Is DHT Essential For Genital Function?

Is DHT essential for proper genital tissue structure/function?

(more detailed: could lack of DHT lead to decreased AR expression in penile tissues & nitric oxide production… leading to tissue changes, ie fibrosis/collagen deposition which cause such shortening/lack of bloodflow?)

I know there is relevant posts to this issue where you dismissed finasteride in relation to a change in penis + testicle size but i thought it was important to bring forward my case to show that there is definately some relation with finasteride and penis size *in a flacid state*, aswell as testicle size. I am a 20 and a half yr old male who has been on propecia since about 19.

id like to point out im not saying that my penis has actually reduced in physical size due to finasteride, but in its flacid state it now resides alot different to how it use to – doesnt hang as much, looks like ive come out of a cold shower – which in turn gives the apperance of a smaller penis.

what ive experienced:
– testicle/scrotum shrinkage. they are no longer hanging as low as they use to, they are now tight and pulled very close to the body. scrotum could almost be described as empty and wrinkly
– reduction of how penis resides in flacid state. In the flacid state my penis is ALOT smaller then usual. penis doesnt “hang” nice and soft and move around loosely as it once did when to flacid. penis is now retreated /retracted as if in cold water, its almost withdrawn and seems it is lacking circulation/blood flow. penis once hung lower than the bottom of my testicles but is now actually pushed up and forward by how tight the testicles now are, and how reduced the size of flacid state is.

i found another person who experienced similar side effects to me. he stated he experienced:
– penile/scrotum shrinkage, loss of length/girth
– loss of morning/spontaneous/nocturnal erections
– empty/wrinkly scrotum
– penis constantly retreating as if in a cold pool
– loss of sensation/numbness to both penis shaft and scrotum, both feel lifeless/not pleasureable
– penile tissue changes: narrowing, curvature, darkening, emergence of numerous varicose veins along shaft

now i have not experienced the severity of his symptons, or his loss of size during erections, but it is quite alarming there are others out there who have experienced this issue and there isnt any warnings out there regarding this alarming side effect

after reading through several links below, one must question – is it possible that lack of DHT leads to decreased AR expression in penile tissues & nitric oxide production… leading to tissue changes, ie fibrosis/collagen deposition which cause such shortening/lack of bloodflow???

DHT is very important in the first trimester of pregnancy when the testicles and penis form. If DHT is not present (mother on finasteride or born with a defect where the fetus does not make DHT), then a hermaphrodite will probably develop. For more on hermaphrodites, see:

It is true that being exposed to DHT blockers before puberty can negatively affect the development of the male external genital system and other secondary male sex characteristics (beard and body hair even after puberty). DHT is essential for development and growth of the male genital system before they are completely mature and reach their final size. Our current understanding is that the use of finasteride after puberty when the amount of testosterone is very high with corresponding high DHT levels, should not impact these secondary sex characteristics and will still allow the penis and testicles to reach their maximum size. There is not much information on the use of this drug in young teenagers and I suspect that prior to the age of 16 when balding may start in a select number of boys, the decision to use finasteride is easier the further into the teens the person is.

You have referred to a study done on 3 groups of young rats. This study has shown that a lack of either testosterone or DHT can affect the growth and development of the external genital system negatively.

In the studies done on humans, there was no report of similar or any other negative changes on the external genitalia in adult men or boys in their late teens. I have been recommending and prescribing Propecia to a good portion of my patients, and as of yet have not heard any such complaints with regard to impact on penis size once the penis has grown to its final adult size. You have to remember that this medication has passed all clinical phases and was proven effective and safe for treatment of male pattern baldness (in adult males). If you are still concerned with the changes in your genitalia, you can see a urologist to make sure you don’t have any other medical problems that have been causing this issue.

Is Cutting a 5mg Finasteride Pill Not Recommended?!

Hi Doc,

So a little background. Have been on finasteride for approximately 8 months. I think its working, since I have not progressed at all and maybe perhaps (don’t know if its my mind playing tricks) have seen very slight thickening.

I asked you a similar question before regarding the generic finasteride from pharmascience. You stated that I should be good with this drug as the company is a reputable one here in Canada.

