After 10 Years on Propecia, I Finally Started Shedding
Dr, I have been taking Propecia for 10 years. If the drug would indeed stop working, what exactly would happen? Would I have some form of telogen shed? Or would the loss be gradual? The reason that I ask is because I am going through a severe shedding phase that began in September of 2008 and it continues until now. I have indeed ordered the “generic” drug from overseas the past couple of years. Could the overseas drugs be bogus and this is the reason I began shedding again?
There’s two possible problems — the generic you bought overseas could be fake, or your finasteride dosage could just be starting to finally lose the fight to your genes. I often talk about a tug of war of sorts, where Propecia works on one side of the rope and your genes work against it. The genetic side appears to be slowly winning. Talk to your doctor about increasing your dosage to 2mg daily and see if that helps.
Whatever you do, do not stop the drug! You’ve taken the medication successfully for 10 years now, and it hasn’t stopped working for you… but if it’s not due to fake overseas medication, then it’s not slowing the hair loss as well as it was before. Stopping it completely will likely result in a massive catch-up hair loss situation occurring.

After Stopping Propecia Does Hair Loss Get More Severe?
Dear Dr. Rassman,
I have a question regarding Propecia side affects on the hair (!) if you stop taking it. If one stops using it after a long period (months or years), are there any chances the hair loss would increase? Meaning, if one took it for, lets say, one year and his body got used to the drug, can the hairloss get worst when he stops using it? I know of course that when one stops using propecia the shedding returns, but could it be more severe than if he had not taken the drug for the same period of time before he stopped?

If you stop using Propecia, you may experience “catch up” hair loss. You will in effect lose all the hairs that grew from Propecia and lose all the hairs that would have been lost if you never took the medication in the first place. I have personally seen such cases and they can be pretty dramatic. The hairs that are lost will not likely come back if you re-start Propecia. I don’t know of any evidence showing that upon stopping Propecia, your hair loss even more severe than it would’ve been.

I have known that this is a side effect of finasteride that few people talk about. Most men who get a decreased libido complain (rightfully) about the drug but when the libido goes up (something I hear about from some patients) few complain on the internet about it. Locating a proper hairline!
Why with eyebrows raised up? Is that how the rule of 3rds supposed to work? Distance from bottom of nose to eyebrows line should be the same as the distance from eyebrows line to hairline? But that should be properly done with eyebrows lifted high?
The hairline ends at the frontalis muscle when you are 5 years old. Ask a 5 year old to lift their eyebrows and you will see their upper middle hairline touch the highest crease of their furrowed brow. The juvenile hairline is always located there. and as a male matures his hairline without balding, it will go up one finger breadth above that highest crease and change its shape from concave to convex taking in a V-Shape. That is why I I always ask my patients to raise their eyebrows, to determine the proper location of the hairline.
Alopecia Areata?
I went to the dermatologist to talk about my hairloss. Due to the how fast I lost my hair (almost 70% density in a year), the doctor did a hair biopsy and apparently the results show Alopecia Areata. I have been going back to the dermatologist once a month for the past 2 months for cortisone shots and have not seen any results. I am a bit skeptical about the Alopecia Areata diagnosis, mainly because their is typically a circular bald pattern associated with it and my hairloss isn’t like that.
The picture you show of the back of your scalp is typical of alopecia areata; however, the quality is poor and out of focus. With good photos, one should see on close examination Exclamation Mark hairs in and around the hair loss areas. This is sometimes a difficult problem to reverse. Because the quality of the photos are so poor, Don’t hang your hat on the diagnosis I gave you above until you either send me better photos or see a dermatologist
