Is Finasteride Growing Black Vellus Hairs at My Hairline? (With Photos)

Hi Doc,

I’m 22-years-old and have been on Finasteride for a little over a year. I had my hair properly examined 4 months ago, and the doctor confirmed that Finasteride has increased my hair density significantly.

I just noticed little black hairs on my hairline. I’m wondering what they are? I don’t believe I had these hairs when I started treatment. Could these be miniaturized hairs that may eventually turn into terminal hairs? They seem a little dark to be vellus?

Finasteride 1mg doesn’t usually regrow the frontal hairline or vellus hairs. With that said, everyone has vellus hairs at the hairline, so perhaps you are just noticing them for the first time as you are paying more attention to it. The only real way to tell is if you have a BEFORE photo of the same are. If your doctor does bulk measurements on your hair, it would determine if there is a reduction of hair mass, usually caused by genetic balding.

Is Finasteride Evenly Distributed in a Proscar Tablet?

I would like to thank you for this very useful resource, and your consideration of submmited questions. I have a question about Proscar- since Proscar tablets are not scored four ways- is the finasteride distributed equally throughout the pill, in other words could their be more potent peices of the pill. This was a concern I had about switching to proscar. Are you aware if the pills have an equall distribution of finasteride? It seems like they may not since they are designed to be halved not quartered.

I can not imagine why Merck would weight the active ingredients in a tablet of Proscar (finasteride 5mg). I can’t say with 100% certainty, but I’d believe the distribution is fairly even throughout, particularly since the pill is likely homogenized. If it’s not perfect, it would probably be within a very small fraction of a percent difference. The Proscar isn’t scored, so you will likely get a little more of less each day anyway unless you somehow manage to get a perfect cut on an odd-shaped pill (see image at right). It’s a good question and although I did a little bit of research on it, I couldn’t find the absolute answer for you.

Finasteride can positively impact a swollen prostate and is typically used in men with prostatic hypertrophy, commonly over the age of 50. Also, in men who used it for a long time, it reduces the risk of prostate cancer by 25%. The recommended dose, however, is 5mg, in the form of Proscar.

Is Facial Swelling a Common Occurence Following a Hair Transplant?

A close friend of mine had a hair transplant and I noted some serious swelling around the face immediately afterward. I did not expect a transplant to produce that degree of facial swelling. Is that normal or is this just a rare case? If it’s rare, why do you think this occurred? Was it the surgery or the medication he was on after? It doesn’t speak to any problem with the grafts, right?

I’m considering a hair transplant but I want to use styling to hide the transplants while they heal — I can’t hide a swollen face, though.

Facial swelling can occur after a hair transplant from either the surgical procedure or just from an anesthetic alone, and it could be quite obvious in some cases. It’s not terribly common to see a large amount of swelling, though I’m not sure to what degree you consider the swelling to be “serious”.

The swelling is more of an annoyance than anything, and it doesn’t signify a problem with the surgery, healing, or future graft growth.

To minimize this risk, I use a single large dose of steroids at the time of the surgery or scalp anesthetic, and it usually prevents its occurrence. We rarely see it, but it does occur from time to time. When it occurs, it usually lasts a couple of days.

I always had little facial and body hair until I was about 20. Now my facial hair grows faster and thicker. Coincidently my hairloss is much worse then before and itches a lot more. Is facial hair growth related to DHT fluctuations do you think?

I believe that DHT is one of the hormones that stimulate our male sex characteristics, like voice change, beard and chest hair, and hair loss in the unfortunate men who have the ‘genetic hair loss’ curse. I have seen in very rare cases some patients that have disappearance or decrease of body hair with Propecia use.

Back in the 80’s & early 90’s this was an acceptable hairstyle to rock for men with hair loss. The shaved head has only been a more recent trend among balding dudes. At some point will the horseshoe be considered a more stylish option than shaving your head? I’m curious because I’m sick of balding dudes all having to rock the same haircut.

Shock loss is common when there is a lot of miniaturization. Therefore, shock loss is more common in younger men, typically under 30 or 35 depending upon what miniaturization presents.

is Dutasteride safe?

Dutasteride is not FDA-cleared for use on hair loss. It also has a very long half-life of 5-6 weeks in the bloodstream and many, many months in your body. That means if you develop side effects and stop the medications, the side effects can last up to a year. Finasteride, on the other hand, has a bloodstream half-life of 4-6 hours, and if you stop it, it will be out of your system in weeks, not months. The FDA also cleared finasteride for hair loss use.

Is Dutasteride Any Better Than Propecia for Treating Hair Loss?

My question is, is Dutasteride really better than propecia? I know its no fda approved for hairloss, but in your opinion do you guys think it is better and more effective? Doctor Rassman/Pak I know you guys rarely prescribe dutasteride, but do you guys have any patients that are on it and have had better results than they did with propecia first? also do you have patients that did not respond to propecia at all, then went on dutasteride and had good response? or at the end of the day avodart and propecia are both the same, and the more dht reduction from avodart does not make any impact more than propecia in terms of hair?

You are correct in stating that we at NHI rarely, if ever, prescribe dutasteride. Some doctors in our community prescribe dutasteride and believe that they gets better results with it versus finasteride. There may be differences in their efficacy, but the higher risk of side effects and long term effects are unclear, especially the risks of sterility that have been reported.

I have occasionally seen patients who have taken dutasteride after taking finasteride, but in my opinion their results are equivocal.
