I understand that using a sauna can remove drug residues from the body. In view of this, my question is can you use a sauna while on propecia? or will it limit the effectiveness of propecia itself?

Hair Transplant Rejection?

Is is possible for transplanted grafts to be rejected months or years after a successful HT? If so how often and to what extent does that happen?

For men, the hair on the back of the scalp is considered permanent. It may thin a bit with aging, but even old men in their 80s, 90s, 100s will have hair at the back of their heads. So if you harvest the hairs from this permanent area, the transplanted hairs will act like the hairs on the back of the head and will have the same life span. This concept is called “donor dominance”. This is why hair transplants work for men. For women, it may be different and there is a small risk the transplanted hairs may not be as permanent as men.

Theoretically it is not possible for successfully transplanted hairs to be “rejected” years or months after it has grown, but I have seen a handful of patients over the last 20 years who have had transplanted hairs fall out. This may be due to a diagnosed condition such as DUPA (diffuse unpatterned alopecia) or other esoteric medical conditions.

Cleanliness is close to Godliness. I can’t comment too much about this tragic report; however, the surgery was apparently performed by a non-physician team, possibly without respect for the cleanliness required of a surgical procedure. Using Rogaine for Frontal Hair Loss

Is Rogaine only supposed to treat hair loss at the crown or can it be used for temple and frontal balding as well? If it is not meant for frontal balding, are there any products or drugs that do help with frontal balding?

I am not sure what you are treating. Do you have male pattern balding? You could have hair loss from other causes. Rogaine (minoxidil) can be used for temples and the frontal area, but it is highly unlikely to regrow hair there. Propecia works best in the crown, but it may be more effective than monoxidil in the frontal area in some people.

Hair Transplants on people with fine hair?

So I am 31/M and have been considering a hair transplant. Not going to get into the details too much but every site I have seen the ‘success’ photos are dudes with major black and thick hairs and any that I have seen that would be considered thin follicle hair just look like they have added on a couple more years before you have to shave it anyway.

So I am asking is there anyone here who has what you would call ‘thin’ hair had a successful hair transplant that doesn’t look completely terrible?

I use a construction metaphor for explaining fine hair vs a coarser hair. Think of fine has as lumbar 1 by 1, and medium hair as lumbar 2 by 2 and coarse hair as lumber of 4 by 4. If you were to build a fence, you would opt for 4 by 4 first for the fence as there is more wood in a 4 by 4 than a one by one. Fine hair reflects a one by one measurement of lumber and a fence made of 1 by 1’s take more 1 by 1’s than if you had 4 by 4 wood boards. A hair transplant with fine hair means that less hair volume will be moved per hair shaft transplanted. Men with fine hair always had fine hair, so that they are not surprised when then get a hair transplant with their own fine hair. I just don’t want a new patient to expect that they will get the results of a man with coarse hair if they get a hair transplant with me. Setting realistic expectation is what it is all about.

I’ve been taking propecia since I first noticed my hairloss, at age 20. Now I’m almost 24, and while I’ve retained some hair in the vertex, there’s been substantially more loss in the front/temple areas. Transplants are probably not a financially plausible option for me presently, so I’ve weighed other solutions (mostly vitamin/herbal supplements), bearing in mind the advice published in your blog. I do not like the Hair Genesis site, because it is very heavy on claims that appear unsubstantiated. As I have no experience with it, I can not tell you what I do not know or have not learned, but when there are so many claims and reflections on FDA ‘endorsements’, I would doubt their credibility. The field of homeopathic and herbal medications is generally unregulated, but it is not proper or ethical to claim FDA ‘endorsements’ like this company seems to have done. To my knowlege, only minoxidil and Propecia have been FDA approved for safe and effective hair loss treatment and there is good scientific evidence that minoxidil and Propecia works. When it comes to natural and herbal medications, there are many harmful natural and herbal medications. Arsenic, of example, is a natural medication that has been used in historical medicine for the treatment of syphilis, but I would not use it for prevention of syphilis nor for any human use as it is a well known poison, so ‘natural’ does not imply safety. The message here is BUYER BEWARE, and remember that just because claims are posted on a website, it does not imply true safety or effectiveness.

Very Thin Hairs on My Comb

Recently I’ve noticed short hairs that are also extremely and unusually thin appearing on my comb after I comb my hair. I only see them on occasion (small minority of my hairs), but nontheless they are there. Does this indicate that the miniturization process has started to some extent, or do normal hairs sometimes go through thin/short phases only to return to normal eventually?

I would want to know if you are male or female and if you are having signs of balding present. What is your age?

People with miniaturized hair will find the hair to be more fragile and easier to be pulled out with combing and brushing. Large-toothed brushes are probably worth using as they do not place the full power of the ‘pull’ on each hair. If you are concerned about miniaturization, get your scalp mapped out for it and you will know for sure.

From time to time we treat patients that will then refer other family members. Both the man below and his brother had hair transplants, and they’ve both been just delightful. Jokes and humor abound during the surgeries and the follow-up visits. Such a good natured, fun person to be around. It took 4 surgeries spaced over a few years, but he met his hair restoration goals and is excited about the results. I am too!

I should’ve explained the case better in the initial post, so I’ll attempt to do that here.

The before photos that are posted above actually reflect far less hair than the pictures indicated. The patient’s hair is fine, so each hair shaft has less bulk than an average person with medium weight hair. The hair thinning went back some 5 inches beyond the leading forelock shown in the picture, but I originally decided that just advancing the hairline would suffice without going back to the thinning area. The entire forelock disappeared after the first procedure and he had more hair loss behind the area that was originally transplanted, so the second procedure was done to thicken up much of the forelock area which was lost.

The original plan that was created brought the hairline down considerably. It took two procedures (3288 grafts total) to get him what he wanted and to fill the area behind the site of the original transplant. For the patient that is taking the drug finasteride, rarely some additional hair loss may occur after the first transplant, as was the case here.

He is a bit of a perfectionist and the two other smaller procedures (553 grafts and 698 grafts over a year apart) were done to deal with some more hair loss behind the transplanted hair. He also wanted to put some more grafts in areas where he thought additional hair would help.
