My earlier post about smelly hair systems generated a lot of good responses…

I loved yesterday’s question on the smelly hair system. It does not even compare to my problem. I have already found the secrets to keeping it from smelling decent, but that does not deal with the ladies when they want access to my head. I am working the internet dating scene and this problem is killing me. I find myself fearing the intimacy that I fantasize about. Now it is not fantasy, but a pending nightmare each and every time I start up a new relationship. If I do not wear the hair piece, I can not be myself. How does one deal with the no-touch zone that puts the fear of God into the rug wearer when a woman wants to run her fingers through the piece?

When a man embraces a woman, there is a tendency for the woman to want to run her fingers through her man’s hair. This is a deal breaker for hair system wearers and it puts them on-guard every time they establish a new relationship, often impacting behavoir as the wearer tenses up during intimate moments. I have spoken to many men and this is a common problem. The answer is that the rug wearer needs to keep one hand free at all times to fend off the roaming hands of the woman. It does put the kybosh on the freedoms one wants in real intimacy and it excludes such activities as bathing, swimming and the like. I have found that these problems are not addressed well in long term relationships either. I have done hair transplants on men whose wives have told me that they have never seen their husband without his wig on, not even after 30 years of marriage. I am afraid there is no real answer. Every hair system can be felt, even with the best of weaves. The only real answer to this question is a good hair transplant.

Two cases of infertile patients with azoospermia or severe oligospermia who showed significant improvements in sperm concentrations 6 months after the discontinuation of finasteride. In one case, improvement in semen parameters prevented the need for testicular biopsy and corrected the azoospermia. Stopping finasteride in the infertility population may improve semen parameters, and may allow for less invasive fertility treatments.

We have already heard about similar, more frequent side effects with Avodart (dutasteride). While I am not completely surprised, I suspect the frequency is very low. I’ve written about sperm count and Propecia in the past:

Hair Salesman

I went to a hair transplant facility the other day and had a consultation. Most of the time I was there was spent with a salesman. Is this usual? Do they generally get a commission of I sign up.

Your question is a good one. Unfortunately, many doctors in this field do use salesmen as communicators for their patient interactions. Although I have never done this, I do understand the huge costs involved in patient education and communication. The salesmen do perform that role well, but in the ending part of the question, you raised the most important issue, the one which defines whose interests are being weighed during the consultation process. The laws of all states generally forbid the payment of commissions to ‘communicators’, but I have no doubt that most salesmen have incentives to sell the hair transplant process. The doctor, on the other hand, is sworn by oath to put your needs ahead of his financial interests. There is clearly a conflict between selling you the transplant and watching out for your needs. You might judge the doctor by how much of his time you are worth.

We do know that for a few patients, these permanent hairs may not be permanent after all. We don’t understand what you are reporting, but I would suggest that you see your surgeon and ask him/her to look at the donor area microscopically and see if those hairs are miniaturizing. If the donor areas have miniaturized hairs, this could explain what you are seeing.

You have what appears to be one or more small areas of necrosis which, when healed, will produce a scar without hair growth. This is usually a failure of the surgeon’s technique for creating recipient sites.

You have a persistent frontal forelock which goes down to the highest crease on your furrowed brow. I feel strongly that when people have these forelocks or remnants of a low hairline that abuts the highest crease on your furrowed brow, then the transplant should incorporate these elements of the old hairline. If not, you may be stuck looking “not normal” and with a hairline that is seen as a hair transplant, regardless of its quality.
I sat on a jury, and the judge had a terrific hair transplant. I knew it was a hair transplant because it was placed too high and high receding hairlines are never tight. This is wrong for both the patient and the surgeon who performed it. Hairline location is critical on the first pass of any hair transplant process.
From the video, I am assuming that your hairline was placed too high and that is your complaint. If the grafts failed to grow, 99% of such failures are technical failures of the surgical team.

Used Estradiol patch, minoxidil & Bicalutamide in 4 months

The use of Bicalutamide and an Estradiol patch in this treatment plan is an extreme approach for men. The Bicalutamide and Estradiol are not in the Standard of Care so we don’t have a side effect profile on this drug in young men, but we do have side effects when used for prostate cancer. The Bicalutamide is a very strong anti-androgen which almost certainly will bring on ED and sexual dysfunction and the Estradiol is a feminizing hormone which might further impact a man’s sexual function. The minoxidil and dermaroller is standard in this approach. I would not recommend that the average guy go this direction in his treatment despite the remarkable results from these drugs. Do you have good sexual function? Is it worth giving up a sex life for the return of your hair?