However today my question is about the 5MG. I take that and cut it into quarters and have been doing so since the beginning of my treatment (so that its cheaper for me)…I read somewhere on your site that this is not recommended….and some people/doctors claim that the ingredients of the pill are not equally divided??? could you elaborate on this…am I fine with what I do or should I switch to the generic 1MG (which is available in Canada)…do you roughly (again roughly) still get 1.25 if you quarter the pill with a pill cutter?? So just wondering if I may be loosing ground with what I do, or that I am infact fine with the above regimen?

Thanks in advance

I am not sure where you read on this site that cutting the 5mg finasteride wasn’t recommended, but I’ve been suggesting doing this for years. You just need a doctor to prescribe the higher dose with the instruction that the pill is to be cut.

Cutting a 5mg tablet of finasteride in quarters is just as good as taking a 1mg pill. In a perfectly ideal world the drug companies would make a 1mg version and sell it cheaper than the 5mg version, but for now the 5mg version is about $30 for 90 pills while the 1mg version is $100 for 90 pills.

So in the interest of cost savings, you can cut the 5mg version into four pieces to make 1.25mg. If you know how to cut a small pill into 5 equal pieces to make 1mg each, go for it! In general, when you cut the pill you will never be precise and you may get more or less 1.25mg. This is alright in my opinion as long as you consistently take the medication on a daily basis. My reasoning behind this is that studies show 0.5mg is almost as effective as 5mg dose, so the daily variation should not matter as long as the drug is taken on a consistent basis.

Is Chlorine in Water Causing Hair Loss?

Hi,
I came to Dubai 2 months back and from the beginning I am losing my hair very fast. When my hair is wet during the shower, it falls out even when I just touch them. I never had any problems with my hairs in my life. My hairs were very strong but dry without dandruff. After a month in Dubai I was feeling a little itching and I saw I was having dandruff, although I don’t have much dandruff now, but it has started. I heard Dubai water is not good for hair because it contains chlorine. I don’t want to lose my hairs and want to grow them big. I was using Vatika shampoo before Dubai and now here I have started using Sunsilk egg and also use oil to keep my hairs soft. My hairs are just falling, how to stop them and make them dark, strong & black. Which shampoo should I use. How is Head & Shoulders? Help me.

Again and again, I tell my patients that they need a diagnosis. You are grasping for straws until you get properly evaluated with a mapping of your scalp for miniaturization. Please see a good doctor first, get examined and have a proper diagnosis in hand, then look for causes if the hair loss results from other outside factors. This, a good doctor can tell you. Chlorine should do no more harm to your hair in normal therapeutic doses than water without the chlorine.

Is Bosley the Best Hair Transplant Place?

Hey doc, I have just found your website and have enjoyed it tremendously. I am 23 and have noticed my hair thinning since I entered college at 18. I tried Propecia but grew impatient. I wish I would have stuck with it but I chose to stop. I have lost enough hair now that I am fairly thin on top and towards the front. I was curious if you would recommend a transplant for someone so young. I am on the bubble about this, but I think that for my mental well being and career that really it is my only option. I am constantly unhappy with my looks and miss dating a lot. Truly the only real thing holding my back is the price, I would do it tomorrow but I just truly do not have enough saved. I think I would need extensive graphs to restore what I have lost. I have only looked into Bosley because it is the most popular and respected name. Would you recommend them or should I look elsewhere? I am very afraid of a poor surgery where I come out looking like a fool for the rest of my life because of some doctors weak job on my head. Thank you so much for your time.

You must be careful that you are not body dysmorphic. Dating and hair loss relate with regard to self-confidence certainly, but losing your hair should not be an excuse to stop living. Your points about career, mental well being, dating, happiness, (I can go on and on) are good reasons many people try to either hold on to their hair, or if they can not or have lost it, they can get it back with good medications or hair restoration surgery. Now with that said, you need to first get a diagnosis established, your scalp mapped out for miniaturization and the pattern of hair loss that you may develop. Many good medical hair restoration practices, such as Bosley or NHI, can supply a good working diagnosis for you. It is important that you find a hair restoration clinic that you feel comfortable with, and that you feel will give you the results you are looking for. I would not go somewhere based solely on marketing. Do your research.

