My question can the hair follicles itself, as time goes on, stop regrowing?
Your pictures shows you are thinning a bit. Some people believe that if they shave it, it will grow back normally, not true. To find out how much hair has been lost, the HAIRCHECK instrument check will tell you (see: https://baldingblog.com/2017/01/10/value-haircheck-bulk-measurements-two-patients-seen-today/) . The hair will keep growing until such a point as your genetic hair loss (assuming that is what you have) advances to your family genetic inherited pattern. That takes time. Based upon the HAIRCHECK results, you may consider finasteride if there is hair loss confirmed and if you are under 30 years old when the drug works best.
Doctor,
I have been on Propecia for close to 6 months now and am noticing some hair regrowth on the crown of my head. Over the same period of time, I have also noticed meaningful thinning of the hair on my legs, in particular in the shin area. I have read that reduction of DHT may lead to loss of body hair. While that is not a cause of major concern, I am worried that it may also lead to the thinning and loss of the eyebrows, as some propecia blogs have suggested. Are you aware of such a side effect and is there any medication to counter the loss of eyebrow hair? Thanks.
I have heard some reports (not actual medical studies, but patient reports) that loss of body (chest/leg) hair may be a side effect of Propecia. One patient reported that his chest hair disappeared and I was envious. However I have not heard about eyebrow hair loss with Propecia use. If it is, then it is rare. Even if there is some thinning of these hairs, in all likelihood it is a temporary problem. There are no medications to counter the loss of eyebrow hair.

hi. i’m only 16 and for a year my hairs been thinning and scalp showing. i usually grow it out very long then shave it. now its shaved and once in a while theres mini hairs no more than 5 millimeters long here and there. i was also on a lot of steroids like protopic and hydro-cortisone for my eczema.
You steroids could be responsible for thinning. You need to discuss this with your doctor. There’s not much more I can offer without knowing more of your history and examining your scalp.
Ever since my hair started thinning it literally took over my life it’s all I think about I’m only 22 and I also overthink and worry about what people would say I’d they noticed it and I’m living in fear, I’m also scared how I’m gonna find a wife if I actually lose all my hair. I also can’t open up to my family about it cuz they’ll make fun of me or clown me it’s just the worse feeling in the world, I’m on oral min 2.5 for 4 months now I’m getting regrowth but I was little far back so it’s gonna take a while I also think that if might not regrow back. What’s crazy is I don’t even have mpb I have seb derm hair loss when I was 16 I never took care of my hair I wouldn’t wash it for weeks and it caused my hair to thin. If I do get my hair back one day I would not complain about anything and I’d be the happiest man alive
Not washing your hair when you were younger, did not cause hair loss. You are not equipped to manage this process, that is clear. You need to see a good doctor who can manage the process for you to make you “the happiest man alive.”
