What Do You Think of Pilofocus as a Better Way to Do FUE?

Pilofocus is also called piloscopy. I have seen this presentation by Dr. Wessley, and I am very unimpressed. It seems like a cumbersome procedure that is slow, and the surgeon will not be able to perform any significant number of FUE grafts during the surgery. FUE is not very scarring when performed by the right surgeon with the traditional approach I defined in the literature, so I don’t see a place for piloscopy regardless of any FDA clearance which may or may not occur.

Green Tea to Treat Hair Loss

Hi,
I am currently 24, I have been orphaned since 4 years old. so I have no idea if this problem of mine is linked to genetics. At about 16 years old I notice my hairline receeding, I was kind of ignorent then. About a year ago someone mentioned the (dreaded fact) since then I’ve been looking for alternatives to Medical drugs. I came across an article written about the 5-alpha Reductase inhibitors found in EGCG (green-tea)I was wondering if you know if and when there might be conclusive evidence with American Science, I do hope I don’t come off sounding like a (Jack-ass), but it really scares me to think that especially in (starting my own family )is lost due to the American Standard. Oh yeah on the Norwood Diagram, I am currently Type II.

What should scare you more is letting the hair be lost while you, as an amateur, try to do what hair professionals do every day. You need a diagnosis (even more important for you, because your genetic family line is unknown) and you need your scalp mapped out for miniaturization to find out if you are balding from genetic causes.

As far as green tea is concerned, I frankly do not know. If it really worked, more and more people would be using it and telling me about it. Just something to think about. I am curious, does one drink it or pour it on their head? Ha!

Thinning on Sides and Back of Head

i’ve been using propecia for about 3 yrs now & minoxodil. for the first 2 yrs my hair stopped thinning & stabilized, but just this yr & more specifically the last 4-6 months. but heres the thing, im thinning on the sides & back corners where your usually not supposed to go bald or lose hair. this started right around the time i started using minoxodil & something from hair club for men on the sides cause before that i wasnt, so i stopped doing that, do you have any suggestions, thx for your time

You need to be evaluated by a good doctor that specializes in hair loss. Loss of hair from the sides and back is unusual in men and common in women. There are too many possibilities here to discuss and there is no substitute for a good doctor to evaluate you. You can contact a dermatologist that specializes in hair loss, or has an interest in this field. If I were to guess on your problem, I would have to make a list of possibilities and that may do more to confuse you than to help you.

Thought I Was Going Bald, Then Thought It Was Just a Mature Hairline

Dr. Rassman, I wanted to ask you about how quickly baldness can set in. I’m a 21 year old male and I’ve noticed that my hairline has been receding over the last few months. Originally, I was convinced that I was going bald, but then I figured maybe it was only my hairline maturing (as the hairline is hovering just around 1.5 inches above the highest forehead wrinkle at the temples), but now I think it may be balding again. It has receded almost half an inch at the temples in about a month, so I want to know if I should expect it to continue receding at this rate, assuming it is baldness and not simply a maturing hairline. Thank you.

If you are balding, then you will have miniaturization leading to the balding pattern. Look to map out your scalp for miniaturization, especially in the frontal corners. The difference between a maturing hairline and early frontal balding can be difficult to spot. The onset of a maturing hairline usually takes a few years, but I have seen it start up and move more quickly. There is no rule you can use.

Hair Becoming Minoxidil Dependent

Dr Rassman
I have heard anecdotal reports that the use of Minoxidil for diffuse thinning in men can cause the good terminal hair one currently has to become Minoxidil dependent and change the “good hair” into a fuzzier Minoxidil produced hair—any truth to that? Thank you

Minoxidil dependence does occur for the thinning hair associated with balding in both men and women. When it works, it can be dramatic, but the dependence is a reality that you must face, for if you stop the drug, you will lose all of the benefits including the hair that is preserved. Those men and women with normal hair do not have to worry about minoxidil dependence.

What Is the Time Frame for Regrowth with Finasteride? (from Reddit)

Finasteride takes months to acquire the real benefits. The hair loss may slow or stop quickly, but its real benefits, if they occur, will take at least a year. If you are fortunate to find a doctor who has a HAIRCHECK instrument, he can report back to you what the ACTUAL benefits of the drug are on your hair between measurements (generally a year).

Hair Density in Scars

How many hairs can you transplant into a square centimeter? I have a scar, and I want to fill it in. I am trying to find a doctor who can fill it in with just one surgery. Is this a reasonable question?

An excellent question! The calculation is one that is used in the industry as a benchmark measurement. The average person with 100,000 hairs on his head has 1250 hairs per square inch or 193 hairs per square centimeter.

But what is the number to use in determining the correct density for a hair transplant? Some doctors use high density numbers to define how good they are. However, there is a balancing act between maximizing density at the time of a transplant surgery and maximizing growth months later. Experienced doctors are the key.

The reasonable upper limit of density numbers from a transplant perspective is up to 120 hairs per square cm. This type of density, however, requires supple skin that still has elastic properties. Scars do not have this characteristic. Also scars have an abnormal blood supply when compared with normal skin. Much of the infrastructure and microcirculation in the vasculature is not present in scar, so transplanting very high densities may not produce good growth.

You need to be examined by a hair transplant expert, who has the experience to give you a reasonable answer to your specific need. While I understand that you would like to have this completed in one surgery, that may not be in your best interest. Only your doctor will be able to advise you.

Tonsillectomy and Hair Loss

I am a class 3A Norwood. (I saw Dr Pak, he’s great by the way.) About 3 months after a fairly invasive surgery, my hair started to fall out and did not stop until recently. I have just been told I need to have my tonsils surgically removed, but it is not an emergency. I am currently taking Propecia and have done so for a year. Should I expect more of the same 3 months after the tonsil surgery? Would there be any benefit to waiting on the surgery or having it sooner? Thank you.

Usually shock hair loss only occurs once, but while it is unlikely, a repeat bout of hair loss is possible. Take care of your medical problem and do not focus on your hair. If you need to have this other surgery, then do it.

Hair Grew Back Wavy After I Stopped Shaving My Head

Hey Dr. Rassman..My question might be a little stupid but I’m hoping you could help me out.. I’ve been on propecia for about 2 1/2 yrs now (I’m 26 yrs old).. During most of that time I shaved my head.. I grew my hair out recently and it came back very wavy. Since then I’ve noticed a large amount of hair shedding.. However the shedding is limited to only when I shower. Between washing my hair (40-50), towel drying it (10-12), and combing it (15), I see almost 80 hairs a day.. After that I put in alot of hair spray and gel and dont see a single hair fall out. Not on my pillow, not on my desk.. nothing.. Is the large number of hairs I’m seeing in the shower ones that just couldn’t come out randomly during the day? My hair is still fairly thick so I wasnt sure if they get caught in my wavy hair..

thanks for the help..

It is normal to lose about 100 to 200 hairs a day. You may also benefit from following up with a doctor who can map your scalp hair for miniaturization to keep a quantitative record of your hair loss pattern as you continue your Propecia. With your hair short for many years, the hair loss you are now experiencing may have been happening with the short hairs such that you never noticed the fallout. The rougher you are with your longer hair, the more hair you may experience. Be gentle with the hair washing as hair that is miniaturized (impacted by genetic balding) is more far more fragile than thinning hair.

With regard to the change in hair character from straight to wavy, it is not unusual for hair character to change as we get older or as genetic balding starts to kick in. Stay on the Propecia.
