RESULTS: We evaluated the long-term (10-year) efficacy and safety of AGA treatment with 1 mg/day finasteride in a large study population (532 patients), as the first study of this kind in Japan, to our knowledge. A high objective efficacy was demonstrated by the MGPA, which revealed improvement and prevention of disease progression in 99.1% of the 532 Japanese men with AGA treated with 1 mg/day finasteride for 10 years.
SEXUAL FUNCTION: In the investigation in 40 years or older of Asians, the incidences of decreased libido and erectile dysfunction were 6.0–38.7% and 40.6–70.0%, respectively [29,23]. Overall, the adverse reactions were all mild, and the incidence was lower than the generic incidence of decreased libido and erectile dysfunction in Asians. Dr. Rassman’s Comment: The efficacy of long term finasteride treatment has been published many times. When compared side-by-side with men who did not take finasteride, the results show that men with genetic balding hold on to more hair over time than the non-finasteride counterparts. The issue of sexual dysfunction is briefly discussed and the comment that the sexual side effects of age parallel the sexual side effects while on the drug (just slightly less according to this study). There were no cases of Past-finasteride syndrome reported in this study, a significant point.

Nervous about starting topical dutasteride!
So I’m getting 90 0.5mg pills delivered to me in the next few days and even though I only plan to apply topically, I’m still a bit worried about the half-life in the bloodstream. I’m hoping that very little will actually make it systemic, but it is much more potent than fin and last much longer. Does anyone have any experience with topical Dut???

Nerve Damage in Donor Area From Face Lift Causing Hair Loss?
I had a face lift in Aug. 2005. By his own admittance, the doctor has damaged nerves behind my ears and neck area. In addition, my healing has caused keloids, which he cleaned up once in April, but re-healed the same. I have had, and still am having a lot of hair loss.
Has the nerve damage caused less blood to flow to the scalp?
The damage has caused numbness, a sense of cold, and a sense of ‘strangling’ in my neck. I’m not sure if this can be re-paired?
The main question has to do with hair loss.

Your problems are clearly not simple and therefore I would require an examination of you with and would require more detail in your history. Keloids in the scalp are very, very rare. Nerve injuries should not impact blood flow to the scalp. Stick with your doctor if you believe that he/she is competent and has your best interests at heart.

Automation of the hair transplant process has been a goal of doctors from the earliest part of their career because of the tedious manual processes used today. In particular, with the large sessions, fatigue becomes a problem and the skill sets required of the staff take years to acquire proficiency.
I have been a pioneer of many of these devices, receiving patents on some of them. Included was the Carousel, and a series of follicular unit extraction (FUE) techniques devised for faster extraction. The NeoGraft machine is just one of the newer devices, but unfortunately it has its problems.
NeoGraft uses a vacuum system to assist in the removal of grafts that are first partially dissected with a sharp punch. The grafts that are “sucked†out of the scalp are collected in a chamber. The grafts are then removed from the chamber and drawn up, again by suction, into a needle and then injected into pre-made recipient sites using air pressure. The NeoGraft seems to eliminate some of the risks of mechanical injury to grafts in traditional FUE by not requiring forceps to remove grafts from the donor site or insert the grafts into the recipient area and, in theory, may save time by eliminating the need to extract grafts manually. A concern expressed by those who have seen the system in action is that the suction removal has a tendency to strip the surrounding tissue from the lower portion of the grafts or pull out just the upper part of the graft. Because the grafts are exposed to a great deal of air movement, the continuous flow of the vacuum adds to the risk of graft drying, a problem well known to be a major cause of poor graft survival. To our knowledge no independent studies have been performed to show that grafts are not harmed by this vacuum technique and it is our concern that the drying action of the air on the exposed grafts may limit their growth. As many of the grafts are stripped from their fat, the risks to the grafts are theoretically significant.
Restoration Robotics (RR) has a better way to do this with a robot, but it is not yet on the market and it will most probably be very expensive. RR have been successful at doing FUE, although slowly, but they have yet to demonstrate an ability to place grafts mechanically.
There is no substitute to learning and mastering the manual skills that are used in the best hair transplant clinics around the world. So without a good automation instrument, many of the doctors who use an unproven method for hair transplantation just makes the risks to the hair transplant procedure greater and the work by the doctors who use them possibly second class. Would you buy grafts from a second class surgical team and pay the consequences of poor growth?

This shows a needle puncture in the skin that is clearly present, after the needle exits the skin. For those people who get Scalp Micropigmentation, this is also present, just in the thousands. The hole usually fills in quickly from below, but when doing SMP, I always ask the patient not to shampoo or use alcohol in any manner for 3 days after getting SMP. If this rule is not followed, then the detergent will go through some of the holes and cause the tattoo ink to dissipate, making what was a “DOT” into a “Blob”

Hassan and Wong are good doctors, but I would want to know what was wrong with you in the first place. Your pattern of balding on the sides is highly unusual and should be matched to a cause. Could you have a condition called Fibrosing Frontal Alopecia which could mess up a hair transplant even in the best surgeons hands. I would ask that you see a dermatologist and get a biopsy of the area where the problem exists. You need a diagnosis, not a hair transplant right now.
