Yes, I have done this many times; however, I require a good understanding of how much miniaturization is present to help my patients understand what might happen to them after a hair transplant if anything. The older the man is, the less miniaturization is usually present, but that is not an absolute rule.

I often tell my patients to check their sperm count before they start taking finasteride and then after a few months later if they plan on having children in the future. We know finasteride drops sperm count so it is something that men should know if they take this drug.

I’ve been on finasteride for 3.5 years. Without it l would be fully bald by now (25) and it’s allowed me to keep a good amount of hair. Generally doesn’t look like l am suffering from hair loss. A bit thinner on the top but decent overall thickness.

I’ve had enough of taking the drug. I have come off it several times during those 3.5 years and l feel remarkably better off it. I feel more significantly more virile and energetic. When l take Fin my libido drops, brain is foggier and l just don’t feel as good. My face is rounder and less defined as well.

Are there alternatives to Fin that work?

Oral minoxidil is an alternative but considering that you have excellent results from finasteride, why mess with the results and risk loss?

 

Is There a Way to Tell FUE Success After Just a Week or Month?

Hi Doctors, great blog here! It’s an amazing cross-reference and informative place.

Just wanted to know if there are any signs of a successful FUE transplant? (other than growth of hair at 8-10 months!!).

Can the patient tell within a week, or a month, whether the surgery he had would be, say, at least 80-90% successful? Are there any signs which suggest that yes the transplant was a success, and that the roots did not dry out, and were placed correctly etc.? ( I’m guessing, that if the transplanted hair kept growing till it fell out after 3/4 weeks, that means the roots were alive and firmly in place, and the surgery was good? )

I only ask about FUE, because it seems the more delicate procedure with a higher trans-section rate.

I know the anxiety you have. Everyone with a hair transplant wants to accelerate the clock, but it makes no real difference if the transplant is FUE or strip. In either case, you will start seeing the results (at least some results) somewhere between 4-6 months, with good growth by the 8th month. At one month, the hair sheds in probably 19 out of 20 people. Just be patient.

Is there any way whatsoever to determine whether a person will at some point in their lives suffer from male pattern baldness, and if so, would this not eliminate the problem of transplanting people too young (say if they have a naturally high/ uneven hairline etc)

There is a test called HairDX that will tell you if you have the gene (70% accurate), but it won’t tell you when that gene will be expressed… or if it will be expressed at all. So on a predictive basis, the answer is unfortunately no. But if you want to know if you are balding (possibly just a little) then you should see a doctor who is experienced in hair analysis for miniaturization and hair bulk, which this will tell you if the balding process has started in you.

Many young men with a balding dad or uncle or brother are expecting hair loss to happen to them, and these are the patients I can help by telling them what, if anything, is going on in their head. If you have an average hair thickness, you might not see thinning until the hair loss is at 50% of the original density. Coarser haired men will often not detect loss even at 60-70% of their original density and blond men may not detect hair loss or thinning to 80-90% of the hair due to the low contrast between hair and skin color.

Is There a Urine Analysis That Could Show Possible Causes for Hair Loss?

I want to rule out other possible causes for my hair miniaturization to ensure that it is definitely genetic since I am not aware of any bald men in my family. My iron and thyroid levels are fine. I read that a urine analysis will evaluate my nutritional state. Is there anything else I should get tested? Like hormones for example?

In general, genetic hair loss in men shows up as a specific pattern (hence the name “male pattern baldness”). That is why there are diagrams and pictures (Norwood) to show what these patterns look like. A fairly competent doctor can determine what genetic male balding is by merely looking at the person. If the hair loss is relatively early in the process you can look at the hair under a microscope to see if there is a miniaturization pattern or a bulk measurement that can give tell you if you’re thinning. You don’t need a blood or urine test for this.

Other types of hair loss, such as from low thyroid or anemia, will present as diffuse thinning with no specific pattern. There is a hormone test to check your thyroid levels, but there is no hormone test for genetic male pattern balding. This can be confusing because a hormone called DHT (dihydrotestosterone) is the cause for hair loss in men, but only if they have the genes for MPB. The quantity or levels of DHT will not determine if you will bald since the underlying issue is genes.

There is a genetic test for balding, but it is not a straight forward test to interpret since it only give you a probability score. The easiest way to test for balding is a good old fashioned physical exam. Finally, I must point out that the gene for balding can skip generations and there is no specific rule on how it is passed down.

I’m 21, had a transplant when i was 19 for the hair line of 2000 grafts but my main problem was density all over. i think i have good density. i opted for a hair piece which i’m really satisfied with it, but i know it has a short timeline so i’m considering a transplant again

You may have serious problems if you had a 2000 graft transplant at the age of 19. I generally look at 19 years old (or even 21 in your case) as a person who is not a candidate for a hair transplant. You need a Master Plan by a competent and caring doctor. The doctor you had is suspicious if he is doing 2000 graft cases on 19 year olds. I would guess that the surgery did not work well, so the hair piece addressed that issue.

Your question on how many transplant procedures are needed for a Class 6 balding pattern does not reflect an understanding of supply and demand (please read this post).

Right now I am 20 years old, full hair norwood 0, but after finding out about this community I can’t stop thinking that I will go bald. My father is bald, but my mom’s side has no hair loss. Is there some test that a doctor can do on me that can determine whether I got my father’s balding gene? Please I need an answer i am getting too much anxiety from thinking about this.

No effective test is available at this time. I tell my patients to look carefully at their family history and find out from those who are balding, what age they started to see hair loss

What science there is relates to wound healing. When wounds occur, the body’s response it to mobilize a very aggressive healing process with the production of cytokines (a type of protein) that invokes the immune system and migration of healing cells. This may cause enough stimulation to invoke a hair response to the healing process as well. When this is done with the drug Minoxidil on board, it brings the effects of the minoxidil more to stimulate local absorption. We seem to read article suggesting that minoxidil with dermarollers, seem to work. That is my best guess.