There is much in this blog about how to shop for hair transplants (my lengthy post from earlier today, The Truth About Cheap Hair Transplants, is a good start), because budget minded people need to understand the entire shopping experience. With a diagnosis in hand, you may find out that you can reverse it (if you have it) with something as simple as Propecia (one pill a day), which works well for many young men. Be careful of the sale pressures to make you buy a hair transplant before you do your comparative shopping and before you know what is wrong with your hair. Knowledge is power, and you as a buyer are very powerful in determining your fate and the status of your hair for much of your life.

Is being bald better than having hair?

A recent study suggests that most women actually LOVE bald men. And that hairy men easily get jealous of bald people Also genetic analysis has found out that balding men have the best genes for strong bodies

Bald is better?

Shaving your head may give women a sense that you have self-confidence and women love men who are confident in themselves.

Is Avodart Off-Limits for Treating Hair Loss Until it is FDA Approved for Such Use?

Hi Dr. Rassman,

I’ve been on Propecia and Rogaine for 3 years and have been delighted with the results……up until now. Propecia and Rogaine thickened the hair all over my scalp and slowed the regression of my hairline. Unfortunately, it’s becoming apparent that my crown is now thinning, an area that had been strong prior to treatment.

I’m 28 now, and, if possible, I’d like to keep my crown hair for another few years, what are my options?

1) Would upping propecia dosage ever be advisable for a 28 year old?
2) Should I consider Avodart off-limits until it is FDA approved for hair loss?
3) What would you do if you were in this situation?

I’ve a check up with the doctor (who I’m very happy with) that originally prescribed the treatment in a couple of weeks, and will pose the same questions to him. However, your opinion is really important to me, and will help inform any decisions I may have to make. All the best

Upping the dose of Propecia (finasteride) usually does not work in your situation. Rogaine (minoxidil) is fairly effective in the crown and worth trying. Avodart (dutasteride) is an option and some doctors will prescribe it, however, the long term risks of sterility are there (probably remote), and the side effects are more frequent than on Propecia.

Try the minoxidil and give it at least 6 months. Then reassess your situation.

Is Avodart Making Me Angry?

Since taking propecia for three months then switching over to avodart for a further six months, i seemed to be very agrivated all the time and angry to the point were i was arguing and getting into fights. Is there any connection with this and taking avodart.

Why the switch to Avodart (dutasteride) from Propecia (finasteride)? Propecia is the only FDA approved oral medication for the treatment of hair loss. Avodart is not. If the Propecia was working well for you and had an acceptable side effect profile for you, you should go back to using it. Avodart has not been shown to help with male pattern baldness better than Propecia unless the dose is three to four times higher than recommended dosages and even this information is suspect. Since it may be causing you severe emotional issues, you should strongly consider stopping this medication. We do on rare occasions see personality changes with these drugs. You may also consider talking to your doctor or therapist about your anger issues if in switching, your anger has not subsided.

Is Avodart Going to Be FDA Approved for Treating Hair Loss?

I read someone’s comment in one of your blog entries from long ago, suggesting that Avodart was seeking approval from the FDA (something about being in Stage 2 trials). We know it hasn’t gotten FDA approval, but are they actually seeking it?

There is a report just published two weeks ago on the U.S. government website ClinicalTrials.gov which indicates that phase III clinical trials are in process for Avodart in Korea. That is great news for us, because the indications are strong that Avodart (dutasteride) will be more effective than Propecia for the treatment of male patterned genetic hair loss. The most pointed questions which need to be answered are the frequency of side effects, particularly sexual libido supression.

There are some studies out there proving the efficacy of Avodart (dutasteride) for prevention of hair loss. It is documented that blocking type one 5-alpha reductase may be as important as type two in treatment of baldness. Unfortunately, the study done at Duke University, published in December 2006, is limited to 24 weeks and we don’t have enough information on the long term effects of dutasteride on hair loss.
